STATEMENT OF LEGAL CONTROL AND GOVERNANCE The Salter School of Nursing and Allied Health is owned by Premier Education Group, L.P., registered limited partnership in the Commonwealth of Pennsylvania. PROGRAM DELIVERY All programs and courses offered by the Salter School of Nursing & Allied Health are offered on campus and taught residentially, except for clinical hours, which are taught at appropriate clinical sites. 5 Welcome to The Salter School of Nursing and Allied Health! The Salter School of Nursing is an accredited private vocational school with a long history of providing career education in the fields of nursing and allied health. We specialize in comprehensive, short term programs, which prepare our students for interesting and challenging positions in their chosen fields of study. The Salter School of Nursing and Allied Health programs are constantly reassessed so that we may continue to be responsive to the needs of the communities we serve. Studies done by the U.S. Department of Labor indicate that the demand for trained workers in these areas will continue to grow in the future. Today, a high school education is considered only the bare minimum for employment. If you want more than an average job, a higher-than-average salary, and the potential to move up the ladder of success, you need the advanced education and skills that will make you an above average employee. There was a time when a college degree was the only key to this success; today, the situation is different. It is not necessary to spend several years in college in order to be a part of one of the fastest growing career fields. Our success is a result of the successful graduates who have benefited from our programs; the history of their achievements forms the foundation on which today s student set their goals. 6 MISSION AND PHILOSOPHY STATEMENT The Salter School of Nursing and Allied Health is dedicated to the provision and promotion of quality, career-oriented educational opportunities for Nurses, Nursing Assistants, and Patient Care Technicians through classroom participations, hands-on laboratory experience and applicable on-the-job training in clinical opportunities. It is our belief: That the courses offered be designed to provide each participate with the knowledge and skills that will foster successful employment in the health care field. That the schedule of courses be designed to offer each participant the opportunity to choose a career path. That an equal opportunity for education and employment be afforded to each participant without discrimination on the basis of race, color, religion, national and ethnic origin, gender, age, sexual orientation, disability, veteran status, or marital status. The Salter School of Nursing and Allied Health fosters a career advancement program, beginning with basic academic skills and an opportunity for those with existing family and/or work responsibilities to acquire a health career education in a non-traditional scheduling format, offered on various schedules. CAMPUS DESCRIPTION The Salter School of Nursing and Allied Health is located on the 4th floor of the historic Jefferson Mill building located on N. Commercial Street in the heart of downtown Manchester, NH. The building is a prominent historical brick mill that has been renovated to showcase the hardwood floors, wooden beams and history of the mill in its heyday. The facility consists of 27,351 square feet and houses 13 classrooms, 2 Computer labs, 1 EKG lab, 1 Maternal/Child Health lab, a mock-operating room fully appointed with appropriate simulation equipment, a Patient Care Technician/Licensed Nursing Assistant lab, phlebotomy lab, two private patient rooms, and three open nursing labs/classrooms. Our campus also has a library with computers for student use, a student lounge, ample bathroom facilities for both males and females, and several administrative offices. All rooms are heated/air conditioned. All classrooms and labs have ample tables and chairs as well as whiteboards and other audio-visual equipment. HISTORY OF THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH The Salter School of Nursing and Allied Health was founded in 1997, by Health Career Training Associates, LLC, and a small post-secondary proprietary school with a Nursing Assistant Program. The primary focus of the Health Career Training Associates School was a competency-based vocational program for nursing students. The Practical Nursing program was developed in 2002 and received approval by the NH Board of Nursing in February of In July of 2007, they gained accreditation through NLNAC (National League for Nursing Accreditation Commission), now called ACEN (Accreditation Commission for Education in Nursing). At this time, they changed the name of their school from HCTA to NESPN (New England School of Practical Nursing) to clearly identify its purpose in the title within the organization. In December of 2009, The New England School of Practical Nursing was purchased by Premier Education Group (PEG). The name of the school was changed to The Salter School of Nursing and Allied Health and remained at the 60 Rogers Street location until February In February 2012, Premier Education Group relocated the campus to 670 N. Commercial Street, Manchester, NH within the historic Jefferson Mill Building. The change in location allowed the students additional classroom space, well-equipped labs and a pleasant neighborhood surrounding other schools of higher learning. 3 P a g e

Clinical Experience hours may be scheduled at any time, day or evening, weekday or weekend, depending upon the needs of the clinical sites. Students are expected to be present at least 15 minutes prior to start time. The Salter School of Nursing and Allied Health reserve the right to alter the class schedule of hours and days of attendance as needed. When a schedule is altered, all in-school students will be expected to adhere to the new schedule. SCHOOL HOLIDAYS AND SCHOOL CLOSINGS Classes canceled for holiday observance on a Monday are usually made up on Friday of the same week. The school reserves the right to utilize an extended day until 5:30 p.m., occasional Fridays, or even Saturdays as make-up for snow days or other unusual circumstances. Evening classes regularly meet Monday through Thursday. Evening classes canceled for holidays, weather, or other reasons are generally rescheduled for Friday evenings. Schedules will vary according to program and start date selected. For a complete listing of the school s yearly Holiday Calendar, please refer to the Holiday Calendar addendum. In the event of inclement weather, delays or cancellations will be announced through the channels/websites listed below. The decision to cancel school will be made by 7:30 a.m. for day classes and by 4:30 p.m. for evening classes. Television: Channel 9 (WMUR) Internet: & Radio: WZID (95.7FM) CAMPUS SECURITY In compliance with the Student Right to Know Security Act, Public LAW , as amended by the Higher Education Technical Amendments of 1991, Public LAW , The Salter School of Nursing and Allied Health provides campus crime statistics and procedures for reporting and deterring criminal activity. Students may secure the crime data by going to Click on Open the Campus Security Statistics website, go to the State section and pick the school s state, click on search at the bottom of the screen, and select the school. Students unable to access this information are advised to see their Director to obtain a printed copy. The following websites may be used to identify those persons registered as sex offenders in New Hampshire CONSUMER INFORMATION THE STUDENT RIGHT TO KNOW Students can request to see the following consumer information from the school, which is available in either the Admissions, Financial Aid, or Campus Director s office: evidence of accreditation and state licensing; the cost of attending, as well as the institution s refund policy (including examples of refund calculations) in the event of withdrawal or termination; what financial assistance is available to students and how recipients are selected; how financial need is determined, including how the costs for education are calculated; information about loans, including the interest rate, the amount that must be repaid, when repayment will begin, and the conditions for deferment if necessary; how satisfactory academic progress is determined by the institution and the consequences for failing to meet those standards; information about campus security; and student completion and placement rates. 8 SEXUAL HARASSMENT STATEMENT AND POLICY Federal and state law (Title VII of the Civil Rights Act of 1964) prohibits sexual harassment. All The Salter School of Nursing and Allied Health students and faculty have the right to study/work in an environment free from all forms of intimidation or sexual harassment. The Salter School of Nursing and Allied Health is committed to maintaining a work and learning environment that is free of discrimination. Harassment consists of unwelcome conduct (physical, visual or verbal) that is based upon a person s protected status, to include sex, race, color, national origin, age, religion, ancestry, or disability. Sexual Harassment is improper and unlawful conduct that undermines the learning or work relationship as well as individual morale. The basic criteria established by the Equal Employment Opportunity Commission help to define the possible actions that constitute unlawful behavior. They include, but are not limited to: The conduct has the purpose or effect of substantially interfering with an affected person's work performance or creating an intimidating, hostile, or offensive work environment. Submission to or rejection of the conduct is used as a basis for an employment decision affecting the person rejecting or submitting to the conduct. Submission to the conduct is either an explicit or implicit term or condition of employment. Obligation to Report The Salter School of Nursing and Allied Health must be made aware of sexual harassment or related retaliation. Therefore, anyone who believes that she or he has experienced or witnessed sexual harassment or related retaliation should promptly report such to the Campus Director. The Grievance Policy will be applied as appropriate. If an investigation is conducted, the complainant and the respondent shall each have the right to: Receive written notice of the report, including a statement of the allegations, as soon after the investigation begins as possible. Present relevant information to the investigators. Receive, to the extent permitted by law and after conclusion of the investigation and appropriate review, a copy of investigator's report. HAZING AND BULLYING POLICY Hazing and/or bullying are forbidden at The Salter School of Nursing and Allied Health and may result in disciplinary action up to and including termination from the school. The act of hazing occurs when activities or acts that initiate a person into any student organization may result in willful or reckless endangerment to the physical or mental health of the student. Such activities may include forced consumption of, or deprivation of, food, liquor, beverage, or drug, and physical attacks such as beating, branding, whipping, and forced calisthenics. Bullying can be verbal, physical, or conducted via telephone or Internet. Bullying in any form against staff, faculty, or students is forbidden and may result in disciplinary action up to and including termination from the program. FELONY DISCLOSURE Please be advised that if you have been convicted of any criminal offense you may not be eligible for certain clinical experiences, externships, internships, or certifications associated with our educational programs. Those with non-felonious criminal backgrounds may also find it difficult to secure employment within the health care setting. *Please be advised that if you have been convicted of any criminal or misdemeanor offenses, you may not be admitted to the program. Once admitted to the program, students subsequently convicted of crimes while attending The Salter School of Nursing and Allied Health may be dismissed from the program. It is the student s responsibility to report changes in his/her criminal record to the Campus Director. 5 P a g e

9 CONFIDENTIALITY AND PRIVACY STATEMENT FAMILY EDUCATION RIGHTS AND PRIVACY ACT At The Salter School of Nursing and Allied Health we care about your privacy. Under the Family Educational Rights and Privacy Act (FERPA) of 1974(PL ) as amended by PL , The Salter School of Nursing and Allied Health will provide eligible students or parents of minors with the opportunity to review the student s educational record, to seek correction of information contained in those records, and to limit disclosure of non-directory information from the records. FERPA also provides that an educational institution may not release information about students nor give access to any unauthorized parties without the student s written consent. The Salter School of Nursing and Allied Health requires students to give the school written permission before any non directory information will be released to a parent or non-authorized party. Release forms are available from school staff. FERPA allows schools to disclose non-directory records without student permission to the following parties or under the following conditions (34CFR 99.31): School officials with legitimate educational interests Other schools to which a student is transferring Specified officials for audit or evaluation purposes Appropriate parties in connection with financial aid to a student Organizations conducting certain studies for or on behalf of the school Accrediting organizations Department of Veterans Affairs Internal Revenue Service in compliance with the Taxpayer Relief Act To comply with a judicial order or lawfully issued subpoena Appropriate officials in cases of health and safety emergencies Federal, State and Local authorities pursuant to specific federal and state laws. Students may also request that their directory information not be released (name, address, telephone, dates of attendance, program, honors, awards, etc.) by submitting a request in writing to the Financial Aid office. Students have the right to inspect and review their records, as well as request amendment of records which they believe to be inaccurate or misleading. Procedure requires that the student make an appointment with the Campus Director to review the documentation in his or her file; the school will need to comply within a maximum of 45 days. STATEMENT OF NON-DISCRIMINATION/AFFIRMATIVE ACTION/EQUAL OPPORTUNITY The Salter School of Nursing and Allied Health is an equal opportunity career education program. If an applicant has an obvious disability such as mobility, sight or hearing impairment, the applicant will be given the institution s confidential disability declaration form and voluntarily disclose the disability. No applicant is required to do this. The only instance when the institution may impose screening criteria is when admission presents a clear safety risk to self or others and these criteria are based on actual risk assessment, not stereotypes or assumptions. The Campus Director is responsible for the determination of risk and decision for admission. Every student accepted into a school program has the opportunity to voluntarily and confidentially disclose/declare a handicap/disability/special need(s) and as result, allows for disability verification and resulting appropriate academic accommodations. These accommodations can occur through a combination of school, governmental, public and private, voluntary and confidential services available to them. 6 P a g e

Students will receive advance notification of the date and time of orientation. It is each student s responsibility to become familiar with the policies and guidelines of the school, which are contained in this catalog, official memos and addendums. 7 P a g e

11 ADMISSIONS REQUIREMENTS The Salter School of Nursing and Allied Health has an open enrollment policy and accepts applications on an ongoing basis. To ensure that only qualified students are accepted into our training programs, all applicants must meet the following Criteria for Admission. All required documentation must be submitted to Admissions before the admission process is complete. All applicants must: A minimum age requirement of 17, prior to the start of classes. Possess a high school diploma or GED. Official high school or GED/HiSET transcripts are required for all Practical Nursing applicants. For all other programs, a copy of a valid high school diploma/transcript, GED/HiSET transcript or attestation of high school/ged completion is acceptable. *For foreign diplomas, translation and assessment of equivalency to a U.S. high school diploma will need to be submitted. The Admissions department can refer you to authorized validation service providers. Complete an entrance interview and take a tour of the school. Complete an Enrollment Agreement with a $25.00 application fee. It is THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH policy that all applicants submit a State of New Hampshire and National criminal background check (CORI-Criminal Offender Record Information/SORI-Sex Offender Registry Information), received prior to class start date. THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH will fund the cost of the National criminal background check, it is the applicant s responsibility to submit and pay for the NH State Background Check. *Please be advised that if you have been convicted of any criminal or misdemeanor offenses, you may not be admitted to the program. Once admitted to the program, students subsequently convicted of crimes while attending THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH may be dismissed from the program. It is the student s responsibility to report changes in his/her criminal record to the Campus Director. Pose for photos for ID badge. Meet with the financial aid office within 3 calendar days of signing the enrollment agreement and produce all required documents for full financial packaging prior to starting school. The Salter School of Nursing and Allied Health requires applicants to have an assessment of their levels of learning. The Wonderlic Scholastic Level Exam (SLE) identifies an individual's capacity for learning, as well as the ability to apply knowledge. 12 ADDITIONAL ADMISSIONS REQUIREMENTS FOR PRACTICAL NURSING APPLICANTS: Practical Nursing Applicants must also pass the TEAS (Test of Essential Academic Skills) with a composite score of 55% or above prior to entering the program; nursing applicants must complete all 4 sections (Reading, English, Math and Science) of the TEAS, Version V. Applicants are permitted to take the TEAS exam up to two times within a 6-month period; no sooner than two weeks between attempts. The 6-month waiting period is based on the date of the second unsuccessful testing attempt. All four sections must be taken each time the TEAS, Version V is taken. If applicants have taken the TEAS, Version V through ATI within the past 12 months and obtained a composite score of 55% or higher, they may request these results be submitted directly from ATI to The Salter School of Nursing and Allied Health for review and acceptance. Students are encouraged to go to the ATI website for practice questions prior to taking the TEAS. A study guide and online practice test are available to prepare for the TEAS. The TEAS test fee is $60 payable by cash, certified check, money order, or credit/debit card. The exam fee is non-refundable. Applicants who miss the scheduled exam date and do not call will be required to pay the full fee in advance to reschedule a new test date. All Practical Nursing program applicants must provide the following prior to their start date. The Campus Director is the only designated person who can make an exception to these policies. Exceptions are generally made only for those applicants who have enrolled within 2 weeks of the start date. A Health/Physical Examination completed and signed by a qualified healthcare provider within the past 12 months from the start of the program Titres, showing immunity for the following: Measles/Mumps/Rubella (MMR), Varicella (chickenpox), Hepatitis B (optional) or a signed Hepatitis B risk waiver and/or signed declination. Proof of Diphtheria, Tetanus, acellular Pertussis (DTaP) vaccination Annual PPD (purified protein derivative) status for tuberculosis exposure. Must have a 2- step PPD Proof of Health Insurance Coverage Proof of Personal Liability Coverage Annual flu vaccine Provide proof of BCLS Healthcare Provider CPR Certification (American Heart Association or American Red Cross) If seeking a review of transfer credits, submit official transcripts from institutions of higher learning previously attended. These should be received two weeks prior to the first day of class of the first term in order to be considered for transfer credit IMPORTANT NOTICE: Students will be required, in accordance with clinical site requirements, to provide a 10-panel drug screening with expanded opiates and an updated NH State Background Check; The Salter School of Nursing and Allied Health will assume these costs. Students may also be required, in accordance with clinical site requirements, to provide an updated health exam, an up-todate flu shot, and/or additional titres at any time during the course of the program dependent on clinical site requirements; these costs are the responsibility of the student. If a student s drug test is returned as positive, the student will not be permitted to attend clinical training due to clinical site requirements. Withdrawal from the program may be necessary and re-entry into clinical courses will be dependent upon the student producing a negative 10-panel drug screening at their own expense. ACCEPTANCE INTO PRACTICAL NURSING A student is considered to be qualified for acceptance into the next available nursing cohort when her/his application file is complete. Acceptance of a qualified student is determined on a first qualified, first admitted basis. 9 P a g e

13 ADDITIONAL ADMISSIONS REQUIREMENTS FOR PATIENT CARE TECHNICIANS AND LICENSED NURSING ASSISTANT APPLICANTS: All Patient Care Technician and Licensed Nursing Assistant program applicants must provide the following prior to their start date. The Campus Director is the only designated person who can make an exception to these policies. Exceptions are generally made only for those applicants who have enrolled within 2 weeks of the start date. A Health/Physical Examination completed and signed by a qualified healthcare provider within the past 12 months from the start of the program. Provide proof of ability to lift 50 pounds alone or 100 pounds with another person, confirmed by a qualified healthcare provider s signature as part of the required physical exam Immunization Record demonstrating current vaccinations for the following: Measles/Mumps/Rubella (MMR), Varicella (chickenpox), Hepatitis B (optional) or a signed Hepatitis B risk waiver and/or signed declination. Proof of Diphtheria, Tetanus, and acellular Pertussis (DTaP) vaccination, according to the immunization records form that THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH will provide to you Annual PPD (purified protein derivative) status for tuberculosis exposure must be provided IMPORTANT NOTICE: Students may also be required, in accordance with clinical site requirements, to provide an updated health exam, random drug screening, flu shot, proof of health insurance, personal liability insurance, and immunization or titre at any time during the course of the program. The expenses of these potential items are the responsibility of the student. ACCEPTANCE Upon completion of the required documents for admission, the applicants are notified in writing of their acceptance. TRANSFER REQUIREMENTS Premier Education Group schools employ practices in line with other higher education institutions in assessing credits from other schools or colleges that are recognized by the US Department of Education. Credit transfer determination is made before the student s start date; The Campus Director may be able to approve an exception if a special circumstance prohibits a determination from being made prior to a student s start date. Hours/credits will be considered for transfer hours/credits toward graduation if the following criteria are met: 1. The Official Transcript must be accompanied by a catalog of the institution, which describes the courses for which transfer hours/credits are sought. 2. Course is adjudged equivalent in educational content, hours and/or credits. 3. For the Practical Nursing program, a grade of B- or better was achieved. For the Patient Care Technician program, a grade of C- or better was achieved. (Does not apply to PEG transfers) 4. Course was completed no more than 5 years prior to date of request. 5. The following nursing courses must be taken at The Salter School of Nursing and Allied Health due to differences in nursing course content: PN101, PN102, PN103, and PN104 TRANSFER OF CREDIT EARNED AT THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH TO OTHER INSTITUTIONS: The Salter School of Nursing and Allied Health does not suggest, imply or guarantee any transferability of courses or credits. The transferability of your school courses to other institutions is based on the other institution's evaluation policies and procedures. An institution's accreditation does not guarantee that other institutions will automatically accept another institution's credits. Students planning to attend another institution should check its policy regarding transfer credits. Upon request, The Salter School of Nursing and Allied Health will assist students looking to transfer to other institutions by providing a transcript of grades and applicable course descriptions to the other institution. 10 P a g e

14 TRANSFER OF CREDIT EARNED AT OTHER INSTITUTIONS TO THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH: The Education Department at the school will review transcripts to determine what hours/credits, if any, will be granted. Students petitioning for transfer credit must submit an official transcript and catalog from the institution, which describes the courses for which transfer hours are sought. Prospective students may be granted transfer credit up to 25% of the total program credits from an outside institution as long as the institution is recognized by the US Department of Education. Premier Education Group students may be granted transfer credit up to 50% of the total program hours/credits within a Premier Education Group school if the new program and all courses are not identical. If a student comes back within the 180 days they resume their original enrollment. Students may transfer credits from a Premier Education School with identical curriculum based on the following: A. Premier Education Group students passing grades will be accepted, however, non passing grades will still be calculated in Satisfactory Academic Progress and in the CGPA (Cumulative Grade Point Average). B. Premier Education Group students returning after one year or more will be required to take a skills assessment test. All skills assessments must be documented and graded. If the skill assessment is not passed, appropriate courses will be assigned to audit or retake courses. ADDITIONAL TRANSFER INFORMATION 1. All courses transferred from another institution will be marked with a T and will count as hours/credits attempted and completed for SAP calculation. 2. PEG Courses that are being transferred from one enrollment at the school to a new enrollment due to program changes, schedule changes, and reentry /reenroll activity will show on the transcript with the original grade earned, including F and W s and will also calculate for GPA and SAP using the original grade. This includes same school codes. 3. Transfer hours/credits cannot be funded with Federal Title IV financial aid. 4. The Education Specialist may exercise their discretion to reevaluate the award of transfer credit. 5. Transfer credits for courses that require a separate pre-requisite course that has not been completed by the student or has been completed but does not meet the transfer credit criteria, will not be accepted. 6. Students who currently hold an active Licensed Nursing Assistant license through the State of NH, NH Board of Nursing may provide a copy of their licensure to The Salter School of Nursing and Allied Health for transferability of the Licensed Nursing Assistant course (LNA011) within the Patient Care Technician program. TUITION CREDIT Students will be charged on a pro-rata credit for tuition based on number of hours remaining in the program. RIGHT TO APPEAL Students may appeal the transfer credit decision by writing to Premier s Education Specialist. 15 ARTICULATION AGREEMENTS The Salter School of Nursing and Allied Health hold articulation agreements with Manchester Community College (MCC) and Nashua Community College (NCC). Each agreement is stated as follows: Manchester Community College THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH graduates who are currently NH licensed LPNs and have met all MCC admission requirements must also: Successfully complete the THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH LPN program with a cumulative grade point average of at least 2.0 (75%) Successfully complete the ATI LPN STEP Test with a required individual score of 66.7% or better (until January 2015) Successfully complete the ATI Fundamental Exam earning a Proficiency Level 2 (commencing January 2015) Admissions of advanced placement or transfer students is contingent upon space availability. Successfully complete a lab practicum in order to receive advanced placement. Admissions of advanced placement or transfer students is contingent upon space availability. 16 FINANCIAL INFORMATION The Salter School of Nursing and Allied Health s Practical Nursing and Patient Care Technician programs are approved by the US Department of Education for Federal Title IV Funding. Federal Title IV Financial Aid is available to those who qualify. All students are encouraged to complete the Free Application for Federal Student Aid (FAFSA) to determine their eligibility for Title IV Federal Student Aid. Student must reapply each year. Students should meet with a representative from the Financial Aid office for assistance in applying for Title IV Student Financial Aid and to discuss all options available to them to help fund their tuition. Payment plan arrangements can be made for balances not covered by Title IV Financial Aid. All payments are expected in accordance with the agreed upon payment schedule. STUDENT FINANCIAL AID INFORMATION: The Salter School of Nursing and Allied Health is authorized to participate in the following Federal Student Aid (Title IV HEA) programs: Federal Pell Grant Supplemental Educational Opportunity Grant (SEOG) William D. Ford Direct Loan Program (Subsidized and Unsubsidized Direct Loans) Direct Parent Loan for Undergraduate Students (PLUS) Federal Work Study Program (FWS) ELIGIBILITY FOR FINANCIAL AID To be eligible for financial aid from federal or state sources, students must meet the following criteria: Be accepted into a certificate program, Be a United States citizen, permanent resident of the United States or other eligible non-citizen, Maintain satisfactory academic progress, Not be in default on any federal loan programs, Not owe a refund on a federal grant, Have a social security number, Male students must be registered with Selective Service, Be able to demonstrate financial need. The school participates in many types of financial aid programs. Two basic types of financial aid are offered: grants, which require no repayment; and loans, which a student may repay after graduation. GRANTS & LOANS Federal Pell Grant: Funds are available to eligible students attending approved schools. This needbased federal grant varies in amount per academic year and is determined on completion of FAFSA. Amounts vary based on need and availability of funds. Supplemental Educational Opportunity Grant (FSEOG) The SEOG grant is reserved for qualifying students with exceptional financial need. Eligibility is determined by the Financial Aid Office and is based on the availability of funds. Direct Subsidized Loans: Loans for students with demonstrated financial need, as determined by federal regulations. No interest is charged while a student is in school at least half-time or during the grace period. Direct Unsubsidized Loans: Are not based on financial need; interest is charged during all periods, even during the time a student is in school and during grace and deferment periods. Federal PLUS Loans: Are unsubsidized loans for the parents of dependent students. PLUS loans help pay for education expenses up to the cost of attendance minus all other financial assistance. Interest is charged during all periods. NOTICE OF STUDENT FINANCIAL AID PENALTIES FOR DRUG LAW VIOLATIONS: Students who have been convicted for the possession or sale of illegal drugs for an offense that occurred or occurs while they are receiving federal students aid (such as grants, loans or work-study), may lose their eligibility for aid. 13 P a g e

17 METHOD OF FINANCIAL AID DISBURSEMENT Disbursements of Pell Grants and student loans are made in no less than two separate disbursements, provided that the student has completed all necessary paperwork to be eligible for the programs and is maintaining Satisfactory Academic Progress at the scheduled disbursements points. ENTRANCE & EXIT COUNSELING All student loan recipients are required by the U.S. Department of Education to receive Entrance and Exit Counseling to ensure they are aware of their rights and responsibilities under the specific loan program which applies to them. Topics include the lender s name and address, amount of the loan(s) borrowed, terms of repayment, interest which will be charged, prerequisites for requesting deferment of repayment, requirements to keep the lender informed, and consequences of defaulting on repayment of a student loan. The Financial Aid Office is responsible for scheduling and conducting entrance and exit counseling with borrowers at the appropriate times. Students are expected to complete their entrance and exit counseling sessions with the Financial Aid office. STUDENT PAYMENT PLANS: Payments may be made in the form of a personal check, a certified check, a money order, debit card or a bank check. Visa, MasterCard and Discover may also be used to pay tuition. Books and materials will not be issued until all current fees are paid. Students who have entered into a payment plan are expected to pay according to the prearranged payment plan agreed upon at the time of enrollment. If students are unable to meet their financial obligations, provisions for late payment may be set up at the discretion of the Financial Aid Director and/or Campus Director. If provisions are not met, it may be necessary to dismiss a student for negligence to pay. All outstanding student account balances must be paid in full prior to graduation or upon withdrawal. Upon satisfactory completion of a program and discharge of all financial obligations, students will be awarded a Certificate of Completion. In the case where a student has arranged a payment plan that extends beyond their graduation date and where a certificate of completion or official transcript are needed in order to apply for certification or licensure, The Salter School of Nursing and Allied Health will release the documentation directly to the certification or licensing body, in partnership with the student. Delinquent accounts that prove not to be collectible may be submitted to a collection agency for processing. SERVICE CHARGE In the event that a personal check rendered for payment of obligations is returned by the bank for insufficient funds or for any other reason, a $25 non-sufficient funds fee will be charged to the student. BOOKS & SUPPLIES Students will be charged for books and supplies received unless, in case of cancellation, they are promptly returned in proper condition for resale. Failure to stay current with your payment obligations puts you at risk for adverse consequences which could include hold back of textbooks. POLICY ON UNPAID DEBT It is the policy of The Salter School of Nursing and Allied Health that students or former students who have any unpaid obligations for tuition or fees due to the school who have not made acceptable arrangements for settlement of obligations, will be subject to the withholding of any books, supplies, grades, grade reports, transcripts, certificates, or program-inclusive certifications/tests and may not be able to participate in the clinical course or continue with the program until satisfactory settlement of unpaid debts. 14 P a g e

18 OTHER FINANCIAL OPTIONS Tuition Payment Plans- A tuition deposit of 25% is required by the first day of class. Two plans are available for paying the balance of tuition; full payment of tuition on or before the date classes start or payment of tuition balance in monthly installments. Payments may be made in the form of a personal check, a certified check, a money order, a bank check, or a credit card. Visa, MasterCard and Discover are accepted. Third Party Payers: Organizations may sponsor students by authorizing the payment of tuition and fees. Students must submit to the Financial Aid Office, a letter of authorization or a purchase order/award letter from the sponsoring organization. Invoices will be forwarded to the appropriate organization. Payments from the organization will be due and payable within 30 days of invoice date. Vocational Rehabilitation- Students with a physical disability may obtain grants-in-aid providing assistance for meeting school-related costs through the NH Vocational Rehabilitation Commission. Veterans Administration Funds- Veterans and dependents of veterans should check with State approving Agencies to determine eligibility. To determine eligibility for VA benefits contact the Veterans Affairs Office to obtain the necessary application (VA Form ) or VA Form ). Once the application is complete, the application must be submitted to The Salter School of Nursing and Allied Health s certifying official. The certifying official needs to certify the student s enrollment at the school to the VA using (VA Form ). The application and the certification of enrollment will be submitted by the certifying official to the Regional Processing Office for payment. It is the applicant s responsibility to contact the school s certifying official to initiate the payment of benefits. It is also the applicant s responsibility to notify the certifying official of change in program, withdraw from a course or school, non-attendance, or other matters affecting academic status. The student may obtain an application by calling or by logging on to the national web site at GI Bill RETURN OF TITLE IV FINANCIAL AID FUNDS The return of Title IV funds as prescribed in Section 484B of the Higher Education Amendments determines the amount of Title IV aid a student has earned at the time a student withdraws. The amount of the Title IV aid a student has not earned is then returned to the Title IV programs. It is a proportional calculation based on the date of withdrawal through sixty percent (60%) of the payment period. The formula in brief is as follows: 1. The school will determine the withdrawal date and the percentage of the payment period attended. 2. The school will determine the amount of Title IV aid for which the student was eligible. 3. The school will compare the amount of aid earned to the amount disbursed. If the amount disbursed exceeds the amount earned, this amount of Title IV aid must be returned. Example: Number of Hours scheduled as of last date of attendance / Number of Hours in the payment period = % of Title IV Financial Aid Earned* *If greater than 60%, the student has earned 100% of the aid for that payment period. Any unearned aid will be returned by THE SALTER SCHOOL OF NURSING AND ALLIED HEALTH to the U.S. Department of Education according to applicable regulations. Upon satisfaction of the Return to Title IV calculation with return of all unearned aid, the student may owe a balance to the school.
