Susan is currently a Senior Consultant with. In this role she is contributing to upstream thinking in relation to innovation and service re-design and the development of meaningful products and processes. She works with organisations to identify the unknown and create resources and tools to translate ambiguity into opportunity. A key part of this work is developing the health care communities experimental discovery mindset for systems thinking implementation and better outcomes for clients. The workshop Integrated care means different things to different people. Be they a corporate director, service provider or a care recipient, a person s perspective is at the heart of any discussion about integrated care. Achieving integrated care requires those involved with planning and providing services to impose the patient s perspective as the organising principle in designing service delivery. Transforming the health care system will require multilevel integration of the like never seen before. Across the different regions in Australia, we need to organise ourselves into think tanks using groups and collaboratives, steering committees and consortia to engage in systems thinking and innovative design. We will achieve upstream thinking for macro and meso level implementation which will bring about innovative system reform and cost effective, efficient quality services at the micro level. This workshop will assist in identifying each delegate s potential role in the integration design landscape. You will take away a better understanding of: Integration Tactical tools for engaging in integration at macro, meso and micro levels Integrated-care (using examples) Why it s such a challenge How to nurture integration What you need to do to support integration *Refreshments provided Paul is currently the CEO of 360 Health and Community, a primary care and community organisation coordinating and delivering services across the state of WA. Paul has overseen significant growth in 360 s service footprint through acquisition and diversification of funding sources. The organisation operates as a social enterprise and now delivers over 200,000 services per year. Paul joined 360 from PwC s Australian Health Advisory Practice where he delivered a wide range of strategic, economic and operational projects across the health system. The workshop When it comes to sourcing income, most mental health services look to PHNs or traditional disability streams. This is understandable, they are the most obvious source of funds, but they re also the most highly sought after. Furthermore, the fiscal environment is tightening and CDC is becoming increasingly critical. The good news is, there are other sources of income available. This workshop will introduce you to the other income streams you can access to ensure your organisations remains sustainable, and delivers the services consumers need, in the new environment.
