To work as part of the engage ORBIT Team providing a seven day a week service to ED and AAU following an Interprofessional Practice model to assess, treat and assist with discharge planning to prevent admissions and unsafe discharges and reduce readmission rates with a focus on older people. To work with the engage ORBIT Team and provide cover to the engage MDTs as needed. To provide specific Physiotherapy input to the engage ORBIT Team as required (e.g. chest physiotherapy, vertigo treatment, falls management and back pain advice). To work as part of a multidisciplinary team, which includes; Nurses, Occupational Therapists, Social Workers, Speech Language Therapists, Clinical Nurse Specialists, Dieticians, Administration Staff and the Medical Team. To ensure and prioritise a focus on patient safety and quality relating to care and processes within the engage Service. To be an active member of the engage and Physiotherapy Teams sharing knowledge and skills. Delivery of organisational KPI s including relevant MOH target, financial budgets and service plans. Dealing with time pressures (ED has a 6 hour discharge target and AAU 36 hours), quick turnover of patients and delays to discharge due to other services involvement (i.e. ACC, Options) or lack of equipment. Clinical role under the guidance of the engage Team Leader working in an interprofessional team in ED/AAU with daily working relationships and responsibilities to the engage service. Professional leadership from the Physiotherapy Team Leader and active participation with the Physiotherapy Team N/A Ensure clinical decisions are made on sound clinical reasoning and in a timely responsive manner. Work within an interprofessional model of intervention and service delivery. Delegate appropriately to other members of the interprofessional team both regulated and non-regulated staff. Accountable for health record entry hard copy and electronic that meets with NZ Health Records standards. Take part and lead quality improvement activities. P Participate and be responsive to feedback and performance planning and review. Attendance, contribution and participation within team meetings and activities that are positive and proactive.

3 Key Accountabilities CLINICAL PRACTICE Be accountable for providing safe, effective and appropriate physiotherapy assessment and intervention using knowledge and skills gained from professional education and practical experience. Use and document a planned physiotherapy process and adhere to professional, service and organisational policies, protocols and practice guidelines. Works within ethical guidelines and legislation e.g. Code of Health and Disablity Service Consumers Rights, Privacy. Develop, provide and monitor intervention plans and goals that: - maximise physical function and independence, - support clinical/discharge planning of the service, - reflect current best practice, - meet the needs of customers within resourses Work autonomously, while assisting to lead, within the team. Be able to assess and treat as an interprofessional team member but be able to identify what situations are out of your own scope of practice and refer on as appropriate. Demonstrates knowledge and application of physiotherapy specific interventions such as chest physiotherapy, vertigo, falls management and back pain treatment. Provide education to the person and the family/whānau as part of the physiotherapy process. Utilise principles of teamship and accessibility when based in a patient care area by providing a range of core tasks, e.g. dealing with patient or clinical enquiries, patient comfort measures, ensuring environment is safe and tidy. Demonstrate essential requirements for Enable New Zealand accreditation in walking and standing. Undertake Enable Service Accreditation training. Provide documented evidence in CPD folder to demonstrate competency. Contributes to the Health Record by providing documented evidence of therapeutic intervention and sound clinical reasoning. Meets expectations of health records standards and policies National and HBDHB. Documentation audit. Demonstrate problem solving skills. Demonstrate that client problems are identified and analysed, appropriate goals are set with the client, physiotherapy treatment implemented and outcomes evaluated. Demonstrate time management skills. Complete agreed competencies for OT & SW so can work as an interprofessional team member. Patient and Customer feedback. Event reporting. Accredited Enable Equipment and Modification Services (EMS) assessor in walking and standing with Service Accreditation.

4 SERVICE PROVISION Ensures that appropriate quality and risk management processes are in place and used effectively in liaison with the wider engage ORBIT team and engage Team Leader. As part of a team, ensure contributions to improving the delivery of the service is based on continued quality initiatives. Be focussed on patient/client safety at all times to identify near misses, and adhering to the HBDHB polices on working safely. Participate using an Interprofessional Practice model and contribute to collaborative intergrated care planning and treatment intervention. Ensure all clinical health record entries, both electroinc and hard copy, are entered in accordance with NZ Health records standards and completed within the shift. To cooperate/assist the engage Team leader in all matters necessary to the efficient management and development of the service in accordance with departmental and Hawke s Bay District Health Board policies and proceedures. To teach/mentor/support Physiotherapy junior staff and students and other junior/new staff in an Interprofessional approach. Proactively contribute to education of other professions in the role of Physiotherapist. Preparation for and participation in Interdisciplinary meetings and systems. Quality and auditing information. Evidence of particpation and contribution at meetings. Evidence of quality initiatives that are focused on patient journey and faciliatative of change and service delivery improvement to benefit and enhance the engage ORBIT service within the DHB. Evidence of involvement with service innovation and development, active contribution to ideas and implementation of quality improvements. Evidence of teaching/coaching programmes to promote and improve the profile of Physiotherapy / the engage service. Evidence of supervising/mentoring of Physiotherapy students and liaison with Physiotherapy schools and students of other disciplines as appropriate. COMMUNICATION & LIAISON Develop an effective professional relationship within the engage ORBIT team as well as the wider engage & physiotherapy teams. Demonstrate effective communication both written and verbal. Develop and maintain good working relationships in the clinical setting. Participate in the interprofessional approach to patient care. Refer clients to other agencies as applicable. Develop knowledge of, and working relationships with, nongovernment agencies and voluntary groups. Promote healthy lifestyles and illness and injury prevention. Liaise nationally with clinical specialists. Work closely with other multi-disciplinary team members. Demonstrate positive interactive attitude within the team. Attend and participate in staff meetings. Actively engage in Performance Appraisal process. Demonstrate interprofessional team (IPT) involvement and participate in IPT/MDT meetings. Stakeholder feedback. Record of involvement in health promotion and preventative programmes, lecturing to groups as required. Ensure education is provided to the client, caregiver and other personnel to promote optimum wellbeing by encouraging selfmanagement. Maintain communication nationally with other specialists. Attend national specialist forums as approved.

5 LEADERSHIP & MANAGEMENT Demonstrate initiative in a variety of situations within the engage ORBIT team. Contribute actively to leadership and management tasks within the engage service. Provide leadership, support and supervision to physiotherapy staff and students as required to assist their development. Delegate appropriately to colleagues and physiotherapy assistants. Assist in developing and maintaining a good working relationship with the wider MDT. Ensure the inteprofessional team, the physiotherapy team and other members of the MDT are aware of developments and changes in physiotherapy practice. Proactively contribute to education of other professions in the role of physiotherapy and the engage ORBIT team. Demonstrate leadership, oversight and support of junior staff and assistants. Completion of delegated tasks. Supervise other physiotherapists and/or assistants. Supervise physiotherapy students. Record of participation in activities. Feedback from IPT and wider MDT, physiotherapy team, engage team, assistants and students. Evidence of continuous professional development (CPD). Evidence of service development and leadership as recorded in CPD folder. PATIENT & FAMILY EDUCATION Utilises the concept of long term condition management with the patient and family members as necessary. Involvement with discharge planning and education as required supporting effective discharge. Development of education material e.g. handouts etc for consumers for health literacy of specific conditions. Supports and participates in specific educational programmes for patients and their family members e.g. falls minimisation. Evidence of educating family/whānau to selfmanage Evidence of effective discharge planning to prevent readmission within 30 days. Evidence of involvement in educational programmes. PROFESSIONAL RESPONSIBILITY Maintain and develop clinical practice within the specialist area whilst continuing to maintain general competency. Set and meet personal learning objectives. Keep abreast of current best practice and trends in physiotherapy treatment. Actively participate in in-service education. Participate in tutorial group and peer support with physiotherapy team. Attend mandatory organisational training as required. Attend relevant training courses internal and external as appropriate. Participate in and provide supervision for other staff. Participate in peer review process. Be involved in the development of the physiotherapy service in a wider forum. Retaining an annual practising certificate. Positive Performance Appraisal. Achieve objectives of personal development plan. Demonstrate competence in area of specialisation and in general. Evidence of research (if funded) and learning within chosen area of specialty documented in CPD folder. 7 Essential and Desirable Criteria: Qualifications / Skills / Experience Essential Treaty of Waitangi Responsiveness (cultural safety) Demonstrates the ability to include cultural safety of the health consumer when relating to care and processes within the Service. Demonstrates ability to apply the Treaty of Waitangi within the Service. Qualifications (eg, tertiary, professional) Business / Technical Skills (eg, computing, negotiating, leadership, project management) Experience (technical and behavioural) Bachelor of Physiotherapy (BPhty) or equivalent. New Zealand Registered Physiotherapist with current annual practising certificate (APC). Current New Zealand drivers licence and ability to drive Excellent and responsive verbal and non-verbal communication written and verbal. Sound clinical reasoning and judgement skills. Will have excellent prioritisation/time management skills. Proven computer literacy ability to use current programmes At least 3 years of working as a physiotherapist with experience working with Older People (65+). Demonstrated competency in current physiotherapy practice and an undertaking to maintain professional development and ongoing education. Ability to organise workload and accept responsibilities for work outputs. Excellent prioritising and time management skills. Excellent communication skills, written and oral. Sound clinical reasoning and judgement skills. Ability to contribute positively to the interprofessional team, the physiotherapy team and the wider multidisciplinary team.
