Employment is limited to 1,040 work-hours or six ( 6) months. Exempt employees are considered at will and serve at the discretion of the appointing officer. The Department of Public Health is accepting applications for one (1) as-needed position in class 2586 Health Worker II (Public Health Programs). The current position is located at the Population Health & Prevention s Tuberculosis Control Section at San Francisco General Hospital. The position will serve primarily in the Tuberculosis (TB) clinic and sometimes in the community. Please note that the applicant pool from this recruitment may be utilized to fill future vacancies in this class. Shift: Monday-Friday (20 hours per week), weekends and/or holiday shifts may be required. Essential Duties: Under supervision, the Health Worker II will assist with a wide variety of paraprofessional duties in the TB program. The Health Worker II will serve primarily in TB clinic assisting with high risk patient populations, private provider referrals, or community Health Clinic referrals and will provide TB screening in the clinic, as well as in the community. The Health Worker II will function as a liaison to patients and program staff relaying current and accurate TB education to patients

2 Page 2 of 6 between the TB Clinic, patient s home, shelter or place of work. The essential duties include: Administers Directly observed therapy/directly observed Preventative therapy (DOT/DOPT), which consists of providing pre-packaged medications to TB patients to observe medication consumption either face to face or on video; Advise and assist patients in utilizing TB control services; Follow up with patients on scheduled appointments via telephone or letters; Serve as liaison between medical staff and patient population; Conduct per-examination vitals, obtain pre-exam weight for patients on anti-tb treatment, and administer questionnaire for side effects of TB medications; Escort and transport patient between TB Clinic residence, or place of work; Transport staff member s to meeting (subject to approval by administration); Administer DOT to active TB patients who cannot physically come to the TB Clinic-using City authorized vehicles; Record and provide patients with incentives to adhere to therapy; Pick-up and deliver supplies and equipment including high-security pharmaceuticals, laboratory tests and mail; Find and retrieve medical records, X-rays, CD-ROM s and other confidential patient records; Communicate with co-workers, supervisors and patients in courteous clear professional manner; Register patients being seen in the TB Clinic; Provide information on clinic procedures, eligibility to patients; Clean and maintain sputum induction equipment; Prepare incentives for patients to improve treatment adherence both in clinic and in the field; Assist Disease Control Investigator (DCI) with field screenings in the community, some early mornings or late evening including industrial environments, residential hotels, homeless shelters, rehabilitation programs, etc. Attend Community Health fairs; Record and report vehicle malfunctions to administration. The Health Worker 2 performs other duties as assigned. MINIMUM QUALIFICATIONS One (1) year of verifiable experience within the last five (5) years, working with a culturally diverse population performing a combination of the following range of duties: providing information, education, intervention and/or referral services; developing of treatment plans; conducting interviews/investigations; advising counseling clients; escorting/transporting clients; providing courier/dispatcher functions and participating in health promotion and health education outreach

3 Page 3 of 6 activities. Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience. SELECTION PROCEDURES The selection procedure will include evaluation of applications and the supplemental questionnaire in relation to minimum requirements. Depending on the number of applicants, the Department may establish and implement additional screening mechanisms to comparatively evaluate qualifications of candidates. Language Proficiency Exam For positions requiring bilingual fluency, candidates deemed qualified who meet the special language condition must pass a departmentally approved language proficiency exam prior to appointment. CONVICTION HISTORY As part of the selection process an image of your fingerprints will be captured and sent to the California Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI). The resulting report of your conviction history (if any) will be used to determine whether the nature of your conviction (or arrest, in limited circumstances) history will disqualify you as a candidate based on the specific requirements of the position to which you are applying. If selected for

Candidates applying for positions with the Unified School District and the Community College District may be disqualified from consideration should their conviction history not meet the standards established under the California Education Code.
