You will also support staff at IRH who wished to stop smoking. The substantial part of the post will be in the mornings on the wards with occasional afternoon meetings. You will assess the patient s motivation and nicotine dependence, and help the patient to plan and prepare for their quit. You will discuss NRT and initiate via prescribing staff. You will continue to support the patient during their hospital stay. 6 Allied Health Professions 29211G/VB Health Improvement Practitioner - Smokefree Community Services Band Greenock Health Centre, PA15 4LY A post is available within Inverclyde Community Health & Care Partnership (CHCP) based in Greenock Health Centre. It is a fulltime (37.5 hrs) fixed term post until the end of March As part of a wider tobacco team you will provide effective intensive stop-smoking support for communities and individuals within the CHCP. Work will be carried out in accordance with the framework provided by the Tobacco Planning and Implementation group at Greater Glasgow and Clyde Board, implemented at a local level by the CHCP with the aim of improving health and reducing health inequalities. The bulk of intensive support involves planning and running stop-smoking groups and, using a protocol specifically designed for the purpose, providing follow-on support to clients who wish to give up smoking. Referrals will come from across the NHS, and significantly from its independent contractors, as well as through many self referrals. You will be expected to stimulate the interest of communities, voluntary sector and other partner agencies to engage with the service. The current model used for our smoking cessation service is evidence based, and developed at the Maudsley Hospital in London. The post may also require you to contribute to broader Health Improvement programmes within the Health Improvement team as required.
