4 ADvantage Certification Process Step 1: Call the ADvantage Intake Unit at: Step 2: ADvantage Intake Personnel will ask questions to find out if you might qualify for ADvantage Services. Step 3 & 4: You must financially and medically qualify. A financial worker will interview you, and a nurse will interview you. Step 5: ADvantage certification (if you qualify) ADvantage ADvantage

5 ADvantage Services When you apply for ADvantage by calling the ADvantage Services Intake Line or going to your OKDHS County Human Services Center, you will be scheduled for an assessment. A nurse will come to your home to complete the assessment. The nurse will ask you questions about your health and questions about what activities you can do. The nurse will ask you to choose what case management provider and home health provider you want to have if you are determined eligible for ADvantage. After the assessment, the nurse will determine if you are medically eligible for ADvantage. During the same time period as the nurse s assessment, an OKDHS social worker will decide if you are financially eligible for ADvantage. If you are financially and medically eligible, a case manager will work with you to decide what services you need. The case manager will also help to arrange the services. 6 Applying for an Aged, Blind, and Disabled (ABD) case with OKDHS Low income individuals with limited resources who are 65 or older or are persons with disabilities may qualify for: State Supplemental Payment (SSP) and Medicaid Step 1: Complete an application form. Take it or send it to your OKDHS County Human Services Center. To find your OKDHS County Human Services Center call the OKDHS Director s Helpline at What happens next: Your application should be processed within 45 days. If you qualify you will receive benefits. You will either establish direct deposit or receive a debit card for the State Supplemental Payment. You will receive a Soonercare card for Medicaid. If you do not qualify, you will receive a notice from OKDHS explaining why you did not qualify. Step 2: The local county worker will schedule an appointment for an interview to go over your application with you. The county worker may ask you to provide additional information in order to process your application. Aged, Blind, and Disabled (ABD) ABD

7 Applying for an Aged, Blind, and Disabled (ABD) cases Application may be completed at the OKDHS County Human Services Center. Helpful definitions: State Supplemental Payment (SSP) - States are required to pass along Cost of Living Adjustments in Social Security Income Benefits. The State Supplemental Payment is a small payment intended to help recipients with their cost of living. Medicaid A program funded by federal and state governments which pays for medical care for those who can t afford it. Another term for Medicaid is Title 19. Soonercare is the name of Oklahoma s Medicaid program. Individuals must be receiving a payment from Social Security. You can contact Social Security by phone at , 24 hours a day 7 days a week. You can speak to a representative 7 a.m. to 7 p.m. on business days. Services available include: Scheduling an appointment, changing your address, signing up for direct deposit of Social Security check into your bank account, and obtaining replacement Social Security or Medicare cards. For information about Social Security visit: Individuals qualifying for ABD cases must meet income and resource standards. Any payment received from Social Security is considered monthly countable income. Eligible individuals may not make over $716 in monthly countable income. Eligible individuals married to a spouse who is not eligible may not make over $1,054 in monthly countable income. Eligible Couples (Couples who are both individually eligible) cannot make over $1,095 in monthly countable income. Resources must be equal to or less than $2,000 for individuals and $3,000 for individual and spouse Aged, Blind, and Disabled (ABD) ABD

8 Supplemental Nutritional Assistance Program (SNAP- formerly food stamps ) Step 1: Complete an application form. Take it or send it to your OKDHS County Human Services Center. To find your OKDHS County Human Services Center call the OKDHS Director s Helpline at Step 2: The local county worker will schedule an appointment for a face-to-face interview. During the interview, a county worker will go over your application. The county worker may ask you to provide additional information in order to process your application. What happens next: Your SNAP application should be processed within 30 days. If you are eligible, you will receive your benefits. Your household will get an EBT (Electronic Benefits Transfer) card that you can use to purchase food. An OKDHS county offce worker will explain the proper use of the EBT card. If you are not eligible, OKDHS will send you a notice of why your application was denied. Supplemental Nutritional Assistance Program (SNAP) SNAP

What Will Happen After I Turn In My Application? The local county worker will schedule an appointment for a face-to-face interview. During the interview, a county worker will go over your application. Also, the worker will make sure OKDHS has all the information needed to determine if you are eligible for food stamp (SNAP) benefits. The following information may be needed to process the application: Identification card showing your name; Proof of your address; Social security numbers for everyone in your household; Proof of the amount of money you or anyone in your household has in any bank account; Proof of all of your household s income; Proof of legal alien status for anyone who is not a U.S. citizen; and Medical bills for anyone who is age 60 or older or who receives a disability check based on a full disability. Please ask interviewee if they are able to go to the OKDHS County Human Services Center. If they are unable to travel because they are homebound, contact the OKDHS County Human Services Center to see if alternate arrangements can be made. How Long Does It Take To Process An Application? If you are eligible, you should have your food benefits within 30 days. If you have little or no income or savings, you could have your food benefits within seven days. If you are not eligible, OKDHS will send you a notice to tell you why your application was denied. If I Am Eligible How Will I Get My Benefits? Your household will get an EBT (Electronic Benefits Transfer) card that you can use to purchase food. 10 Senior Citizen Hearing Aid Project (SCHAP) You receive: An appointment with an Audiologist 1 non-digital hearing aid is provided for the ear with the least hearing You also may receive: An amplified phone Step 1: Complete an application with verification of your total income and turn it in. If your income is above the guideline you will be required to make a co-payment. Step 2: You will receive a listing of Audiologists you can choose from. Step 3: Call and make an appointment to see the Audiologist. Step 4: The Audiologist will contact you for a fitting when they receive the hearing aid. Step 5: If you requested an amplified phone it will be mailed to you. Senior Citizen Hearing Aid Project (SCHAP) SCHAP

11 Oklahoma Department of Rehabilitative Services OKDRS Senior Citizen Hearing Aid Project (SCHAP) 1. Be at least 60 years of age. 2. Be a resident of the state of Oklahoma. 3. Be on a limited income. See guidelines Have a 35 decibel hearing loss in your better ear. 5. A SCHAP audiologist will administer a hearing test and determine your degree of hearing loss. Seniors may receive: 1. An appointment with an Audiologist non-digital hearing aid is provided for ear with the least hearing. (A non-digital hearing aid is the cheapest kind of hearing aid; they run approximately $600 retail) 3. An amplified phone (A letter from the Information & Assistance Specialist (I & A) or Audiologist explaining how the senior will benefit from the phone needs to be included. The phone has a 1 year warranty. There is a card in the box with a # to call if there are problems during the 1 year warranty period.) These 3 items (1. Audiologist appointment, 2. Hearing aid and 3. Amplified Phone) need to be written on the application. Because of high demand and limited funding, you might be put on a waiting list. 12 Sertoma Hearing Aid Recycling Program (SHARP) SHARP accepts donations of used hearing aids. These hearing aids are then recycled to provide hearing aids to qualified low-income people living in Oklahoma for a one-time $50.00 processing fee. Qualifying persons Have a hearing loss that would benefit from the use of a hearing aid as determined by an evaluation. Are low-income. Step 1: Call SHARP offce at Step 2: Make an appointment with an audiologist for a hearing evaluation. Step 3: Complete the SHARP Application. Attach hearing test no older than 6 months. Include one-time processing fee of $50. Step 4: Wait for notice of approval by the SHARP Board. (They meet monthly.) Step 5: Return to the audiologist for fitting of one behind-the-ear recycled hearing aid (The audiologist will contact the applicant for a fitting). Step 6: Continue with follow up visits to the audiologist as needed. Step 7: Return the aid to SHARP if it is no longer being used, so someone else will benefit from the aid. 13 Sertoma Hearing Aid Recycling Program (SHARP) Eligibility SHARP is a non-profit program funded by Sertoma and LaSertoma civic clubs. They accept donations of used hearing aids. These hearing aids are then recycled to provide hearing aids to qualified low-income people living in Oklahoma for a one-time $50.00 fee. Applications for Nursing Home residents are also available. Eligibility Persons with a hearing loss that would benefit from the use of a hearing aid as determined by an evaluation provided by an audiologist. The applicant has to have a hearing loss of at least 30 db in the better ear to qualify. Persons that qualify are low-income. Although there is no set income requirement, each applicant is considered individually, with income and expenses taken into account. Application Process Complete the application form and be willing to document financial need with tax returns, check stubs, etc. If currently residing in a nursing home, complete and return the Nursing Home Questionnaire. Attach your $50.00 processing fee with your application or request a fee waiver, in writing, on application. (Fee will be returned in the event you are determined ineligible.) If applicant does not have $50, they can request this fee be waived. Include request with the application. 1. Attach a copy of a recent (within 6 months) hearing evaluation by an audiologist. 2. Take care of any medical problems indicated by the hearing evaluation. 3. 14 OASIS Respite Respite is for caregivers who need a temporary break from full-time caregiving duties. Caregivers can use respite hours in the manner that best meets personal needs and desires. There is no income limit for: 1. Persons caring for individuals 60 and older. 2. Grandparents 55 or older raising grandchildren. Step 1: To apply for services call Oasis Respite: Toll Free In Oklahoma City Metro: Step 2: When you receive your application, fill out and return it. OASIS will forward the application to the appropriate office. Processing of your application may take several weeks, depending on availability of funds. You will be notified if you qualify or do not qualify. If you qualify, you will be notified about the dollar amount of vouchers you qualify for. OASIS RespiteÚ OASIS

15 OASIS Respite Respite is for caregivers who need a temporary break from full-time caregiving duties. (Interviewer explains Respite vouchers help caregivers pay someone to temporarily take their place as care provider. ) Caregivers can use respite hours in the manner that best meets personal needs and desires. (Interviewer explains Respite can be used to visit family or friends, to run errands, spend an evening at the movies, to take a vacation, or to just catch up on much needed rest. ) There is no income limit for: Persons caring for individuals 60 and older Grandparents 55 or older raising grandchildren How much voucher money will be received? The voucher amounts for respite services range from $200 to $400, depending upon the funding source and availability of funds. The vouchers are good for three months. Once the vouchers expire, the caregiver can reapply by contacting OASIS. Vouchers are given to caregivers to buy respite from whomever they wish, as long as the person chosen is at least 18 years of age and does not live in the home of the caregiver or care receiver. If the caregiver does not have anyone to provide respite care, OASIS may be able to assist in locating a provider. 16 Low Income Home Energy Assistance Program (LIHEAP) Step 1: Apply at a local OKDHS County Human Services Center during the designated application period. Step 2: You will need to bring a copy of your bill. If you get a gas bill, you need to bring your gas bill. If you have total electric it will be your electric bill. Step 3: You will need to verify your income and resources. What happens next? If you are eligible, you will receive an approval letter telling what you are eligible to receive. 17 Low Income Home Energy Assistance Program (LIHEAP) Apply at a local Oklahoma Department of Human Services (OKDHS) County Human Services Center dur ing the designated application period. All application periods are dependent on available funding. To find out when the designated application periods are, watch local news for announcements or call an OKDHS County Human Services Center. Heating assistance applications are usually taken the first working day in December. Energy crisis assistance applications are usually taken for 2 weeks starting the first Monday on or following March 15th. Summer cooling assistance applications are usually taken for 2 weeks starting between July 15th and August 1st. Four of the main eligibility requirements for LIHEAP assistance are: 1. The number of people in your household; 2. Verification that you pay your heating expense (provide a current heating bill); 3. Your household income does not exceed 130% of the Federal Poverty Guidelines; 4. The household s cash resources do not exceed $2,000 for 1 person; $3,000 for 2 persons add $50 for each additional person in the household. 18 Calling the Hotline for Legal Aid Services of Oklahoma, Inc. Hours of operation: 8:30 a.m. until 5 p.m. Monday through Friday, except holidays. Step 1: Call to leave a message for a Legal Aid Services application specialist. You will press 2 twice to get to the Senior Law Project. Step 2: Listen to the message. When the recording tells you to leave a message leave your name, the county you live in, your daytime phone number including the area code. Leave a brief description of your legal problem. Please say your phone number slowly twice. If you are calling on behalf of someone over 60, please leave your name, and phone number and the name of the person you are calling for. Please say your phone number slowly twice. If you have an address, please leave the full address, saying it slowly twice. What happens next: You will get a return call from an application specialist within 2 business days. They will ask you some questions to determine if you qualify. If you qualify they will refer you to an appropriate staff person who will contact you. You may also call the local offce. You will not be turned away if you identify yourself as a senior. Title III Seniors Program (seniors aged 60+ qualify) The Senior Law Project provides free legal advice, counseling, and community education for those who are 60 years of age or older. 21 Older Blind Independent Living Program The Older Blind Independent Living Program expands independent living services for people who are 55 years old and older and legally blind. Rehabilitation teachers located in Division of Visual Services offces throughout the state provide one-on-one assistance to older people in adjusting to blindness and regaining or maintaining maximum independence and self suffciency. The Older Blind Program operates a telephone line which provides recorded information about grocery ads, newspaper articles, television guides and other publications.
