9 Education Blueprint: Years 1 to 3 Year 1 Year 2 Year 3 Competency and Capacity Building Community & Hospitals Skill-specific Education: HPC/EOL pain and symptom management for communitybased Registered Nurses and Registered Practical Nurses Assessment tools (Edmonton Symptom Assessment System, ESAS) and available community support services Right Patient, Right Place, Right Time: participants knowledge and understanding of the needs of the person and family at end of life, weaving communication skills throughout, so that patients are cared for in the most appropriate setting Community Resource Fair *Offered to community, acute and residential hospice and retirement homes Self-Care for Personal Support Workers (focused on compassion fatigue targeted specifically to PSWs working in the community, community living, and LTC homes Learning Essential Approaches to Palliative and End-of-Life Care Education Program (LEAP); targeted at family physicians and nurses in W/E, C/K and S-L Palliative Care in Acute Care (hospice palliative care education targeted to health professionals working in acute care (Hotel Dieu, Windsor); planning activities aimed transfer of knowledge to clinical practice Data collection by GEM nurses to gain a better understanding of retirement home use of acute care (ED visits, Hotel Dieu, Windsor) Palliative Care Education with Dr. José Pereira, University of Ottawa (health professionals from Chatham, Sarnia, and Windsor)
