10 Affordable Care Act of 2010 (ACA) By 2014 the ACA is expected to insure 32 million currently uninsured Once fully implemented it is estimated 92% of Californians will be insured. Meeting #8 10
11 What is covered now under ACA Young adults can stay on parents insurance until age 26, and children can t be denied insurance due to pre-existing medical conditions. Ended lifetime dollar limits on coverage. Insurance companies cannot cancel your policy when you become ill. Seniors can get 50% discount on brand name drugs when they fall into the Medicare donut hole. Some wellness exams and most preventive care and screenings no longer require Richmond co-pays. 12 What is the CA Waiver? California s Five Year Bridge to Health Reform Increases access to health coverage and health care Mandates managed care for certain populations Promotes Public Hospital System Transformation Meeting #8 12
13 Low Income Health Plan (LIHP) Provides care to low-income citizens or permanent residents between the ages of 19 and 64 who are uninsured and ineligible for other publicly funded programs such as Medi- Cal and Healthy Families Two Programs: Medi-Cal Expansion (MCE) up to 133% FPL Medi-Cal eligible in 2014 fully federal funding Health Care Coverage Initiative (HCCI) 134% - 200% FPL - State Exchange with federal subsidy Meeting #8 13
14 Primary Care Medical Home Patients have a health care team that is tailored to patient s health care needs Coordinates the patient s care Integrated primary and behavioral health care Proactively provides preventive, primary, routine and chronic care Rely less on emergency department care Incur fewer avoidable hospital stays More satisfied overall Richmond patient Health experience. 15 Delivery System Reform Incentive Payments (DSRIP) DSRIP was designed to: Expand access to timely and appropriate care Enhance quality of health care Improve health for patients and communities Introduce and test key concepts of Health Reform Some CCHS milestones: collecting demographic data and comparing them to quality data measuring patient experience/including patients in process redesign and leadership teams introducing alternative "patient centered" models of care delivery improving patient safety through more stringent interventions Increasing primary care capacity new clinics extending clinic hours, virtual doctor visits. Meeting #8 15
