2 Introduction My name is Helen McCallum; I am the Clinical Skills Facilitator for St Marys Theatres. I would like to offer a warm welcome to all Trainee Operating Department Practitioners and Student Nurses. My contact details are: - -helen.mccallum@cmft.nhs.uk Tel I am a scrub and anaesthetic practitioner and when I am not in theatre you will find me in the Resource and Education Room. Educational Philosophy Statement. It is our philosophy that every student is an individual and as such has their own individual learning needs. The nursing and operating department practitioners working in the Women s Theatres at Saint Mary s Hospital will endeavour to facilitate and promote the achievement of the individual students learning needs. We aim to provide an environment that is conducive and conductive to learning, by supporting the student through their theatre experience by allocating a mentor to each student. About us The mentor is responsible for discussing with the student the aims and objectives which the student has identified and wishes to achieve and experience during their allocation. We aim to provide annual updated teaching packages and resources to cover the 3 areas of clinical practices in Anaesthetics, Scrub and Recovery. Our ultimate goal is to promote awareness of the preoperative nursing care and technical support services in the theatre environment, whilst maintaining the patients dignity with safety and working as the patients advocate at all times. Our Set up Our Theatre Matron is Angela Williams and our Theatre Team Managers (Band 7 s) are Stephanie Feely, Shaun Caine and Darren Hardman. Our workforce is made up of fifteen teams including two permanent night teams. We specialise in Gynae oncology, Gynae general and urogynae, maternity and IVF. We carry out major and minor cases both elective and emergencies.
3 Saint Mary s Theatres has 7 operating theatres: Theatre 38(Elective Gynae Surgical) Theatre 40 (Elective Gynae Surgical) Theatre 41 (Elective Gynae Surgical) Theatre 42 (Obs and Gynae urgencies.) Theatre 43 (Elective C-Section/Obstetrics) Theatre 44 (Obstetric Urgencies). IVF Theatres. IVF Theatre. (Old St Marys) Saint Mary s Theatres has 7 operating theatres: Orientation On the first morning of your placement you will be given a tour of the department, and introduced to staff and your mentor. This orientation will cover: Sickness and Absence. If sick you should inform the Theatre co-ordinator on as early as possible. This phone is a dect phone and is answered 24/7. You must take the name of the person who you reported your sickness to. You should me as well as informing the University. When you are fit to resume work please contact theatres to inform them or days off will count as sick as per university policy. Fire Policy. You will be shown fire exits, extinguishers etc. The Trust Fire and Evacuation Policy is available to read on the intranet and in the RED file kept in the Theatre coordinators office. Emergency Equipment. You will be shown where all emergency equipment is on your induction day. Moving & Handling. Please show your clinical teaching in this area before moving patients. Uniform. Theatre wear is provided. A plain wedding ring and one pair of plain stud earrings are permitted. False or acrylic nails are not allowed. Hair should be tied back if shoulder length and makeup should be minimal. Students must wear white theatre hats to denote their supernumerary status.
4 Infection Control. The infection control policy is available to read on the intranet. If leaving the department you must change into your own outdoor clothes. Hand washing is essential and is regularly audited. Meal breaks. Meal breaks are 30 minutes long and you can use this time as you wish, however you must get changed before leaving the department. The staff restaurant is open 24 hours a day, providing hot food during meal times and cold snacks are available at all other times. Respect. As a teaching hospital the department often has visiting Surgeons as well as Consultant Surgeons, Consultant Anaesthetists, Registrars both surgical and anaesthetics, SHO s, Radiographers, staff and student nurses and student ODP s. Please address these people appropriately respecting their position. Hours of duty You will work 37.5 hours over four days. You can work early, late or night shiftsplease discuss this with your mentor. The following are the most common shifts: 08: :00-21:00 11:00-21:00 21:00-08:00 The learning environment The Clinical Skills Facilitator is in post to support mentors and students and is a link to the university. Internet access is available in the department for learning when appropriate. There is a student board on the corridor near the changing room. There is an excellent library onsite which you will be taken to on induction to register. There is a range of articles on line and hard copies available to photo copy. The trust library provides a quiet study area if needed. Students are expected to engage in many aspects of theatre practice. Throughout your time with us you will never be expected to work alone and will be supervised by a qualified member of staff at all times. There may be times that you find particularly stressful during your allocation, so please let the staff working with you know how you feel so that we can offer support. For more information speak to your mentor or a member of the education team. You will have regular meetings with your Clinical Skills Facilitator to discuss your progress and any issues. Please bring all relevant paperwork with you.
5 You will be working with your mentor and following their shift pattern. Any off duty requests can be made in the request diary which is hanging up near the student notice board. Theatre Areas of Work. There are three areas of theatre work Anaesthetics, Surgery and Recovery. The multi-disciplined team works together to provide a safe and caring environment for all patients. We strive to ensure that protocols and policies that you observe during your placement are evidenced based. Whilst in Theatre you will work alongside the multi-disciplinary team with your mentor. It is important that you stay with your allocated practitioner and discuss any moving with your mentor. You may not be working with your Mentor on a daily basis but they are present in the department. Your mentor will complete all necessary paperwork at the appropriate time and ensure that you have the opportunity to plan and achieve relevant outcomes. Reception/Anaesthetic room We receive our patients in the anaesthetic room and it is important that we gain the confidence of all patients, so during this time when documentation is checked, staff are expected to act in a professional, pleasant and caring manner. Practitioners work with the anaesthetist to ensure a safe, skilled and efficient anaesthetic delivery. Surgical Procedure Once anaesthesia has been safely induced the patient is transferred to the operating theatre, positioned and prepared for surgery. The Scrub Practitioner is responsible for ensuring the surgeon is provided with everything needed to perform the surgery and to ensure a sterile field is maintained. The circulating staff provide additional equipment as well as recording data into a theatre register and ORMIS computer system. This role involves safe transfer of specimens to the appropriate laboratory, disposal of clinical waste and carrying out all counts with the scrub nurse. Anaesthetic practitioners in the theatre assist the anaesthetist in maintaining anaesthesia, recording fluid balance, transfusions and recording the patients vital signs. Recovery Once the surgery is completed the patient is transferred to the Recovery room. The recovery practitioner is responsible for maintaining a safe patent airway, monitoring of vital signs, checking wound sites, assessing the patients pain level and if necessary administering analgesia. After complete recovery, completion of documentation and concise report to the ward nurse the patient is discharged to the
6 ward. In some circumstances the patient is sent to the High Dependency Unit or Intensive Care Unit post-operatively. Confidentiality As trainee professionals we expect you to observe all aspects of client confidentiality. All people must be treated equally and fairly, without discrimination because of race, gender, disability or religion. Evidence of learning You are expected to provide evidence of your learning to mentors and Clinical Skills Facilitator this can be in the form of Workbooks, assignments or reflections. Study ideas. The nature of theatre means that we do have very busy periods and quiet periods but here are some ideas for self-directed, delivered or facilitated learning. Drug calculations Common drugs and side effects Resuscitation trolley contents Difficult airway trolley contents Defibrillator testing Basic surgical instruments Basic life support Fluid balance Relevant A&P What to do at the start of your shift You are required to sign in on a daily basis. The sign in file is located at the reception desk at the front door. This system is monitored so please ensure you sign in with the correct details. When you start your shift you will need to go to the allocation board in between theatres 41 and 42.This board has the theatre number e.g. 40 and the anaesthetic, scrub and circulating practitioners for the morning and afternoon. You will find your name on the board alongside your mentor. If your mentor is not on duty you will have be allocated to work with someone else. Please remember never to leave the department without telling your mentor or CSF. Spoke Placements specific to our clinical area:- Pre-op Clinic. Ward 62(Gynae). Midwifery Led Unit/Central Delivery Unit. IVF. NICU/PICU.
