2 1. Welcome & apologies L Thomson welcomed those in attendance, apologies were noted. 2. Minutes of Previous Meeting The minutes from the meeting on 05 th December 2014 were accepted as an accurate record. 3. Matters Arising Terms of reference A. Adamson has confirmed the remit of the group. Health Literacy A definition of health literacy has been circulated to members along with a copy of Louise Hammell s thesis. 4. Progress Reports Lindsay informed that she has recently attended the Scottish CPA Network meeting to update members on the NPHN Mental Health sub group report. The remit for the meeting is to find out how far leads for the implementation sub groups have got and what the Health Boards are doing. Lindsay also highlighted that after today only one further meeting will be required. 4.1 Competencies John Porter & Jayne Miller Members noted the update paper, John Porter informed there have been a number of meetings with Jane Cantrell at NES to establish the core skills, learning issues and population of the portal. Members agreed that the current resources populated are relevant. The following actions and recommendations were agreed:- ACTIONS Education packages on NES portal. List as stated plus to include New to Forensic suite of programmes (New to Forensic, New to Forensic for Psychological Therapies, New to Forensic Medicine). Text should also include that this is learning based with a mentor. Psychiatrist competencies to be adopted by Forensic Network and NPHN Check NES view via Jane Cantrell on development of competencies for other groups Competencies of other groups to be considered by Forensic Network professional groups. If relevant, to be adopted by Forensic Network and Prison Network 4.2 Independent Advocacy Tom Jackson & Louise Hammell Tom Jackson & Louise Hammell were unable to attend the meeting, John Porter will link with Tom as advocacy is essential. A list of boards that have prisons and advocacy need to be identified as it is a legal requirement, information will be shared with Scottish Government, Mental Welfare Commission and each Health Board. The following actions and recommendations were agreed:-
3 The results from the questionnaire analysis TOM JACKSON If no results telephone round all 15 Prison Healthcare Managers to find our if advocacy is available Give advocacy position to Board, SPS and Mental Welfare Commission. 4.3 Telemental Health Cathy Dorrian Cathy Dorrian informed that HMP Perth has been very proactive within mental health with discussions ongoing to set up Tel. CBT from NHS 24. Cathy also highlighted that there hasn t been much progress with HMP Shotts in terms of the TV channel, however video clips have been sent to Jim Kerr and Gary Tanner for approval. There is also a meeting scheduled with Linda Graham in relation to beating the blues as a stand alone service via CD's which prisoners can use on the new Prison PC's. Overall the 3 main prisons are being more proactive and 6 are operating with video conferencing really well. Members in attendance discussed Cathy s update paper and what is currently available in each prison. It was suggested a table should be prepared that includes the following headings: health board, prison, available technologies, main contact and way forward. Cathy agreed to populate. ACTION: CATHY DORRIAN Cathy informed that she is moving post, and suggested a named contact would be required to take the Telemental Health work forward. Stella Clarks post has also been advertised. Lindsay agreed to write to Moira McKenzie, Head of Telecare Development SCCT to make sure custodial healthcare remains part of the strategy, and to also inform that input from Cathy Dorrian has been valued. 4.4 Comorbidity Tom Byrne T Byrne was not in attendance to discuss. Lindsay will schedule telephone call to discuss progress to date. 4.5 Needs Assessment Jayne Miller & John Porter David Thomson has now moved post and is no longer able to be part of the group. John Porter and Jayne Miller will now take this section forward. Members discussed if National Needs Assessment should take place again or if the International Meta Analysis data should be used (25000 prisoners), Cooke 1994 or Davison John agreed to draft a paper for the Advisory Board regarding National Needs Assessment and will include a list of options for consideration, showing advantages and disadvantages, costs will also be included in this paper, Lindsay agreed to help John with the costs. The inappropriateness of Vision was also discussed, this should be explained to the boards. A suggestion to the ongoing technology problems was the tool Andrew Forrester presented at the Clinical Forum Prison.
4 4.6 Service Mapping There is currently no lead for this section as Jim McGuinness has now retired. Lindsay has received response from Jim Crichton informing there is no national recipe for national community mental health team. Lindsay also informed that the Sandy Simpson papers would be very beneficial for this section. Lindsay will also link in with Linda Irving regarding service mapping in prisons and standards. A copy of the questionnaire completed by Jo Brown was also circulated to members, this does not fulfil the need of the group. It was agreed that a baseline paper is required showing the number of prisoners per staff and the composition of a mental health team. John agreed to raise this issue at the next 5 Nations meeting, Lindsay also agreed to contact Paul Gilberton? & Nicola Swinson will liaise with Professor Jenny Shaw regarding types of staff to prison ration, and formula elsewhere. Louise will send Nicola page 19 from the NPHN MH Sub Group report. ACTION: NICOLA SWINSON & LOUISE BYRNE Cathy Dorrian will send details for Katharine Bissett to Louise to find out if she has any information that can help. John Porter will ensure that the key messages from the Mental Welfare Commission Mental Health of Prisoners: Themed Visit Report into Prison Mental Health Services in Scotland (2011) are met. 4.7 Models of Care Melanie Baker Melanie has received the standards from Craig Stewart re: ICP. Melanie also informed that she is now part of the Scottish Patient Safety Programme which is being piloted in Glasgow, this will involve community mental health teams in the next 18 months and therefore may have possible relevance to prisons. In terms of the recommendation for standardised process for family and carers to liaise with a prisoner mental team, Melanie agreed to develop signpost to mental health service/professional if concerned about a relative in prison. ACTION: MELANIE BAKER The prison mental healthcare standards will be an issues to be considered by the NPHN board, John Porter agreed to discuss this with Jayne Porter and Lindsay with Linda Irving. The HMCIP report will also be checked for the health standard. & LINDA IRVING The defined governance arrangements should be in place Memorandum of Understanding NHS and SPS. John Porter will check this and inform Melanie. 4.8 Learning Disabilities Fergus Douds & Rosemary Duffy Members noted the update papers from Fergus Douds & Rosemary Duffy. Lindsay will follow up with Fergus regarding the Implementation of No-one Knows, and progress re awareness, screening and directory.
5 4.9 Problem Behaviours Nicola Swinson Nicola informed different discussion have already taken place looking at difficult to manage people within prisons. The SOLS team are in the process of going back to being the Sex Offender Liaison Service due to funding issues. A protocol will be available for the next sub group and will be circulated at the end of April. ACTION: NICOLA SWINSON Nicola also highlighted that it has been difficult to engage with psychologists in prison, this will be run past Paul Davidson and Rachel Roper. Nicola has also been in contact with Dr Jon Patrick regarding MBT for prisoners, there have been implications for staff receiving training due to the amount of time spent on mandatory modules. It was recommended to await outcome of evaluation from MBT, Nicola will find out when this will be. ACTION: NICOLA SWINSON It was also noted that there many be a conflict of models as Dr Adam Polnay is delivering mindfulness training to patients and staff Placement of Prisoners Craig Stewart & Lesley McDowall Craig was unable to attend the meeting, members noted the update paper and discussed current issues with patients moving between prisons. John Porter will follow up on progress with Craig. 5. Any other business Cathy informed that Adam Mahoney has had success in running mindfulness, and that the Mastermind online CBT is developing well. 6.
