1 Individual (Job) Description The department Acute Medicine The Department of Acute Medicine comprises 8 Consultant Physicians. All acute medical emergency patients are admitted to the Acute Medical Unit (AMU) and approximately 50% of such admissions are looked after entirely by the Acute Medicine Team. On average there are about 60 new admissions in any 24 hour period and over 40% of them are taken over by the Acute Medicine Team. All Foundation Doctors in Acute Medicine posts are ward during the normal working day and expected to deliver the daily medical care of all the patients on Acute Medicine Ward. This is a rapid turn-over ward with several new admissions every day. They work closely with Core Medical Trainees and Registrars. On Acute Medicine Unit they are responsible to provide ongoing care to the patients in the unit. All Foundation trainees in Medicine (including those working entirely in Acute Medicine) participate in Acute Take rota. When on take in the AMU Hub the Foundation trainees are expected to clerk new patients, arrange their basic investigation, initiate appropriate treatment and present them to the Consultant Physician of the day. They are supervised by the SpRs during this. Alumhurst ward is a 20 bedded inpatient Old Age treatment ward in St Anns Hospital Poole, with an average length of stay of upto 3 months. The F1 Doctor will be primarily on the ward and will provide medical backup along with a Consultant and two CT1-3 trainee doctors in the team. Additional support will also be available from a ST4-6 trainee. The trainee will have two sessions of working in the community with an Old Age Community Mental Team.

3 The overall educational objectives are to provide the trainee with the knowledge, skills and attitudes to deal with patients presenting with psychological or behavioral problems associated with severe mental disorders. They would be expected to: Assess new admissions by taking direct/collateral history, carry out mental state examinations and clerk in new admissions, Undertake physical examination of the inpatients and request appropriate investigations (inc. arranging scans, doing ECGs, liaising with GPs, managing straightforward physical conditions, prepare patient s for ECT etc.). Prescribe safely, particularly psychotropic drugs Communicate effectively with the patients, relatives and colleagues within the multidisciplinary team and the Community Mental Health Teams. Keep an accurate and relevant medical record electronically Manage time and clinical priorities effectively Use evidence treatment and/or guidelines when treating patients Understand legal and ethical issues in relation to the management of detained patients. Rotation: Bournemouth Hospital Foundation Programme The Liaison Psychiatry Service at RBH provides a psychiatric service to inpatients and attenders at the Emergency Department at RBH. It is part of a wider team which includes the Liaison Psychiatry Service at Poole Hospital. The F1 doctor will be part of the Liaison Psychiatry Service and will be at RBH. The service is for people between 18 years and the end of life. We receive approximately 800 referrals per year, reflecting the type of psychiatric conditions frequently seen by doctors not working in psychiatric settings. Commonly seen referrals include admission following selfharm, depression and anxiety complicating medical illness, medically unexplained symptoms and somatoform disorders, poor compliance with medical treatment and alcohol misuse. We also see patients with organic psychiatric disorders and where capacity for treatment is being questioned. The department itself consists of 9 full time Gastroenterology Consultants, 1 Staff Specialist, 2 interventional Fellow (Dr Katie Smith), 2 Nurse Specialists in Research, 3 Hepatitis Nurses and 1 IBD Nurse. There are 3 Gastroenterology Registrar s, 3 SHO s, 2 F2 s and 2 F1 s The department operates an extremely busy but comprehensive and dynamic service for the care of medical gastroenterology inpatients and outpatients. Typical conditions that are expected for the inpatients is care of the sick liver disease patient, acute gastrointestinal haemorrhage, complex hepatobiliary disease, complex inflammatory bowel disease etc. We accept tertiary referrals for complex cases from the CountyIt and have close ties with Southampton General. In Gastroenterology we perform upwards of 11,000 endoscopies a year. All gastroenterology care is Consultant led and with the close liaison between Interventional and Diagnostic Radiology, Upper and lower GI Surgery and finally palliative care with focus on fully multi-disciplinary approach. The F2 post is on the Gastroenterology Unit on Ward 1 and also Gastroenterology Outreach. Doctors are expected to be able to be dynamic and flexible in terms of where the provision of care is most needed either on Ward 1 or Outreach. In addition to on-call for acute and General Medicine, doctors contribute to the day to day care of medical and Gastroenterology inpatients. Ward 2 is the main Respiratory was and includes the Acute Lung Unit with two non-invasive ventilation beds. Three times weekly bronchoscopy list, respiratory function lab, diagnostic services. There is 1 F1, 1F2, 2 CMTs and 1 GPST and a specialist registrar. It is a dynamic and academic stroke centre which offers a comprehensive stroke service including hyperacute and acute stroke care, stroke rehabilitation and 7- day TIA service. It is a busy stroke unit which is closely linked to Wessex Neurological Centre in Southampton, and admits 700 new strokes and 300 TIAs per annum. Stroke Unit in Royal Bournemouth Hospital

15 Clinical Supervisor(s) in this Professor Joseph Kwan The F1 doctor is responsible with other staff for the patient care in the Stroke unit, and the maintenance of the patient s medical record. They will have opportunity to work with the consultants in all aspects of the stroke service. Whilst they are not expected to routinely assess patients in the outpatient department (e.g. TIA clinic), they are encouraged to attend number of clinics during their rotation to gain further experience. They are also expected to attend the structured FY Teaching Programme provided by the Trust, and the twicemonthly grand rounds / teaching provided by the Department, And the weekly Grand Round and MFE meetings.. The F1 doctor will be responsible for such other specific clinical duties as allocated by consultants including occasional emergencies and unforeseen circumstances.
