3 Program Overview Mums and Kids Matter is an initiative funded by the Australian Government through NSW Health. It is designed to support mothers living with severe and persistent mental illness and complex needs, and their children (0-5 years), by providing specialist short-term accommodation and support services to help them recover. Care is provided via in-home interventions in local communities; specialist support in short-term non-acute residential care and/or tertiary support in specialist facilities. The program is underpinned by a basic belief that retaining intact families, rather than having children placed in out of home care, is beneficial for the child, for the mother/parents, for the immediate family, and for the community, as well as being more cost effective. The primary focus of the program is to address the complex needs of mothers with mental illness and their children (0-5 years) by providing mental health care and parenting support, whilst also catering for needs related to physical health, accommodation, vocation/education, disability and other co-morbidities. 1. Overview of team The multidisciplinary team works with for the benefit of each mother and her child(ren) to meet her recovery goals. Team members who work directly with each mother and her child(ren) consider physical, psychological, social and spiritual needs within the context of the mother s lived experience and in partnership with her family, significant others and the broader community. They provide expert care to program participants during their stay in relevant, tertiary in-patient facilities, in the Mums and Kids Matter residential facility and at home, in their chosen community. They also provide care during transition periods between these facilities. They ensure that each mother and her family is linked to various local resources and services so that they are provided with every opportunity to fully participate in community life. 2. Overview of role The Clinical Psychologist is an appropriately qualified, skilled and experienced mental health professional who understands the person centred recovery model within the context of a step down mental health service. The Clinical Psychologist will work skilfully within a multidisciplinary setting to: Coordinate care for clients Oversee regular data collection Monitor, evaluate and report against program performance measures Contribute towards staff training as required Undertake project work e.g. Carelink The Clinical Psychologist will work collaboratively with staff of Mums and Kids Matter to ensure the effective running of the program. Relationships Reports to: Coordinator Allied Health, Mums and Kids Matter Works with: Other team members, referrers including but not limited to mental health case workers, Family and Community Services workers etc. 4. Major role responsibilities 4.1. Our clients Coordinate care for clients Be an active part of the Mums and Kids Matter s multidisciplinary team to develop and roll-out care planning, care reviews and transition and/or exit planning meetings Undertake comprehensive planning to best meet the needs of the individual consumer and to promote recovery Work with mothers and their families and/or carers and as part of the Mums and Kids Matter multidisciplinary team, coordinate care for clients Assist identify, mitigate and manage concerns relating to child safety, health and wellbeing by: Identifying risk and potential risk early, and in association with each mother s unique circumstances Acting according to relevant policy and procedures, to ensure the safety and well-being of the child(ren) Discussing concerns with the Coordinator Allied Health, the Early Childhood Facilitator and/or other relevant staff as required (including the Quality, Risk and Compliance Specialist) Working in collaboration with Mums and Kids Matter staff, family and other stakeholders to develop safety plans to address identified Risk of Harm and Risk of Significant Harm concerns Continuing to monitor risk Making reports to the NSW Department of Family and Community Services in line with mandatory reporting legislation that includes, but is not limited to, the NSW Mandatory Reporter Guide Making reports, as relevant, in line with Wesley Mission s policies and procedures. Identifying early if the risk becomes too great for the mother and/or her child(ren) to live safely at Mums and Kids Matter or in the community with her child(ren) Invite, analyse and respond to consumer feedback, complaints or grievances and help develop remedial actions Regularly meet with the Coordinator Allied Health Apply expert mental health knowledge to improve client care Performance measures 100% clients (that you coordinate care for) have a documented, care plan containing interventions that are individually tailored to meet the needs of each client Every transition is supported by a member of team and is documented Clinical Psychologist Manager s initials Employee s initials 4

Our people (our team) Train team members to record data and collect outcome measures in accordance with best practice Provide the team with clear direction and mentoring on expected deliverables, identifying opportunities and managing issues as they arise Comply with all Human Resource (HR) policies and procedures and consult the HR department as required Work effectively with all staff of Mums and Kids Matter to ensure the effective running of the program Work effectively as part of the Mums and Kids Matter multidisciplinary team, within the framework or person centred recovery model Ensure a high level of staff morale and collegiate culture that supports professional excellence. Lead staff by example to promote interdisciplinary support and cooperation for the benefit of clients Support the Operations and Clinical Manager to execute Wesley Mission s strategic and Mums and Kids Matter business plans Exercise any other authority which may be delegated from time to time with due care and professionalism, maintaining an awareness of organisational expectations and standards at all times Performance measures Meet and/or exceed individual KPIs Ensure all frontline workers record outcome measures for every client Active and positive participation as a key member of the MAKM multidisciplinary team 5.3. Our stakeholders (those we serve or work with outside the organisation) Build strong and effective relationships with key stakeholders and harness their support for the Mums and Kids Matter Program and for Wesley Mission Identify, develop and maintain positive and effective partnerships with relevant services to support clients of the program Work cooperatively with Wesley Mission staff in all work settings, demonstrating generosity, Christlike servanthood, and a willingness to contribute towards the success of Wesley Mission as a whole Performance measures Positive feedback received from referrers Positive partnerships developed and maintained 5.4. Our systems Actively participate in and contribute to client information system (Carelink+) project Ensure adherence to our project delivery processes Regularly review systems and processes to seek more efficient and effective methods of delivery to ensure continuous improvement Train staff in record keeping Ensure effective and high quality clinical documentation is maintained by frontline workers Be an advocate of the Wesley Mission brand, ensuring brand compliance and use of correct templates. 6 Performance measures All care plans are complete and uploaded to Carelink+ Complete project work as requested Spot audits demonstrate improvements in record keeping in Carelink Our financials Operate within the financial policies and budgets of Wesley Mission When entrusted with Wesley Mission assets, including vehicles, mobile phones and internet access, protect the asset from harm or loss and keep discretionary and personal costs to a minimum Performance measures Operate within budget Professional responsibilities Assist the Operations and Clinical Manager in maintaining a positive service culture within Mums and Kids Matter (MaKM) that is proactive, non-judgmental and reflective of Wesley Mission s Christian values. Apply and promote a person centred recovery approach to practice and service delivery Manage all aspects of your workload including meeting individual KPIs as advised by the Coordinator Allied Health and amended from time to time Write reports and complete evaluation activities as requested by the Coordinator Allied Health. This may include but is not limited to data collection and monthly reporting requirements against targets.
