1 Team Leader - Home Support (Ongoing, fulltime position based at Hervey Bay) The Institute for Urban Indigenous Health (IUIH) was established to provide a coordinated and integrated approach to the planning, development and delivery of primary health care services to Aboriginal and Torres Strait Islander populations. The IUIH Aged and Community Care Service provide in-home care services to eligible clients in the Hervey Bay and Maryborough regions. The Services will work with the local Indigenous community and relevant key stakeholders to increase access and provide a coordinated comprehensive care service with a focus on assisting the eligible aged receive quality in house care and home support. The primary purpose of this position is to in accordance with the clients assessed needs: Manage the contract relationship and arrangements with the Client/Carer and review ongoing service needs Coordinate a team of care workers in the delivery of the agreed suite of services ensuring quality, timeliness and effectiveness of service delivery Competitive Salary Package arrangements are available. Key Duties Coordinate services and activities based on the client s individual assessed needs. This may include the coordination of Transport Services, Meal services, Personal Care, Social Support, In-Home Respite, Domestic Assistance, Garden and Home Maintenance Manage, schedule and support the Community Support Workers in providing client care services, on a day to day basis To project manage installation of best practice in operational service delivery Key requirements Certificate IV in Community Services or Aged Care or equivalent qualification, and/or equivalent relevant experience Demonstrated interpersonal skills that enable you to effectively communicate and liaise with a diverse range of clients to provide a quality client service and maintain positive working relationships. 2 Job Title Division/Department Location Reports to Team Leader Home Support IUIH Home Support Hervey Bay Regional Home Support Manager Direct Reports Position Status Senior Support Worker Community Support Workers Ongoing, full time for 38 hours per week Date of Approval July 2016 ORGANISATIONAL CONTEXT The Institute for Urban Indigenous Health Ltd was established to provide a coordinated/integrated approach to the planning, development and delivery of primary health care services to Aboriginal and Torres Strait Islander populations within the South East Qld Region. The IUIH is a lead agency working in partnership with key stakeholders to support the effective implementation of the Council of Australian Governments Closing the Gap initiatives. The IUIH Aged and Community Care Service provides in-home care services to eligible clients in the SEQ Region and also provides services in the Wide Bay region in Hervey Bay and Maryborough. The Services will work with the local Indigenous community and relevant key stakeholders to increase access and provide a coordinated comprehensive care service with a focus on assisting the aged receive quality in house care. POSITION OBJECTIVE The primary purpose of this position is to in accordance with the clients assessed needs: Manage the contract relationship and arrangements with the Client/Carer and review ongoing service needs Coordinate a team of care workers in the delivery of the agreed suite of services ensuring quality, timeliness and effectiveness of service delivery The role will be required to perform in accordance with Client Care plans, the Community Care Common Standards and the IUIH Policies and Procedures.

3 GENERAL RESPONSIBILITIES The primary duties and responsibilities of this position include, but are not limited to: Service Planning Demonstrate practical and innovative leadership in the design implementation and evaluation of daily programs for clients Liaise with the Assessor and Community Nurses to ascertain Client needs Identify a range of services that enhance inclusion for elderly Indigenous persons who are residing in their own homes within the community Collaborate with other service providers to ensure effective service delivery, and increase community networks Coordinate and review service arrangements with Service Providers Assist in the planning, implementation, review and evaluation of programs Client Management In accordance with the Client s assessed needs; Develop a Client Contracted Care Plan that details agreed service delivery items, costings and review processes Liaise with the clients, carers/family to ensure services meet the individual needs of the client Maintain up to date Client Care plans to reflect required changes, within the scope of assessed needs whilst helping clients to get the best possible value from their funding Review client care plans and maintain update client information Undertake routine home visits, in a non-intrusive manner, seek client feedback/ liaise with client/carer and to perform spot checks (quality audits) and to ensure care delivery is of a high standard, aligns with current care plans, quality measures, and Community Care Common Standards. Timely and thorough investigation of incidents, complaints / concerns of the clients, their families and external stakeholders Demonstrate respect and understanding of the clients including issues of cultural appropriateness and antidiscrimination Conduct regular service satisfaction audits Provide reports on client usage against Care Plans and allocated budget Coordination of Care Delivery In accordance with Client Care Plans; Coordinate services and activities based on the clients individual assessed needs. This may include the coordination of Personal Care, Social Support, In-Home Respite, Domestic Assistance, Garden and Home Maintenance, Meal services and Transport Services, as required. Delegate care activities to appropriately skilled staff within current legislation and regulatory requirements. Manage, schedule and support the Community Support Workers in providing client care services, on a day to day basis Set staff rosters and manage day to day variations as required

4 Administration Ensure all client files are regularly and comprehensively updated along with implementation of Care Plans Complete required documentation for the relevant funding bodies Complete required Quality Assurance documentation Provide relevant statistics and reports as required by management. Attend program meetings as requested Self-Management & Team Contribution Maintain confidentiality in line with Code of Conduct & comply with all Corporate Policies and Procedures, observe and implement all Moreton ATSICHS Community Aged Care policies and procedures Participate in the performance review process, commit to own self development, maintain professional competencies Contribute to process improvement Contribute to effective team performance, work collaboratively with other team members Assist with induction and other training for other team members, as required Promote and ensure a safe and healthy work environment Work within a legal and ethical framework Demonstrate confidentiality in all matters relating to services provided to clients and service information Develop and Maintain Linkages Foster productive working relationships with other IUIH staff in the delivery of seamless support services Establish strong working relationships with, clients, their families, industry professionals, relevant Government Departments and agencies and members of the community SELECTION CRITERIA Demonstrated skill and/or ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander persons. An understanding of the health and social issues impacting upon indigenous people in the area Well-developed understanding of care delivery in community care setting and within continuous improvement Excellent organisational and problem solving skills Demonstrate respect and understanding of client issues including cultural appropriateness and antidiscrimination Demonstrate excellent oral and written communication skills Ability to complete required paperwork QUALIFICATIONS AND EXPERIENCE Minimum Certificate IV in Community Services or Aged Care, or relevant level of experience Certificate IV Frontline Management or equivalent or working towards Cert IV TAE or working towards - desirable Experience in leading a team in Aged care or Community care / Health care essential

5 PRACTICAL REQUIREMENTS Some work out of normal hours of duty may be required Current C Class Drivers Licence (Qld) and the ability to travel across South East Queensland, as required. Your AHPRA registration will cover the requirement for a Criminal History Check. If you are not an AHPRA registered health professional, a Criminal History Check (CHC) with no disclosable outcomes that preclude you from employment is required on commencement. All appointees are required to obtain and maintain a current CHC. Any changes to criminal history should be advised to the supervisor as soon as practicable.
