1 Butterfly Project Signals Tool Many people using Dementia Care Matters Inspiring Action 50 point checklist and Dementia Care Matters Measuring Outcomes Matters (MOM) tool within Dementia Care Matters Butterfly Projects have asked for a practical, simple measurement tool on the projects progress. The Signal tool has been devised to monitor project progress either by a Dementia Care Matters Consultant or for in house use by staff managing or working within a Butterfly Project care home. Please circle the relevant traffic light to indicate current progress on each of the project outcomes: Red = not started/not in place Amber = commence/in progress Green = achieved/fully implemented The project is divided into 4 quarters i.e. a three month period per quarter in which the outcomes below need to be in place to be likely to achieve Dementia Care Matters quality of life kitemark. 1 First Quarter Managers baseline appraisal on passion/project leadership potential 2 Nurse teams capacity to lead project assessed Individual nurses appraised on being a new culture nurse 3 leading person centred dementia care 4 Brochure/families briefed on person centred model 5 Staff express home s one key purpose 6 Small scale domestic living maximum 12 people 7 Staff wearing their own clothes 8 All toilets communal
