1 WELCOME STUDENTS! This packet provides material to assist you in preparing for the Great Oaks training programs at Health Professions Academy (HPA), and Scarlet Oaks Career Campus (SC). All documentation and payments must be submitted to the HPA office on Central Parkway. Carefully review the follow program requirements. Items with an asterisk may have a cost. 2 Program Requirements Detailed Instructions 1. Attend an information session at HPA Information sessions are offered most Tuesdays at 11:00 a.m. For further information and to register for an information session please contact our office at BCI & I and FBI background check (within one year)* Complete BCI & I and FBI background checks within one year of class start date. Bring a valid driver s license or state ID and Social Security card. (Please use nursing code # ) Allow 4 to 6 weeks for background check processing. After submitting your information, you will receive a username and password via , along with instructions on how to log in and use KeyTrain/Career Ready 101. Expect to receive this information within 5 business days. Ohio Means Jobs For FREE practice tests visit jobseeker.ohiomeansjobs.monster.com, click on the Backpack icon at the bottom of the page and create an account. Once your account is created choose Plan It from the menu at the top of the page and then choose Assessments and Training. Launch a test in each of the following areas: WorkKeys Applied Math Practice Test 1 WorkKeys Locating Information Practice Test 1 WorkKeys Reading for Information Practice Test 1 Call one of the testing sites on the next page, when you have achieved at least a level (4) on each practice assessment and feel that you are ready to schedule the WorkKeys. 3 PROGRAM REQUIREMENTS DETAILED INSTRUCTIONS (Continued) Testing Sites for WorkKeys Arrive 30 minutes before test time and bring a valid driver s license or state ID (required) The test length is 55 minutes (for each of the three assessments) The $50 cost will be collected at the time of scheduling (There are no refunds available for WorkKeys registration. If you need to change your test date and you contact us at at least 2 hours before your scheduled testing, we are able to offer a 1 time transfer to a future test date. If we don t hear from you at least 2 hours before your scheduled test date, we are unable to offer a transfer) Achieve a level (4) on each of the three assessments Each testing site also offers free test preparation classes on site. Physical (within one year)* Obtain a physical. Have your doctor or nurse practitioner complete the enclosed physical form. An alternative physical form may be accepted if you obtained the physical within one year of class start date and the form is signed and dated by your doctor or nurse practitioner. 5. Two-Step PPD (TB) Test: (within one year)* TB test must be completed within one year of, and prior to class start date. TB tests may be obtained from your doctor, clinic OR the Hamilton County Public Health Tuberculosis Control Clinic: (513) , 184 East McMillan Street, Cincinnati, Ohio, Call for hours of operation and cost of tests. 4 PROGRAM REQUIREMENTS DETAILED INSTRUCTIONS (Continued) 6. Copy of high school diploma or High School Equivalency (HSE) Submit a copy of your high school diploma or High School Equivalency (HSE). If you do not have a copy of your diploma, a transcript or statement printed on school letterhead stating your graduation date is acceptable. 7. Picture I.D. U.S. Government issued, non-expired, signed, photo I.D. (Examples: driver s license, signed military I.D., conceal carry license, passport.) 8. Social Security card Original social security card; no copies will be accepted. 9. Uniforms for classes and clinical training* A regulation uniform consists of scrubs (any color or print - no cartoon prints) which must be worn to all classes and clinical training. A plain white T-shirt or long sleeve shirt can be worn under uniform top if so desired. A white lab coat or white sweater only may be worn over scrubs if needed at the clinical site. Uniforms must be clean, ironed and wrinkle-free. No hats, caps or scarves are to be worn in the classrooms or clinical sites. Shoes should be clean and in good repair, flat, non-skid, closed toe and closed heel. Color preference is white or black. 10. Basic computer knowledge Students must have basic understanding of computer use address Please provide us with an address. 7 Great Oaks Health Professions Academy Medical Verification Form To the Health Care Professional: PLEASE READ I have, this day, given (print name) a thorough physical examination and based on my findings, which include medical history and physical examination; I believe he/she is physically and mentally able to undertake the Nursing Assistant or Patient Care Assistant Programs at Great Oaks Health Professions Academy. The student is in good health. He/she is free of any communicable disease, can lift 50lbs, and has no known deficits that would interfere with the ability to participate in a clinical setting. ************************************************************************************************************ It is essential that nursing students be able to perform a number of physical activities in the clinical portion of the program. At a minimum, students will be required to lift patients, stand for several hours at a time and perform bending activities. The clinical nursing experience also places students under considerable mental and emotional stress as they undertake responsibilities and duties affecting patients lives. 8 Great Oaks Health Professions Academy TB Form Documentation of a TWO-STEP TB TEST FORM Two-Step TB skin test, QuantiFERON TB Gold, or chest x-ray is REQUIRED for this program. Please include results below or attach results to this form. Documentation of a Two-Step TB (Tuberculosis) Skin Test consists of an initial TB skin test, and a boosted TB Skin test 1-3 weeks apart. If you have a positive skin test, provide documentation of a negative chest x-ray within the last five years (Please attach the chest X-ray documentation to this paper if there is a positive reading). 9 Great Oaks Health Professions Academy Program Policies We are Great Oaks the first choice in providing innovative career training to empower individuals and communities. The Health Professions Academy s goal for you is to be excellent caregivers and to grow professionally and personally. Because of this, students are required to dress and conduct themselves in an appropriate, professional manner for effective school and job performance. Appropriate behavior, appearance, health, and safety contribute to your success. Evaluation Methods: There will be written tests, as well as an evaluation of your caregiving skills. Written Tests the passing grade is 80%. There will be an opportunity to re-test if needed. No grade higher than 80% will be given on a retake exam. A 100% is required on all skills listed on the Performance Log. Clinicals are pass/fail. All requirements must be completed by the end of the scheduled date of the course. A certificate will only be awarded to those who meet all program requirements. We make every effort to assist you in your success. Attendance: Attendance policies are modeled after the workplace, where we hope students will become employed and/or continue to be employed. AS MANDATED BY THE DEPARTMENT OF HEALTH, PERFECT ATTENDANCE IS REQUIRED IN ALL CLASSES, LABS AND CLINICAL ACTIVITIES. Students missing an entire class or more than eight (8) cumulative hours may be required to withdraw. All hours must be made up within sixty days of the last day student attended class. If not made up in this time span, the student must take the entire course over. In the event of unavoidable absence, tardiness, or early departure (due to hardship), the student is required to give prior notification to Program Coordinator, Julia Ancona at (513) no later than 30 minutes before his/her arrival or before leaving the site. Leave a message on the answering machine if necessary. Students must attend and satisfactorily complete all required work, including skills in order to go to clinical. Certificates will not be issued if the student misses any part of the program. A student s ability to make up missed time is not guaranteed. HPA (at Ohio Means Jobs) parking is beside the Center, both upper and lower decks. Scarlet Oaks parking is available on the right side of the building (beyond the bus lane) by the fire tower. Inclement weather: Please listen to any of the major radio or TV stations for information on school delays or closings. Students are expected to attend clinical training on time - even in inclement weather. HPA (at OhioMeansJobs) follows the University of Cincinnati delay and/or closing schedule. Scarlet Oaks program follows the Scarlet Oaks delay and/or closing schedule. Clinical Attendance & Expectations: All clinicals are mandatory. Students arriving late will not be admitted to clinical. Clinicals are pass/fail, and a failing clinical grade will result in an F for the class regardless of other scores. Students are expected to be at the clinical site in uniform, on time (even in inclement weather) and to remain for all of the assigned hours. Students are not permitted to leave early, come in late, or leave the site without permission of the Great Oaks Health Professions Academy program coordinator or the clinical supervisor. Students are expected to exhibit professional behavior at all times. Cell phones are not permitted at clinical training or during the state test. Smoking is prohibited on the clinical site property. No eating or drinking on clinical units. Students must follow the policies of the facility when on clinical. 10 Dress Code: Students are required to dress and conduct themselves in an appropriate, professional manner for effective school and job performance. Facial hair including beards, sideburns, and mustaches shall be clean and neatly trimmed. Students without beards must be clean shaven. Hair must be clean, combed and neat. Extreme hairstyles or unnatural hair color (i.e., blue, green, etc.) is not appropriate in a professional work setting. Hair that is shoulder length or longer must be secured so as not to make contact with patients or equipment. No perfume, cologne or aftershave is to be worn in classes or at the clinical site. No artificial nails, nail art, acrylic nails, etc. Fingernails are to be clean and trimmed so that they are no longer than a ¼ inch past the tip of the finger. Clear polish, if worn, must be in good repair without cracks or chips. Jewelry is limited to a plain wedding band, post earrings (no more than three earrings per ear and no hoop earrings). Rings, hoops, studs or other jewelry worn in a non-traditional manner (other than ears) are not acceptable for a professional workplace. Tattoos should be appropriately concealed by clothing regardless of seasonality. Items needed for class: Basic computer knowledge Black or blue pen Paper You may bring highlighters or two packets of 3x5 index cards (to be used as flash cards). Classroom Expectations: Smoking is prohibited on Great Oaks and Ohio Means Jobs properties. Cell phones must be kept on vibrate and out of sight during all HPA classes. Students are not permitted to eat during class. Non-alcoholic drinks are permitted providing the drink is in a container, cup or glass with a lid. Students are expected to clean up after themselves. Students who DO NOT comply with the program expectations will be dismissed from class, and the incident will be reported to the student s funding source and/or employer.
