O = Observation A = Audit RD = Return Demonstration JOB SPECIFIC RESPONSIBILITIES WT Written Test CS Case study S Simulation, Scenario INITIAL OF Participates in data collection and analysis for ongoing performance improvement and maintains VAD implant database. Obtains INTERMACS and all IRB approved patient consents, performs protocol psychological testing, and enters the information on INTERMACS database on a timely manner. Focuses on customer service and works proactively with the VAD and transplant teams emphasizing cooperation and communication with VAD candidate patients. Page 1 of 5

2 Provides age appropriate care to all patient populations for which staff member is responsible and trained for. Continuously collects and tabulates VAD program data and presents to QA/QI teams information regarding performance measurements and performance improvement plans. Is able to evaluate patients and start the process for possible VAD patient candidacy following the Inclusion and Exclusion Criteria Evaluation Guidelines. Is able to communicate with Finance Department, in a timely manner, regarding possible candidates for VAD implantation and assists /collaborates with insurance approval / authorization process. Is able to communicate with social worker and other multidisciplinary team members and collaborates in the VAD approval or denial decision making. Is able to effectively communicate approval or denial to the patient and referring physician in a timely manner. Is able to define theory and operation of each VAD system, components, VAD parameters, alarms, troubleshooting malfunctions, and overall operation of each VAD system: HeartMate XVE LVAD HeartMate II LVAD Thoratec PVAD/IVAD VentrAssist LVAD Levacor LVAD Jarvik 2000 LVAD HeartWare LVAD CentriMag VAD Abiomed Impella Abiomed AB5000 Abiomed BVS VAD KNOWLEDGE AND SKILLS RESPONSIBILITIES Is able to describe all potential risks unique to the patients with VADs. Is able to discuss and describe in detail appropriate steps to follow when responding to critical VAD alarms or system malfunctions. Is able to prepare and plan appropriate teaching and education regarding the VAD system to the patient, family members, and care givers prior to VAD implant. Page 2 of 5

3 Is able to enumerate and list the risks and benefits of the VAD system, and explain them to the VAD candidate patients, their families, and / or care givers. Is able to set up and plan the VAD patient s hospital discharge including contacting Emergency First Responders, hospital s Emergency Department, Primary Care physician, Power Companies, and provides information and training as well as establishing a plan of action in case of system malfunction Is able to independently conduct daily rounds and perform a full physical assessment on all the hospitalized patients with VADs, write daily patient care notes, and discusses plan of care with attending surgeon, cardiology team, and multidisciplinary team; ordering tests as appropriate and necessary. Is able to independently see VAD implanted patients in outpatient clinic, interpret labs, perform full assessment, evaluate pump function, dictate or write clinic note, and discuss findings with attending; ordering tests as appropriate and necessary. Is able to enumerate and describe equipment needed at home, for the VAD implanted patient, upon discharges. Is able to list the diagnostic and radiologic tests the VAD patient can and cannot have and list reason(s) why. Is able to define VAD patient s recommended activities and restrictions. Is able to list VAD patient s proper anticoagulation and rationales for its / their use including communication with Anticoagulation / Thrombosis Teams. Is able to demonstrate proper technique / steps for connecting the patient to their controllers, batteries, power modules and troubleshooting equipment issues. Is able to troubleshoot all alarms and explain to the patient and caregivers the steps to take in an emergency. Is able to explain what happens if the VAD stops functioning, and demonstrates appropriate trouble shooting. Is able to set up and conduct a sterile driveling dressing change for each device including teaching caregivers how to complete dressing change. Is able to provide education and verification of skills / understanding to the patient, family members, and / or care givers regarding the VAD function, operation, and troubleshooting. Is able to grade the skills test and provide positive / constructive feedback to the patient, family members and / or caregivers. Page 3 of 5

4 Is able to demonstrate appropriately and independently how to set up and prepare the VAD pump prior to surgical implantation as applicable or instruct others to do so as well. Is able to check, set up, and trouble shoot and monitor all external VAD equipment, controller and monitors in the operating room and transfer safely to the ICU.
