3 CEO MESSAGE AH Christmas is always a special time for the Adventist Health family. This year, however, it has been made even more special with a series of new openings and exciting events, marking a momentous new era for our hospitals and the communities we serve. The opening of the Minimally Invasive Spine Surgery (MISS) Center at the Hong Kong Adventist Hospital offers new hope for patients suffering from different kinds of spinal problems. With new state-of-the art facilities, our dedicated team of experts can now offer patients a much less traumatic approach to manage their conditions one that results in less post-operative pain and a shorter hospital stay. A Special Care Unit (SCU) has also been set up on the second floor, providing enhanced observation and nursing care services that are above the level provided in a general ward. The Medical-Surgical Unit has been restructured and redecorated as well, and is now equipped with 16 beds including one VIP Suite. In addition, to enlarge our clientele and for the convenience of busy professionals, we have opened the Adventist Medical Center, a satellite multidisciplinary specialist clinic in Causeway Bay. In October, Hong Kong Adventist Hospital, together with Temple University Health System, held an international symposium at the JW Marriott Hong Kong. The event, entitled Medicine in the 21st Century, was aimed at highlighting the latest developments in cardiology, oncology, and robotic surgery in America. It was attended by hundreds of specialists from different fields, reflecting the dedication and passion of doctors who continue to improve and learn. As humble herdsmen, administrators at Adventist Health will push forward in their ongoing mission to provide doctors with the best facilities to benefit our patients. The Topping Out Ceremony for the Tsuen Wan Adventist Hospital s new building in October heralds the impending move of the supporting services to the new building, followed early next year by clinical services, including the Out-patient Clinic, and the Paediatric and Surgical Units. Other specialty services will follow in the near future. The new building is indeed an important milestone in the continuing development of TWAH. The additional space it provides will allow for the introduction of state-of-the-art diagnostic and medical interventional technologies, which will help us better meet the demands of the community. It will firmly establish TWAH as a modern, well-equipped regional hospital, providing quality private medical services in Hong Kong. 4 HK MEDICAL NEWS Staying Vigilant Against the Low Risk of Ebola Despite three indigenous infections outside Africa, the risk of the Ebola virus spreading beyond its place of origin is still quite low. Hong Kong, however, should not let down its guard and vigilance is still important. Dr SETO Wing-hong Infection Control and Microbiologist Consultant On August 8, the World Health Organization (WHO) declared Ebola a Public Health Emergency of International Concern (PHEIC). This declaration acknowledges that the Ebola virus disease (EVD), which was formerly known as Ebola haemorrhagic fever, is a health problem with potential to spread beyond its country of origin and therefore necessitates an international collaborated response by WHO member countries. This declaration does not mean EVD is a global pandemic but rather is a formal call for an appropriate international response to contain the outbreak in Africa. To help put it into perspective, it is worth looking at the wild polio virus. This was also declared a PHEIC in May 2014 because resurgence was noted in 10 countries, but it has never come close to becoming a pandemic. The fact is that the chance of EVD spreading beyond Africa is still rather low. Past Clusters Ebola is not a new disease and has been around for at least 40 years. Large outbreaks are not unprecedented and clusters of hundreds of cases have been previously reported, with all of these occurring in Africa. However, three confirmed indigenous cases of EVD have been reported outside Africa (two in USA and one in Spain). The cases in USA occurred in Dallas, Texas, after an infected man travelled from Liberia to the United States where he went on to infect two medical workers. It should be noted that all 48 family contacts of the Liberian patient did not come down with the disease during the 21 days of mandatory close observation. This is definite evidence that EVD is really not that infectious and certainly not airborne. It is also noteworthy that countries sharing borders with the three affected countries, Liberia, Guinea, and Sierra Leone, are presently all Ebola free, which affirms again the limited infectivity of EVD. Such low level of spread across borders in Africa and to other continents is not unexpected, because human transmission of the Ebola virus requires close contact with infected blood and secretions. Presently, global pandemics usually occur with respiratory infections such as influenza and SARS because transmission is much easier when a cough is a predominant symptom. This is not the case for EVD. In addition, the Ebola virus is a fragile filament (classified as a Filovirus) that breaks up in the air, unlike the more robust, round-shaped influenza virus. This also explains the firm consensus that EVD is definitely not airborne. Low Risk However, despite the low risk of an Ebola global pandemic, the current outbreak is still the largest on record, which makes vigilance of utmost importance. 2 In August, the WHO released updated infection control guidelines and at a recent meeting, Hong Kong s Centre for Health Protection agreed that these guidelines were adequate for Hong Kong. In infection control, overreacting can be just as dangerous and it is important to note that the WHO guidelines contain no recommendation for using coveralls such as barrier man suits or full hoods to cover the head. This is because the de-gowning process involved with these types of personal protective equipment (PPE) can result in unnecessary contamination. Staying in Control The chances of an actual case landing in Hong Kong are certainly low but to be vigilant, health workers should bear in mind the following key action points in the event of a suspected case of EVD:. Take a detailed TOCC (travel history, occupation, contacts of the patient, and clustering), especially with regards to the area where the patient has come from. There are only four African countries with infections at the moment, but it is important to stay alert to new additions. This is perhaps the single biggest action that can be taken to avoid EVD from entering Hong Kong.. Place the suspected patient in a single isolation room.. Equip health workers with the appropriate PPEs including gloves, impermeable gowns, medical masks, and eye protection. All these items are available at hospitals. They will also require rubber boots, which are also available at the relevant clinical areas.. Inform the infection control unit immediately so that it can provide real time supervision.. Maintain hand hygiene and needle safety. These are extremely crucial but should already be part of normal practice. The WHO guidelines also contain numerous recommendations regarding post clean-ups, management of linen, waste management, laboratory precautions, and post exposure management. 13 EVENT HIGHLIGHTS TW Marking a Milestone The Topping Out Ceremony for Tsuen Wan Adventist Hospital s new building was held in October Hundreds of guests gathered at the beginning of October for the official Topping Out Ceremony for the new building. With better diagnostic facilities and backup support and a wider range of specialist services, TWAH will provide more acute services in different specialties to meet the growing demands of the community. The hospital s new 25-storey, 470-bed building will house 10 operating theaters and four endoscopy suites - four times more than its current capacity. The cardiology program will also be further developed in both diagnostic and interventional areas, providing support for family physicians and generalists in the Out-patient Clinic delivering 24-hour urgent care to the community. The Out-patient Clinic will move to the ground floor in May 2015, and the Clinical Laboratory and Rehabilitation Center will relocate to the first floor. The Pharmacy Department and the Specialist Center will be located on the second floor, the Paediatric Unit will be on the 15th floor, and the Surgical Units will be on the 16th floor. 15 HOSPITAL NEWS HK Exchanging Ideas on Infection Control Taiwan s infectious disease and infection control specialists visit Hong Kong Adventist Hospital Dr LEE Chun Ming, president of the Infection Control Society of Taiwan (ICST), led a team of 17 infection control specialists from Taiwan to Hong Kong Adventist Hospital in August. The delegation exchanged infection control insights with peers at HKAH and enjoyed tours of the operating room and a number of units including the Short Stay Unit, Endoscopy Unit, Central Sterilization and Reprocessing Unit, and Obstetric Unit.» Dr LEE Chun Ming and colleagues from Taiwan» Ms Clara LEUNG, Director of Nursing, presents a certificate of appreciation to Dr LEE and colleagues. Assessing Swallowing Disorders Dysphagia is increasingly prevalent as the population ages. Complications can be severe or even fatal, and the disruption of the swallowing process can significantly undermine a patient s quality of life. In response, Hong Kong Adventist Hospital has assembled a team of respiratory physicians, speech therapists, and dietitians to facilitate the Dysphagia Rehabilitation Program. In addition to taking a thorough medical and feeding history of each patient, the medical team also conducts physical examinations and further investigations. HKAH offers the fiberoptic endoscopic evaluation of swallowing (FEES), a sensitive and safe investigation that evaluates the anatomy and swallowing action of a patient to help assess the severity of abnormal swallowing. Under the supervision of a respiratory specialist and a speech therapist, a visual display of the swallowing process allows the patient or family members to learn about various swallowing techniques in order to specifically cope with the patient s condition. 16 TW EVENT HIGHLIGHTS Tsuen Wan Adventist Hospital Opens Basic Life Support Training Center Tsuen Wan Adventist Hospital has established the Basic Life Support Training Center (BLSTC) to offer both the American Heart Association (AHA) Basic Life Support Provider Course and Renewal Course to its staff. These classroom-based courses teach participants how to:» recognize life-threatening emergencies» provide cardiopulmonary resuscitation (CPR) to victims of all ages» use an automated external defibrillator (AED) in a safe, timely and effective manner The course will be offered once every month and is mandatory for all clinical staff. It is imperative that all hospital staff are capable of recognizing cardiac arrest, performing chest compressions, and using AED to ensure high survival rates in cases of in-hospital cardiac arrest. The training at the TWAH center is a hands-on, skillsbased program in line with current guidelines from AHA, and emphasizes integration of behavioral, communication, cognitive, and technical skills. BH CHARITY EVENTS Making a Healthy Lifestyle a Family Affair It s never too early to start learning about healthy choices. This past summer, we held our first NEWSTART program for children and it was a huge success. Working with the Social Welfare Department to select participants, the program was made available to underprivileged children. We hosted a three-day camp in July for individual participants and one in August for family groups. The eight active children took part in lectures, games, exercises, crafts-making, and stories where they learned more about NEWSTART principals and nutrition.
