A chronic disease is a serious medical condition or illness that is long lasting or recurrent. It is defined as a disease which has lasted or is expected to last for at least six months. Chronic disease self management (CDSM) is defined by the Centre for the Advancement of Health as supporting the person with the chronic disease to engage in activities that protect and promote health monitoring and managing the symptoms and the signs of illness, managing the impact of illness on functioning, emotions and interpersonal relationships and adhering to treatment regimes. The chronic disease self management support role is an integrated and shared healthcare approach that can enable a wider group of support for clients to self manage their chronic disease. The Chronic Disease Self Management Support Program is a short course designed to develop the knowledge and skills required to support an individual or client with a chronic disease though assessing their chronic disease and risk factors, exploring readiness for change, identifying health behaviours to change and working with the client and their health professionals to develop a chronic disease self management plan. The program is driven by a desire to create better health for clients, reduced hospital admissions, better patient compliance and the provision of care by those best qualified to provide advice. 3 The ability of qualified health practitioners, General Practitioners and other practitioners like Aboriginal and Torres Strait Islander Health Workers and other allied health professionals to work collaboratively with people managing chronic disease offers individuals a unique, client focused health outcome. The chronic disease self management support role is an integrated and shared healthcare approach that can enable a wider group of support for clients to self manage their chronic disease. The program is driven by a desire to create better health for clients, reduced hospital admissions, better patient compliance and the provision of care by those best qualified to provide advice. Course Content The Chronic Disease Self Management Support program aims to cover key content areas. These areas include: Definitions Chronic Disease Self-Management Support Program Role Current health care burden of chronic diseases High risk population groups Strategies to reduce the impact of chronic disease Models of self management Motivational interviewing Basic human needs and requirements for wellbeing Barriers to behaviour change Goal Setting Lifestyle issues Chronic Disease Self Management planning education Chronic Disease Self Management communication and documentation Health Care communication frameworks Professional, legal and ethical issues to consider Resources and support Chronic Disease Individualised Self Management Care Plan Putting it all together On successful completion of the program, Students will be issued with a nationally accredited Statement of Attainment for the Chronic Disease Support Program incorporating the competencies CHCCCS001 Address the needs of people with chronic disease and CHCCCS023 Support independence and wellbeing.

4 Chronic Disease Self Management Support Program Role The chronic disease self-management (CDSM) support role is integrated and shared healthcare approaches that can enable a wider group of support for clients to self manage their chronic disease. The CDSM support program acknowledges that one health professional or organisation does not need to provide everything for self management support. The program will assist you to: Identify what will best meet a persons needs Know what options are available How to access services and continued support Reinforce messages given by the clients GP and other health care professionals all vital to the CDSM support role. The CDSM support role is based upon a preventative and proactive approach including: Educating the client regarding their chronic disease and lifestyle changes based upon guidelines as specified by their GP (some clients will already have a GP Management plan or care plan) How to best assist the GP manage the client s condition (using research approved best practice guidelines) Use evidence based self management care models Develop goals with the client and a plan for heath behaviour change (objective and achievable goals) Support an individual to accomplish these goals through identifying barriers to behavioural changes and providing lifestyle modification advice Work as a multidisciplinary team to achieve the best health outcomes for an individual Course Structure Participants will be required to complete professional reading to ensure a level of base knowledge and understanding of chronic disease self management in Australia. Competency will be assessed on: Course assessment questions (a pass mark of 80% is required) Demonstration clinical assessment task The assessment portfolio does not need to be completed until after you have attended the one (1) day lecture.
