1 JOB DSCRIPTION TITL: MPLOYR: LOCATION: RPORTING TO: HOURS: SALARY: FIXD TRM: Social Prescription Navigator Basildon Billericay & Wickford Council for Voluntary Service (BBWCVS). Based at George Hurd Centre, Audley Way, Basildon but with travel throughout the area of benefit including Pitsea and Laindon. Social Prescription Service Project Manager 35 hours per week SO1 (SCP 29-31) 25,440-27,123 (full time 35 hrs) 18 months MAIN DUTIS AND RSPONSIBILITIS The Social Prescription Navigator(s) will be responsible for delivering a person centred service, supporting people to improve their health and wellbeing by connecting with services in their community. The navigator will be a vital and visible community resource, responsible for patient engagement and developing effective working relationships with health care professionals and third sector providers. 1. Service Delivery 1.1 Undertake one to one holistic patient assessments and develop personalised action plans. 1.2 Proactively identify people s needs using approved tools and procedures to ensure people get the right support to meet their needs. 1.3 Provide information on services locally, including signposting and co-ordinating pro-active referrals. 1.4 Assist with the development of the service database including the menu of services. 1.5 Help embed the Social Prescriber service within the care/support pathways. 1
2 1.6 With support from the SP Project Manager, work proactively with health care professionals to stimulate referrals in line with agreed key performance indicators 1.7 Provide timely feedback to referral partners on the support provided to patients. 1.8 Support project development by raising awareness of the service through training, presentations and events. 1.9 Support the Social Prescriber Project Manager to plan and deliver at two Health and Wellbeing clinics to enable people to participate in their recovery, through information sharing and the promotion of positive lifestyles Promote a culture based on the principles of person centred support, ensuring dignity, choice, respect, independence and rights are upheld at all times 2. Service Development 2.1 In partnership with the Project Manager develop and review operational plans for the effective delivery and ongoing development of the service. 2.2 As part of the wider social prescriber team develop and maintain quality assurance and improvement systems that monitor standards and support high quality service delivery. 2.3 In liaison with the Project Manager and key stakeholders, contribute to the strategic planning and development of the service. 3. xternal Partners and Communication 3.1 Develop and maintain effective relationships and communication with stakeholders including health professionals and wider social prescribing initiatives. 3.2 Develop and maintain effective working relationships with key community-based service providers to ensure effective and smooth onward referrals e.g. Health Trainers, coaching services, support groups and employment providers. 3.3 Develop and maintain good communication with people accessing the service to ensure the needs and preferences of individuals are met. 4. Monitoring, Reporting and valuation 4.1 Produce and submit service monitoring reports to the Social Prescriber Manager in the agreed format and in accordance with agreed timetables 4.2 Maintain systems to keep accurate records relating to the delivery and outcomes 2
3 of the service including patient case notes 4.3 In partnership with the Social Prescriber Project Manager develop and implement referral feedback process e.g. to GP, clinical nurse specialists 5. Training and Development 5.1 Attend one-to-one meetings and annual performance appraisal meetings with the Project Manager 5.2 Undertake regular training for the development of the service and individual career progression 6. Other Responsibilities 6.1 Adhere to BBWCVS s policies and procedures at all times including qual Opportunities, Health and Safety, Lone Working, Confidentiality and Data Protection protocols and Safeguarding 6.2 Responsible for producing own correspondence, performing and maintaining administrative tasks 6.4 Undertake any other additional tasks as reasonably deemed appropriate to meet the needs of the service. 3
4 PRSON SPCIFICATION Skills, Knowledge, and xperience The ideal candidate will have knowledge, experience, skills and a proven track record in the following key areas: xperience ssential Desirable Shown by At least 2 years experience of working in the voluntary and community sector or a health/social or community setting (e.g. advocacy, nursing, psychology, physiotherapy, counselling, coaching or social work). xperience of working in a health/social setting is preferred and desirable. xperience of developing and maintaining partnerships with a range of partners from the public and voluntary sectors. xperience of using and maintaining databases and client notes. xperience of working with health care professionals. xperience of working with and supporting volunteers. xperience of monitoring outputs and outcomes of projects. Skills and Knowledge An understanding of the concept and benefits of social prescribing. Presentation Ability to develop and deliver quality services.
