5 This time of the year the report has had a past tendency to reflect on our misfortune regarding the redevelopment at West Heidelberg. Not this time! We are pleased to inform our members that the past year s activities have been overshadowed with the relocation of our services to the four corners of the City, the demolition of the existing building and, most importantly, the commencement of the building process. All this on top of another strong performance by the Banyule Community Health Service. In May 2005 the Honourable Bronwyn Pike, Minister for Health participated in the turning of the sod ceremony signifying the start of the construction stage for the redevelopment. She stated that she was delighted to take part in the project as this investment signifies the commitment the government has to the promotion of primary care in Victoria and making West Heidelberg an even better place to raise a family. It is expected that the new facility which will house a new eight chair dental clinic, amongst other things will open sometime early in With the redevelopment beginning in the latter part of the year our main focus was to ensure the sourcing of alternative premises to continue our service provision at the current level. For this we are very grateful to the Office of Housing for the use of their Oriel Road offices. At the same time the final stage of redevelopment at the old Diamond Valley Community Hospital was nearing completion allowing us to relocate the dental clinic to Greensborough. The opportunity to expand the Greensborough programs at the new site in Grimshaw Street has benefited the local community with the much needed introduction of programs and services, furthermore, the introduction of the family law practice by the West Heidelberg Legal Service has by all accounts filled a void with a service that is gaining in patronage. This year again saw our involvement with the Banyule/Nillumbik Primary Care Alliance resulting in the development of an integrated health promotion plan, improved service coordination (e-referral) and catchment planning. Our partnership with Austin health continues with delivering community based services aimed at chronic illnesses management and reductions in inappropriate presentations to the emergency department. All these partnerships are about delivering the right service at the right place at the right time. Moreover, our involvement in the delivery of the municipal health plan for the City of Banyule has once again highlighted the commitment Banyule Community Health Service has to working collaboratively and cooperatively with other organisations to deliver better outcomes to our community. Financially, Banyule Community Health Service has performed well considering the major developments at Greensborough and its relocation to alternative sites. Add to this the continuing emphasis on information technology and the need to upgrade our equipment has led to an inordinate amount of our resources to be spent on this area. Programmatically, we continue to meet our targets although some areas have been slightly affected with the relocation due to the tyranny of distance as clients traverse the City in search of our new locations. We wish to put on record our congratulations to Bernie Geary on his recent appointment as Commissioner for Children and our acknowledgement of the tremendous contribution he has made to the Health Service. Bernie has been involved with the Community Health Centre for 27 years in a number of roles; staff member, Chief Executive and Board Member. Unfortunately, with his commitments as Commissioner, Bernie has had to resign from the Board. We wish him well in his new and very challenging role. We wish to express our gratitude to all staff who continue to work under difficult circumstances due the current redevelopment and the need to relocate; to all the volunteers, especially Brian Dawson who has provided the important thread that keeps the various sites together, for their commitment to the community; and to the Board of Management who continue to provide ongoing support and leadership. Your combined efforts have once again raised the bar in the pursuit of good practice and excellence in community health. 6 A Fertile Year for Allied Health The Allied Health team has continued to foster a planned approach to promote service access to communities of interest and to further develop integrated client focused services. In spite of difficult times experienced by staff and the community during the relocation period to current temporary premises and staff changes due to parenthood leave, the team kept their spirits up and worked very hard throughout the year to meet service targets. This successfully resulted in providing a total of 18,553 interventions/treatments to 5,013 individual clients. Approximately 20 per cent of these clients received podiatry, occupational therapy, physiotherapy and carer education and support services in their own homes. Additionally, the HARP Community Link program which continued to develop as a service over the past year, provided a package of highly responsive and expert services to 804 clients to assist them to continue managing at home while recovering from a short-term medical problem. Community Link services included respite care, personal care assistance, nursing, physiotherapy, occupational therapy and occasionally help with the gardening or pet care. Allied Health group interventions, health education and health promotion programs during 2004/2005 included: > Strength training programs conducted across both sites (West Heidelberg and Greensborough) > Hydrotherapy groups conducted across both sites > Gentle exercise programs > Tai Chi classes > Spinal care programs > Pulmonary Rehabilitation programs > Activities of Daily Living (ADL) tours > Foot health clinics > Speech Pathology parent education programs > Speech Pathology and Occupational Therapy (SPOT) group for pre-schoolers > Banyule Carers Support group > Planned Activity Program (PAG) for Somali men and women > Active participation and contribution to Women s health programs > Active participation and contribution to Diabetes education programs and Banyule Diabetes support group which turned 21! at the beginning of this year > Active participation and contribution to BCHS s Multidisciplinary focus groups: mental well-being and social connectedness, emotional health and Access to services targeting the ATSI community. 7 Allied Health team achievements in collaborative health promotion partnership activities included: > Successful completion of the Falls prevention project > Better health self-management project > Banyule-Nillumbik Physical Activity Network > Prime Time program aimed at increasing participation of older people in physical activity > Banyule-Nillumbik Well for Life project > Co-ordinated Care Trial stage 2 > Post Acute Care program > Acute Primary Care Interface project > Neurological Complex Care project aimed at integration of services and partnership approaches between acute, sub-acute and primary care services, through the service co-ordination strategy > Establishment of Bellfield local service partnerships to strengthen linkages and identify activities for networking. Achievements in Allied Health service development include: > Increased funding to Dietetic services by 0.2 EFT in January 2005 to provide early intervention in childhood nutrition and healthy weight promotion. > A one off grant funded to OT services (0.4 EFT) helped meet service demand and reduced the waiting list from 9 to 3 months. > Establishment of a new Allied Health Assistant position at 0.3 EFT within the HARP Community Link program > Implementation of BCHS s service co-ordination system incorporating 1.0 EFT position funded from the Allied Health budget > Integration of HARP Diabetes program into existing Community Health Services Banyule, Nillumbik and Darebin. Some of the specific achievements of the HARP Diabetes program include: the establishment of the Diabetes Co-ordination team; the development of a unique diabetes self-management tool; the establishment of strong links with collaborating CHS s and a broader understanding of other work environments; high level of GP engagement as evidenced by a high GP referral rate and involvement of GP s in consultation meetings; and a reduction in hospital ED presentations, unplanned admissions and length of stay. An overall achievement in working collaboratively with the acute sector programs has been a better understanding of the value of working together to strengthen the continuum of care from acute to community through an integrated service system, clearer referral pathways and enhanced models of care. 8 Community Development and Counselling Team Societies that enable their citizens to play a full and useful role in the social, economic and cultural life of their society will be healthier than those where people face insecurity, exclusion and deprivation (DHS Plan for Action Promoting Mental health and Wellbeing) The Community Development and Counselling program is made up of the following teams: > Community Health Nursing, > Drug and Alcohol counselling > General counselling and community development and > Service Coordination Over the past year nine thousand direct hours of service and approximately four thousand health promotion hours of service were delivered to Banyule residents from both the West Heidelberg and Greensborough sites. Additionally the Financial Counsellor saw one hundred and seventy three clients. The drug and alcohol counselling team saw approximately six hundred adult clients and one hundred and fifty adolescents. All of the above services work in close partnership with the community and other BCHS services such as medical, dental and allied health. Our external partners include Mental Health Services, Acute Health Services, Aboriginal Health Services the Migrant Resource Centre, the Ministry of Housing, Centrelink, Austin Health, Victoria Police, Education and Employment Services, Banyule City Council, the Department of Human Services and more. Achievements: > Health promotion programs were delivered in partnership with young vulnerable mothers; with women suffering from post and antenatal depression; with people suffering from chronic obstructive pulmonary disease; with adolescents; with people who are wanting to move on after a serious depressive illness; with women facing mid life; with Somali women and families; with people managing diabetes and more. > More than three thousand clients were prevented from having to navigate many service systems and undergo unnecessary multiple assessments by using the central intake and service coordination system. > More than two hundred clients participated in drug and alcohol counselling via Courts referrals. These clients include those who are on community-based orders and those released from prison on parole. > Strengthened family connectedness through fun and activities at the Family Camp at Phillip Island organised and auspice by the Program twice yearly. > Worked with youth within the school and community settings to build resilience and improved living skills. > We provided counselling for adults, children and families suffering from depression and anxiety associated with grief and loss, separation and divorce, family violence, homelessness, chronic illness, loss of employment, serious injuries and parenting issues. > We worked with public tenants in West Heidelberg to establish Public Housing Tenants Group in partnership with the West Heidelberg Community renewal Group. > Supported sufferers of breast cancer through the Breast Cancer Support Group and by inviting special guest and former Commonwealth gold medal champion Raylene Boyle to deliver a keynote address during Breast Cancer week. > Through the Hospital Admission Risk Program we worked with more than one hundred clients suffering from Chronic Obstructive Pulmonary Disease in an effort to maintain them in their own homes.
