2 KEY TASKS EXPECTED OUTCOMES Service Delivery Services are delivered according to contracted obligations and boundaries Ensures services are child and family centred and age appropriate Systems and processes are in place to facilitate teamwork across all disciplines Develops a resource pack for families of children diagnosed with ASD, in consultation with clinicians and families Keeps abreast with Ministry of Health communications Works with other ASD co-ordinators to share information Is involved in the assessment process for children with suspected ASD at Assessment the request of the clinician coordination Gathers information from a variety of agencies in order to assist the diagnostic process Manages the ASD virtual team as and when required Identifies possible duplications in assessment processes and works with professionals to eliminate all real and unnecessary duplications. Clinical case work Post-diagnosis service coordination Plays a role in the functional assessment of the child when required. For children diagnosed as having an autism spectrum disorder the Coordinator will: 1. ensure a management plan is put in place that centres around the developmental needs of the family within the context of their family/whānau /fono circumstances 2. ensure that the family and likely service providers (e.g. respite providers, behavioural support providers) are involved in the development of the plan 3. ensure that the family are given prime responsibility for the maintenance of the plan 4. support the family at planned intervals to ensure that the developmental plan is kept up-to-date and relevant to the changing needs of the person with autism (such support is likely to include liaison with health and education professionals). Discharges children from the service in a timely manner Quality of Service Reviews systems /processes as and when required Contributes to the development and update of policy and procedures. Feedback from consumers is sought in conjunction with the Consumer Advisor and is utilised to inform quality improvement initiatives. Ensures that complaints and incidents are investigated and responded to according to policy. This includes (but is not limited to) investigating, analysing, acting, planning and reviewing. Contracted quality standards are met. Audits are carried out in a planned way and outcomes are utilised to inform quality improvement initiatives. 3 KEY TASKS EXPECTED OUTCOMES Documentation Is responsible for setting up appropriate documentation processes to meet contractual requirements Performance Monitoring Effective Communication Effective Partnerships Completes reports in line with contractual obligations and at the request of senior management Staff receive appropriate information quickly and consistently Regular communication forums for staff are available Communicates with clients when required Consumers have a clear understanding of the service provided Communication with service users, referrers and other providers is comprehensive and occurs at regular intervals. Develops a Memorandum of Understanding for partners involved in ASD service Develops relationships with community agencies to facilitate close coordination and collaboration Liaises with other services and agencies, as appropriate. Makes contact with community organisations, other services and parent groups where appropriate and as casework demands. Meet with other agencies, where appropriate, to exchange ideas, establish needs and develop a network of information resources. Service transitions achieve the best outcomes for children and families. To recognize Individual Responsibility for Workplace Health and Safety under the Health and Safety in Employment Act 1992 WDHB Health and Safety policies are enacted according to agreed plans. Workplace hazards are identified and reported, including self management of hazards where appropriate.
