1 1) Purpose of the Agreement The provision of quality education and training of social work and social care professionals depends on the effective partnership between the Education Provider and the placement organisations involved in providing practice learning experiences. Practice learning experiences are provided in a wide range of organisations including social services, health, voluntary and independent sector settings. In this Agreement, the Education Provider refers to Sheffield Hallam University (SHU), and the word 'Agency' is used for all organisations providing practice learning experiences for SHU social work and social care students. 2) Objectives a) The Agency will: i) Provide support to the student(s) whilst engaging in the learning processes of a quality and diverse placement i Offer placement opportunities without discrimination on the grounds of age, disability, gender, race, religion and belief or sexual orientation Work with SHU to offer any reasonable adjustments that are needed for students with additional needs Provide a named link/responsible person who will oversee the day to day activities of the Student(s) and/or the running of any training programme delivery within the Agency v) Ensure that the students receive an appropriate introduction and induction to the practice area in which the learning will take place including providing students will access to all relevant policies and guidelines. vi) Be responsible for providing on-site supervision, assessment and support, and adhering to SHU guidelines (e.g. Practice Learning Agreement, Practice Learning Documentation, etc.). This will include supervision in accordance with professional standards where applicable - for social work students either an on-site practice educator or, an on-site practice supervisor and an off-site practice educator who meets TCSW PEPS (2012, updated 2013); for youth and community work students a Work Based Learning supervisor who meets National Youth Agency (2010) standards. v Keep a record of where the student(s) is/are placed that indicates the on-site practice supervisor(s) (if needed) the practice educator(s), the training and support systems and the line management during the placement process vi Ensure that the student(s) is/are informed early in their placement of Agency policies, procedures and guidelines and receive a comprehensive, mandatory induction, including Health and Safety information that will be part of their placement ix) Maintain open communication with SHU, and within the Agency to ensure that the student(s) receive quality placements. 2 b) The Agency will honour their commitment: i) For on-site practice supervisors and/or practice educators to attend SHU practice preparation sessions, when appropriate or necessary, or to send a member of staff who will be responsible to cascade the information within the Agency i To negotiate with SHU other relevant meetings staff will need to attend that are part of the placement process To work in accordance with SHU Criminal Records Consultative Panel (CRCP) procedures and processes 1 To ensure staff, e.g. on-site practice educators, off-site practice educators and on-site practice supervisors, who supervise SHU students, have had an Enhanced DBS Disclosure carried out v) To confirm that the Agency has a DBS policy and safeguarding in recruitment and continuing employment vi) To confirm that the Agency Board of Trustees and/or employers have seen the Enhanced DBS Disclosures of their own staff and are aware of any criminal convictions disclosed v For the Agency to actively engage with the SHU Practice Learning Placements Team to promote placements at their Agency vi To provide students with learning opportunities to enable them to demonstrate capability to meet audited placement learning outcomes. ix) To promote encouragement by key personnel, who will provide peer support to assist students in their growth and development whilst on placement. c) The Agency will ensure that: i) A practice report is written for each student by an appropriate person, normally the supervisor, who has completed all professionally prescribed preparation for the planning, support and assessment of student learning where required by the relevant Professional and Statutory Regulatory Body (PSRB) and/or National Occupational Standards. i Each report is submitted to SHU within the specified timescale. They offer placements to re-sit student(s) who need a repeat placement, and for those students who may have additional support needs The confidentiality of all records and personal data relating to students is maintained at all times in accordance with the Data Protection Act 1998 v) They submit placement confirmation forms and any relevant invoices to SHU at the end of each placement. 3) Agency location a) The student(s) will be based at this placement but it can include some outreach locations. The Agency must specify any additional locations that may want to use prior to the placement of the student(s) to ensure that these have been covered by an approved audit process. 1 Please refer to the procedure Joint SHU and SU CRCP Policy DBS which can be viewed and download from the SHU website at under Policies and Guidance. 3 4) Agency hours of service a) The hours for students on placement will be confirmed during the Learning Agreement meeting, in accordance with the requirements of the appropriate PSRB, where relevant. 5) Student travel expenses a) The Agency will provide guidance in relation to travel expenses incurred by the student while carrying out duties and responsibilities on behalf of the Agency. 6) Health and Safety Issues a) SHU will: i) Inform students of their individual obligations to comply with Agency policies in relation to Health and Safety Take all reasonable care to ensure that students are appropriately prepared to carry out the tasks to undertake practice learning. b) The Agency will: i) Treat the student as an employee for the purposes Health and Safety legislation, in accordance with The Health and Safety (Training for Employment) Regulations i Undertake appropriate risk assessments, provide Health and Safety briefings, provide protective equipment (where necessary), and competent supervision appropriate to the needs of the student in training. Provide a Health and Safety induction at the commencement of the placement to inform the student of local arrangements for Health and Safety and their individual obligations to comply with the Agency Health and Safety policies. Report to SHU any Health and Safety incidents or near-misses involving the student to enable SHU to discharge its responsibilities for ensuring its student's Health and Safety. 7) Insurance and Liability a) The Agency will: i) Hold and maintain adequate public liability, employer liability, or other appropriate insurance or cover as are necessary to the Agency, its staff, the student and SHU in respect of personal injury or death, or injury, loss or damage to property, and vicarious liability 8) Quality Assurance Be deemed to be the employer of the student and of the staff engaged in their supervision and assessment for the duration of the placement. a) The Agency will: i) Work with SHU to implement quality assurance measures and act on recommendations arising from the quality assurance of practice learning Ensure that relevant SHU procedures are followed for the purpose of audit and quality assurance. 4 b) Practice placement audit i) All Local Authorities, NHS organisations and/or other organisations in the private and voluntary sectors involved in providing practice learning experiences shall be audited by SHU prior to a student being placed with them. i This is to ensure that the Agency meets the requirements of PSRBs and any additional Health and Safety and Quality Assurance requirements of SHU. No student shall be placed with an Agency unless there is a satisfactory audit in place. SHU is responsible for carrying out audits on all their active practice placements. For an Agency new to SHU, a full audit will be carried out and on successful completion they will be added to a register of approved agencies. Each Agency will receive a copy of the completed audit and a copy of this partnership agreement for their retention. It is essential that the Agency can produce these documents if requested by a representative of SHU, a student on placement with the Agency or by the PSRB, where applicable. c) Placement evaluation i) On the completion of a placement, the Student and the supervisor/practice Educator (On-site or Off-site) will each be required to complete a practice placement feedback form. This is to assist SHU in monitoring and evaluating the quality of the practice placement. i 9) Disciplinary a) SHU will: Each practice placement form has been reproduced in an electronic format to aid completion. These are held in secure, Google Drive folders. The Student Placement Team will the web link of the applicable practice placement feedback form to the Student and to the supervisor/practice Educator. The information from the completed practice placement feedback forms will be held in strictest confidence by SHU and shall not be divulged to any third parties. i) Inform students that they continue to remain subject to SHU Disciplinary Regulations whilst at the Agency and that they must also abide by the rules and working practices of the Agency. The Agency shall ensure that the student(s) is/are aware of the rules and working practices at the Agency workplace and give the student(s) guidance on acceptable conduct in the workplace. i Respect the principle that the Agency has the right to remove a student from a placement in any case where it reasonably considers this is necessary having regard to the student's conduct or professional suitability. The Agency must inform SHU immediately in this case. The Agency and SHU will organise a meeting as soon as possible and follow SHU procedures for the concerns, suspension, termination, and any other procedures taken in connection with the student in a placement setting. Cooperate with the Agency in dealing with any complaint or personal injury involving a student in a placement setting. Support the appropriate staff to participate in any concerns, disciplinary and appeals processes relating to a student's performance in the practice learning setting at the request of the Agency. 5 10) Information/Communication a) SHU will provide information to the Agency including: i) Planned developments and curriculum updates i Placement assessment setting Planned numbers of students for each placement setting for each year s of students for each placement setting v) Updated lists of tutors vi) SHU policies which may impact on the Agency staff v Materials designed to support students' learning in practice settings vi Electronic version of Practice Learning Documentation/Portfolio (in pdf format). 11) Confidentiality a) SHU will ensure that each student is aware of the need to keep confidential the personal and confidential data to which she/he has access during the placement, to abide by the provisions of the Data Protection Act 1998 and the data protection policy of the Agency. b) The Agency will ensure that the student(s) is/are made aware of the workplace arrangements for data protection and data security as part of their induction process 12) Agreement Duration, Review and Termination a) This agreement shall be effective for two years from the date on which it is signed by both parties b) The agreement shall be reviewed at a defined frequency once every 2 years as a minimum, with the following exceptions where earlier review is required: Where a placement evaluation has raised formal concerns about placement performance Where the HEI has raised formal concerns about placement performance Where another public body including but not limited to the Care Quality Commission (CQC), Office For Standards in Education, Children's Services and Skills (OFSTED), Health Care Professions Council (HCPC) raises formal concerns about the organisation Copyright Sheffield Hallam University 2015 Version: May 2015 Page 5 of 6
