2 Day by day, Presence Health works to ensure that everyone has access to the right care in the right place at the right time. It s a tall order and a complex task, but we are committed to connecting people with the care, experts, resources and education they need to be their healthiest, no matter where they live or what their income. We are passionate about creating an inclusive culture of health that transforms lives and strengthens communities. This report is brimming with evidence of our efforts programs offered, dollars spent, people served, number of employees, doctors and sites of care. But it s the individuals who are impacted by our care, not the numbers, that matter most. 3 Access to Care Obesity Picturing a Healthier Community Take a look at some of the ways we worked with individuals and community partners to tackle our top four health priorities. Mental Health Chronic Disease 58 people trained as Certified Application Counselors to assist thousands of residents understand and enroll for health coverage. Irma R. reduced her total cholesterol by 47 points and relieved symptoms of depression with the help of a chronic disease management program. We offered 189 community education events from free screenings and exercise classes to health fairs and physician lectures. Carlos G. When he lost his job and health insurance, we helped Carlos G. obtain health care coverage through the Marketplace.
4 8,262 people reached out We collaborated with community for help and called our 433 Behavioral Health Crisis Line. to address the highest priority local health needs. partners After participating in a diabetes self-management program, Eugene K. lowered his A1C from 7.9 to 6.6 and his blood sugar average from 180 to action teams worked directly in our communities to place resources where they are most needed. 2,796 people participated in our programs targeting obesity. Our vision of a healthy community. A place where people are connected and everyone feels comfortable, safe and protected. Where people support each other and come together to make life better for themselves, their family and their neighbors. There is a focus on being and staying healthy. And where everyone has access to affordable health care when they need it.
5 Making an impact with over $550,000 a day in community benefits. In 2013, Presence Health s contributions added up to over $199 million to improve the health of our communities. $65.2 million Financial Assistance (at cost) The cost of providing free or discounted health services for those who could not afford to pay and met the criteria for financial assistance. $6.8 million Community Health Improvement The cost of providing activities and programs aimed at improving community health, including assessing community needs and program administration. $30.3 million Health Professions Education The cost of educating future medical professionals in programs that result in a degree, certificate or license in a health care field. $18.4 million Subsidized Health Services The cost of providing clinical services despite the financial loss to Presence Health. $1.3 million Research The expense of conducting research studies and publicizing the results to increase the health knowledge of community members. $3.2 million Cash and In-Kind Contributions The value of contributions and services provided to other community organizations. $73.9 million Medicaid (at cost) The unpaid costs of public programs for those with low income or a disability. These calculations are based on Internal Revenue Service Form 990, Schedule H. Additional benefits provided by Presence Health include $208.5 million in bad debt expense and $132.2 million in Medicare shortfall.
