2 CHANGE YOUR LIFE SM WITH FLORIDA HOSPITAL Employee Wellness Program 2015 We just completed the first year of our comprehensive Change Your Life Employee Wellness program and have seen great progress with the building of our wellness culture! This document provides valuable information about our program, incentives, and the exciting changes we have coming in What is the Change Your Life employee wellness program? Change Your Life is a wellness program that provides you with the tools, education, and resources necessary to help you live life to the fullest, according to the eight CREATION Health principles. CREATION Health is our philosophy for whole-person health and includes eight principles to guide you on your wellness journey. These eight principles are: Choice, Rest, Environment, Activity, Trust, Interpersonal relationships, Outlook, and Nutrition. Why do we offer this program? A healthy workforce reflects Florida Hospital s Christian mission, allows us to provide better care to our patients, and is key to our future success as an organization. We hope you re as excited about this program as we are and that you ll choose to take your first step or continue your journey toward a healthier and happier you! Why should I join Change Your Life Employee Wellness? The benefits of this program include the opportunity to: n Track your health values using our free personalized online dashboard n Access health and wellness programs throughout the year n Earn LifePoints for attending health and wellness programming that can be redeemed for great rewards n Receive e-newsletters containing valuable health information, tips for improving your health, healthy recipes, and discounts/ offers at area vendors What are the incentives? There are two levels of benefits! 1. Cash: If you carry our Florida Hospital health insurance, you and your spouse will have the opportunity to qualify for cash incentives based on your screening results, program participation, and other criteria described later in this document. This means you and your spouse could qualify for combined incentives totaling up to $936 per year! 2. LifePoint Rewards: All Change Your Life members can earn LifePoints for the programming and activities you participate in throughout the year. Best of all, you can earn LifePoints and redeem them for rewards whether or not you qualify for the cash incentives. How do I get started? GENERAL INFORMATION: 1. Join today! Log on to to join as an employee member and learn more about the program. Spouses who join should also answer yes to the employee question during registration. 2. Stay tuned! While not part of the cash-incentive structure, the LifePoint programs and activities offered through Change Your Life are designed to reward you for making healthy lifestyle choices! LifePoints are earned through participation in Change Your Life programs, screenings, activities and events, which are categorized according to the CREATION Health principles for a balanced, healthy lifestyle: Choice, Rest, Environment, Activity, Trust, Interpersonal relationships, Outlook, Nutrition. LifePoints can be redeemed for rewards, and you can track your LifePoints on your personalized dashboard at The best part is that everyone can earn Lifepoints just for participating in our Change Your Life programs and activities! Exciting new features coming in 2015 with WebMD! Coming new in 2015, Adventist Health System is launching an all-new system-wide partnership with WebMD. This new vendor will bring us a full-service wellness e-portal with access not only to an online health risk assessment, but also to online learning modules, trackers, and other exciting features to support you in your wellness journey. What s changing and what does this mean for me? n Beginning in 2015, Change Your Life will utilize WebMD for administering our online employee/spouse health risk assessments. n WebMD is a web-based wellness platform and does not replace our Change Your Life wellness program. It will only enhance what we can offer you as part of our comprehensive approach to wellness. n WebMD works with Summit Health to administer onsite employee wellness screenings. Is participation mandatory? No, participation is completely voluntary, and your results are kept strictly confidential, reported only to you from a third party vendor (your employer does not have access to your personal health information). We encourage you to share your results with your physician.

4 Summary of WebMD Features: After January 1, 2015, go to to register. n Health Risk Assessment n Once you ve registered, your next step is to take the short and insightful Health Assessment. n Complete the confidential questionnaire about your current health status and lifestyle, and you ll receive a detailed, easy-to-understand report that provides an evaluation of your health, along with a personalized plan to help you reduce or eliminate the health risks identified. n Health Assistant n You can take action toward better health with specialized support that conveniently fits into your schedule. Your Health Assistant is like a coach in your pocket. Whether you are setting a goal to lose a few pounds or manage a condition like diabetes, your Health Assistant can help anywhere, anytime. Find inspiring activities, track your progress, and read information all with an aim toward feeling your best. n Your Health Assistant is a research-based, online tool that can help you make healthy lifestyle changes and manage chronic conditions. It s not only easy to use, but motivating too, helping you make better choices for better health. You can access the Health Assistant from your computer or from the palm of your hand with your smartphone. n WebMD Health Coaching SM n Some people enjoy the structure and one-on-one support of a personal coach to help them make healthy changes. If this sounds like you, it s time to meet your WebMD health coach. n Through a series of convenient phone conversations, your health coach can help you establish your health goals and create a customized plan that really works for you. Your coach will help you stay focused on lifestyle behaviors that can maximize your long-term health. n Some of the topics you discuss may include: n Exercise n Nutrition n Emotional health n Strength training n Weight management n Stress management n Quitting smoking

5 n WebMD Daily Victory TM is a mobile app that lets you take a simple approach to physical fitness. Track your exercise progress, find support, and claim victories all on your smartphone! Here s how: n Start small. Five minutes of walking is a great way to start. n Complete one activity each day just one. n Tap the app to indicate it s done. That s it! n Complete three days in a row, and the app encourages you to bump up your activity level. n Daily Victory is a great way to build small steps into healthy habits, day by day. Change Your Life, CREATION Health, and now WebMD I m CONFUSED! Let s straighten out any confusion you might have: 1. Change Your Life with Florida Hospital SM is the brand for our wellness membership program and offers access to ongoing programs, events, challenges, newsletters, education, and more. The Change Your Life program is based on the eight principles of CREATION Health and all activities are grouped into one of these eight categories. 2. CREATION Health is a philosophy for whole-person health and includes eight principles to guide you on your wellness journey. These eight principles are: Choice, Rest, Environment, Activity, Trust, Interpersonal relationships, Outlook, and Nutrition. 3. WebMD is the company Florida Hospital will use to power our online wellness portal and health risk assessments. WebMD works with a provider called Summit Health to conduct our onsite wellness screenings. Who can participate? Change Your Life with Florida Hospital: n Anyone (employees, family members and community members) can join Change Your Life, access the personalized dashboard, participate in events/programs, and earn Lifepoints that are redeemable for rewards in our online store! WebMD portal, online health risk assessment, and health screenings: n All part-time and full-time employees, as well as covered spouses (spouses who are on our health plan), are eligible to access the new WebMD wellness portal, participate in our annual comprehensive health risk assessment and screening, which includes individualized health profile reports to support your wellness journey Financial Incentives: n Employees and spouses who carry our health plan will have the opportunity to qualify for incentives of $528 per year for covered employees and $408 per year for covered spouses ( covered means that you carry our Florida Hospital health insurance plan). Please read further in this document for additional details on how to qualify for the incentive. When/where will the next health screenings take place? Our next health screenings are scheduled for February-March 2015 at each hospital campus. Log on to the WebMD portal in January to see the variety of screening dates and times offered, and RSVP for your appointment! Eligible employees/spouses MUST RSVP online through WebMD if they wish to attend a screening. What will be measured at the health screening? n Prior to being screened, you will complete a health risk assessment survey online through the WebMD portal. n You must schedule your screening appointment in advance through the WebMD portal. n At the screening, you will have the following measurements/ tests taken: n Total cholesterol n Blood glucose n HDL cholesterol n Blood pressure and pulse n Total cholesterol/hdl ratio n Measured weight n LDL cholesterol n Measured height n Triglycerides n Waist circumference

6 What is a health risk assessment? A health risk assessment is a questionnaire used to assess your overall wellness, identify your areas of health risk, and provide you with a comprehensive report. You are the only person who will receive this report and are encouraged to share it with your physician. The goal of the report is to provide you with the baseline information you need either to start setting goals to improve your health or to continue your healthy lifestyle. Does HR or my boss have access to my health risk assessment or screening results? No. All personal health information from the screening results and program participation will be kept strictly confidential. Management will only have access to group data (de-identified), provided to us by WebMD, which will be used to develop programs to meet your needs. How do I qualify for cash incentives? 1. You and your spouse must be covered under the Florida Hospital health insurance plan to be eligible for cash incentives. 2. Join Change Your Life as an employee member by visiting 3. Beginning in January 2015, access our all-new WebMD wellness portal. Create your login, take the online health risk assessment, and schedule your health screening, which will take place in February-March Attend your scheduled screening appointment. 5. Earn 100 WebMD points by the November 30 deadline! How do I achieve the 100 WebMD points to earn the cash incentive? In order to qualify for the cash incentive, covered employees and spouses must earn 100 points on the WebMD portal by November 30, * Please note, that all 100 points must be earned by the deadline in order to receive the cash incentive. There are no partial payments awarded if less than 100 points are earned. Here s how the program points are structured: Requirements Points Health Risk Assessment Completed 25 Health Screening Completed 25 Health Metrics HDL >= 40 5 LDL <=130 5 Triglycerides <=150 5 BMI <27 5 BP <130/85 5 Glucose fasting <115 5 Total Cholesterol < or- Alternative for missing a health 5 points each call metric: Health Coaching Calls (up to 7) Preventive Activities (choose from 15 a variety of activities listed on the WebMD portal to total 15 points. List will be available on the portal beginning in Jan. 2015) Total possible points: 100 What happens if I fail the health screening? There is no such thing as failing. The goal of this program is to help you learn ways to live a healthier lifestyle and to reward you along the way. The health screenings will be offered annually, and we encourage you to participate in all the programs and activities offered through Change Your Life throughout the year. If you do not initially qualify for the financial incentive because you missed one or more metrics measured during the screening, you will have the option to complete one telephonic health coaching call for each missed metric.

7 I participated in the 2013 Employee/ Spouse health screenings. What happened to our Know Your Number website, and can I still access my report? Know Your Number, powered by Biosignia, Inc., was the previous vendor we used for our online health risk assessments. We will no longer use this vendor once we launch WebMD in January Don t fret! You can still access your personal wellness profile on the Know Your Number website! n Click the link on the CYLemployees.org website, or type the following web address in your browser (be sure to type the n Forgot your Know Your Number login information? Contact us at CYL@ahss.org and we d be happy to help! Where can I find a Primary Care Physician (PCP)? If you carry the Florida Hospital insurance, you may visit to view a list of physicians within our network. What does an annual physical with my PCP entail? Your annual physical with your PCP should be a routine part of your preventive health care. Typically, during an annual physical, your PCP will review your health history and vital signs, order various lab tests depending on your needs, and perhaps most importantly, discuss prevention of chronic diseases and help guide you on your wellness journey. During this visit, we encourage you to speak with your doctor about screenings that you may be due to receive, and share the results from your personal health profile report! Questions? 8 3Wellness Checklist Getting started on your path to Wellness: Where do I begin? The first step toward your healthy lifestyle is about selfleadership taking charge of your personal wellness! Then, follow these steps to get the most out of the program! n Get familiar with the tools and resources offered through the Change Your Life website ( and the WebMD portal to understand what is available. n Register on the WebMD portal after January 1, 2015 and take your health risk assessment. n Schedule your screening through the WebMD portal for February or March, n Participate in the various wellness challenges and programs that are offered all year long through our Change Your Life Employee Wellness program or the WebMD portal, and follow recommendations for preventative health and wellness care visits with your physician. The goal here is to make wellness your new norm and part of your daily routine. n Get to know your department s CREATION Health Coach! Don t have a coach? Want to serve as your department s CREATION Health Coach? It s easy! us at CYL@ahss. org! This is a great way to hold yourself accountable to your goals while supporting your teammates on their journey. n Once the results of your health risk assessment and screening are processed (after February 2015), you will be able to access your personal health profile on the WebMD portal. You are encouraged to review this report, share it with your physician, and use it as your starting point toward the creation of a healthier you. n After you review your personal health profile report, choose one or two goals to start working on. Opt for a health coaching call, available through WebMD! n If you carry Florida Hospital s health insurance, keep track of your WebMD points during the year to reach 100 points by November 30 so you can take advantage of the cash incentives!
