The data are presented on both county and state levels. 3 o o o o o o o HIGHLIGHTS Medi-Cal funded a total of 65,617 fee-for-service (FFS) abortions 1 in 1997 and paid an average of $329 per procedure, totaling $21,583,13 million for all abortions. There was an estimated 6,693 abortions for enrollees in prepaid health plans and managed care programs excluding Santa Barbara and San Mateo. The average monthly rate of abortions per 1, eligible women 15-5 years old was Approximately one-third, 32 percent, of Medi-Cal fundec;l abortions were performed for women 2-2 years old. Abortion services to hispanic females accounted for 31 percent of total abortions exceeding, for the fifth year in a row, that of white females. OBRA eligibles accounted for about ten percent of total abortions. Two thirds of the abortions, 67 percent, were done at a physician's office averaging 5 abortions per office location. The cost of an abortion performed on a hospital inpatient basis averaged $1,979 per abortion. Abortions done at a physician's office averaged $286 per abortion which is lower than that of abortions done at clinics and hospital outpatient. The number of Medi-Cal eligible women who received abortion services was 61,589. Ninety-four percent of these women had only one abortion in The term abortion as used in the report refers to induced abortions only. i

6 INTRODUCTION The number of induced abortions funded by Medi-Cal during 1997, on a fee-for-service basis, 65,617, is about 21 percent lower than in Medi-Cal payments for these abortions was 2 percent lower than in The decline is partly due to the implementation of health care plans (HCP) as explained in "Data Qualification" on page 2. Claims for services rendered to enrollees in these plans were not completed for the period of this report. The estimated number of non-fee-for-service abortions is 6,693. While the number of FFS Medi-Cal funded abortions decreased 21 percent, the number of eligible women ages 15-5 decreased 2 percent from 1996 to This resulted in about five percent increase in the rate of average monthly number of abortions per 1, FFS eligible women 15-5 years old. This is the first increase in the rate of abortions since Unfortunately, the total number of abortions for California women in 1997 is not available. The 1996 preliminary total of 28,18 was estimated by the Center for Disease Control (CDC). It reflects a three percent drop from the 1995 estimated total of 289,987. Medi-Cal funded an estimated 112,99 abortions during 1996, including an estimated total of 3,21 abortions for health care plans' enrollees. Accordingly, Medi-Cal funded abortions accounted for percent of the 1996 CDC preliminary estimate. 1

7 DATA QUALIFICATIONS This report discusses abortions which occurred during calendar year 1997 and appear on the Medi-Cal paid claims file between January 1997 and September The 21-month file was used in order to allow for delays in submitting or processing hospitals, clinics and physicians claims. Abortions performed for enrollees in health care plans (HCP), otherwise known as managed care, are not included in this report. Encounter claims for these HCPs, excluding San Mateo and Santa Barbara, are reported on sporadic basis and therefore are incomplete. Due to the limited reporting it is not known exactly how many abortions these plans have accounted for during Abortion estimates for non-ffs enrollees in counties where HCPs are implemented are provided in Table A below. 8 Medi-Cal FFS Eligibles, Induced Abortions, and Expenditures While total Medi-Cal FFS expenditures increased 91 percent during the past ten years payments for Medi-Cal funded induced abortions for the same period fluctuated from a low of $21,583,13 million in 1997 to a high of $.7 million in The 1997 total expenditures is 29 percent less than the 1988 total 1 The average monthly number of FFS eligible women ages 15-5 dropped 2 percent from 1996 to 1997, while the number of abortions for the same age group dropped about 21 percent. 9 Distribution of Medi-Cal Funded Induced Abortions by Age and Ethnicity Women ages 2-2 accounted for the highest percent of abortions among all ethnic groups. For the fifth year in a row, hispanic women have accounted for the highest percentage of abortions, 3 percent during 1993 and 199, 31 percent during 1995, 33 percent during 1996 and 31 in Prior to that white women accounted for the highest percentage followed by black women. Compared to 199, the percentage of abortions for women of Asian and Pacific Islands origin to total abortions has remained the same (five percent). 10 Distribution of Medi-Cal Funded Induced Abortions by Age and Aid Category As in previous years, eligible women in the aid category AFDC (families) accounted for the highest percentage of Medi-Cal funded induced abortions, 85.7 percent. The second largest group is the OBRA 1 which accounted for ten percent of total abortions, the same as in It is important to keep in mind that the majority of managed care plans do not cover services for OBRA and income disregard eligibles. The totals provided in these tables are strictly for FFS eligibles. Otherwise eligible amnesty aliens who are aged, blind, disabled, or under 18 are entitled to full Medi-Cal services. Those who are not, are entitled to restricted Medi-Cal services during the first five years of amnesty, and to full services thereafter. OBRA applies to other aliens such as undocumented and temporary visa aliens. They are only entitled to restricted Medi-Cal benefits. 11 Distribution of Medi-Cal Funded Induced Abortions by Type of Facility During 1997 the largest number of Medi-Cal funded abortions was performed at physicians' offices, accounting for 67 percent, followed by clinics, 19 percent. In comparison, fifty-nine percent of Medi-Cal payments for abortion services was to physicians. Abortions performed at physicians' offices were the least costly. At an average cost of $286 per abortion it was $25 less than the cost at a clinic and $219 less than the cost at a hospital outpatient department. The overall FFS average cost per abortion declined from $368 in 199 to $327 in 1996 (see Table 1) however, the 1997 average was half of a percent higher than the Even though only one-half of a percent of all abortions were performed at hospital inpatient departments, Medi-Cal payments for these abortions accounted for three percent of total Medi-Cal abortion expenditures, averaging $1,979 per abortion. The average cost per abortion, excluding those done in hospital inpatient departments was $32. 12 The number of clinics providing abortion services remained just about the same as in 1996; 191 clinics in 1997 compared to 189 in The number of abortions done at these facilities dropped sixteen percent. The number of all other types of facilities providing abortion services for FFS eligibles declined along with a noticeable decline in the number of abortions. Data on the number of abortions performed for enrollees in the HCPs are not completed and therefore we can only estimate such totals as done in Data Qualifications on page 2. 13 Frequency of Medi-Cal Funded Induced Abortions During 1997 a total of 61,115 FFS eligible females ages 15 to 5 had at least one abortion. Ninety four percent of these women, 57,336, had only one abortion. The average monthly percentage of women who had at least one abortion was less than one percent of FFS Medi-Cal eligible women ages 15 to 5. 14 County Distributions of Medi-Cal Funded Induced Abortions The following four tables display the distribution of abortion services by county. Individual county data do not necessarily reflect the same number and expenditure distributions which exist statewide. Data provided in Tables 7, 8, 9, 1 and 13 are distributed by county of eligibility, i.e., the county that appears on the Medi-Cal card of the beneficiary. It should be noted that this is not necessarily the county of residence at the time of service, since a beneficiary could have moved from one county to another, and her eligibility might have been transferred at the time the service was rendered. Data in these tables may also include services performed outside California for a Medi-Cal beneficiary. Data provided in Tables 11 and 12 are distributed by county of provider; i.e., county of occurrence. 9
