2 Intensive Recovery Intervention Service (IRIS) Mental Health Services for Older People (MHSOP) Description of the Service The service provides specialist assessment, active therapy, treatment, and the opportunity for recovery, for older people with a mental health problem. The Intensive Recovery Intervention Service enables people to have the choice to live as independently as possible at a time of crisis. We believe people experiencing mental health problems are entitled to the highest quality care. The service is delivered in people s own homes and we make every effort to prevent unnecessary admission to hospital or residential care. The service provides a flexible, creative, client-centred approach to enable the person with mental health difficulties to engage in services and be supported to remain in their own home. Eligibility Criteria The service is delivered within different localities across Nottinghamshire County. The service works with people who present with higher risk, complex mental health problems who require more frequent care. The patient must have a GP in the locality area of the service (see back page for details) Be aged 65 or over with mental health difficulties The service also sees patients of any age with working age dementia

3 People living alone who are at risk of self-neglect or are a danger to themselves or others People who live with their carers and are under extreme stress People in secondary, respite and residential care who could return home given an intensive period of support The individual must also: Wish to remain in the community Have the potential and motivation for independent/ supported living Wish to be discharged early These guidelines are flexible to ensure that the needs of the individual are met. Exclusion Criteria Under 65 with a mental health problem which is not dementia People with only a physical condition Care home residents unless the plan is to return home End stage palliative care is not routinely provided The Intensive Recovery Intervention Service Delivers care within community settings, usually in the person s home Time-limited, person centred and according to assessed care needs Free of charge to the service user Flexible 7 day week, between 7.00 am to 10.00pm to meet client needs and is open 365 days of the year Operates an open access referral process Ensures effective care planning and coordination with seamless handover to other services and agencies

4 Provides support, advice and education for carers, families and to home care providers and other involved workers Has links with community care and social services Has residential step-up care, which is available for those people who require urgent assessment, who are unable to stay at home and do not require inpatient admission. The focus will be on short term residential support, normally for hours with a view to the person returning home The team commissions services to meet the longer term needs as appropriate, including self-directed support packages We try to: Avoid unnecessary admission to hospital Support timely hospital discharge Avoid inappropriate or premature admission to long-term care Prevent avoidable admissions to short term care Provide a specialist, multiprofessional assessment Promote faster recovery from illness Prevent avoidable deterioration Maximise skills and independent living Build capability and quality into mainstream home care service provision

5 Carer/service user statements We would like to say a huge thank you for all the help and support you have given our mum over the last few months which made life so much easier for us. Also for the time, effort and consideration you put into mum s care and needs. I would like to thank you for the care and support you offered my wife as she transferred from the care home to her real home here with me. Without your knowledge and expertise I would have been lost. The team s caring nature was always to the fore and the smiles were an additional bonus. I just wanted to say a very big thank you for all your support and care over the past few months. The team do such a good job and it s a pity you can t be there all the time for people like me. You have been a lifeline and I will miss the team s hard work and dedication.

6 The team is made up of Community Psychiatric Nurses (CPN), Occupational Therapists (OT), Physiotherapist, Social Worker, Assistant Practitioners (AP), and Community Support Workers (CSW).
