1 Salud! Services Report Fiscal Year 212 october 1, september 3, 212 The major accomplishments of the Salud! Program in 212 include: 234 vineyards, wineries, and contractors participated in the Salud! program. 4,135 individuals were registered in the program, representing an overall increase of 13% in program participation since 211. A total of $167,873 was provided in Grants for Care for 3,294 encounters to help Salud! clients with healthcare expenses. There was a 6% increase in the number of Grants for Care provided to Salud! clients from 211 to individuals participated in the Salud! Services mobile wellness outreach program, receiving screenings for cholesterol, blood glucose, blood pressure, and body mass index. 214 Salud! clients received $28,6 worth of services at mobile dental clinics at no cost to the individual. Female vineyard workers utilize the Salud! Grant for Care process more frequently than males. While women make up only about 2% of the primary worker population, they received about 41% of the Grants for Care given in the 212 Fiscal Year. Salud! offers an increasing number of services to family members of vineyard works. In 212, about half of all grants for care provided went to vineyard workers, the other half going to spouses, children, and other family members of vineyard workers. Children made up about one-third of the Salud! population in 212. However, they received only about 12% percent of the total grants for care given this fiscal year. Salud! Services attributes much of this to the high number of children that have Oregon Health Plan health insurance. Salud! Services works with many community partners to assist vineyard workers and their families in gaining access to valuable resources in their communities: 2 Salud! Clients received High School Equivalency (GED) program information from Portland Community College Rock Creek, and seven individuals are now enrolled and testing for their GED. Approximately 5 families received information on qualifying and registering their children into the Healthy Kids program provided by the Centro Cultural of Washington County. Migrant Education provided countyspecific information about their program to 15 individuals.

Grants for Care and may not reflect discounts given by providers or the full amount paid by clients. WHAT DOES A GRANT FOR CARE PURCHASE? Each time Salud! Services provides a Grant for Care (GFC), the financial burden of healthcare services to the client is reduced. In addition to monetary assistance, Salud! GFCs also provide education, empowerment, and access to care for the vineyard workers. $25 Diagnostic Services - Assists with the financial burden of laboratory cost to aid in the diagnosis of patient illness. $4 Primary Care Services - Assists with the financial cost of primary care services, prenatal care, well baby child exams, facilitates access to already overbooked Federally Qualified Health Centers, and encourages regular care from primary care clinicians. $6 Dental Services - Facilitates access to and assists in the financial burden of dental care at Federally Qualified Health Centers, private providers, and emergency dental situations, and promotes regular preventative dental care. $15 Vision Services - Facilitates access to and assists with the financial burden of regular, preventative vision care and vision hardware. $6 Mobile Dental Clinics - Pays for one mobile dental services clinic to reach Salud! clients at their place of work or nearby, and provides dental care procedures for 1 to 15 individuals, often resulting in diagnosis of dental issues, treatment, or referrals to other clinics. Varied Amounts Other Services - Provides financial assistance to Salud! clients for major medical, dental, and prenatal needs, empowers Salud! clients to take an active role in their treatment, and allows Salud! staff to advocate for clients in the healthcare setting. 2

As the Salud! client base continues to grow and age, the need for services is expected to expand in the years to come. 5 WELLNESS CLINIC AND SERVICES SUMMARY Each year between the months of May and September, the Salud! team offers clinical outreach services providing workplace health screenings for seasonal vineyard workers and their families. These wellness clinics have grown in scope over the years, and now offer vision screenings, dental cleanings and exams, and physical therapy consults through a partnership with Pacific University. STAFF RESPONSIBILITIES Health education and outreach at wellness clinics and health fairs throughout multiple counties. Early intervention and prevention through health screening services. Influenza and tetanus vaccinations. Access to and delivery of primary care and other healthcare services including major medical, prenatal, dental, and vision. Case management to families and individuals by providing education, guidance, and support to clients, and advocating for client needs within the healthcare system. Occupational health and safety workshops including CPR and first aid certification. Establish and maintain working relationships with private healthcare providers to facilitate access to care, particularly in counties where community health centers are not available or services are not provided. Promote proactive health behaviors such as prenatal care, routine dental examinations and cleanings, and annual women s health exams. Teach classes for chronic disease management (Tomando Control de Su Salud). Maintain consistent records of client participation and utilization of services. Ongoing communication and customer service. Participation and collaboration within committees and other partners to improve minority health in Oregon. 5
