To deliver specialised physiotherapeutic intervention to patients using highly developed skills in assessment and treatment of patients. This includes the delivery of exercise programmes and giving advice, information and emotional support to patients and carers in a wide variety of settings. To participate in the rotation scheme, this involves working across the Trust, over six sites in a range of ward settings, outpatient departments and in the patient s own home. To contribute to the development of evidence based practice and clinical effectiveness during each rotation. To participate in the emergency and on call rota across the Trust. Key working relationships The Physiotherapist will report directly to the more senior physiotherapists. They will be accountable to the Clinical Lead of that rotation. The post holder may be directly responsible for development and/or supervision of assistant staff. The post holder will provide support and teaching/training to students on placement or newly rotated staff. The post holder will work closely with the multidisciplinary team including Nursing staff, Medical team, other therapists and support staff. This will include excellent feedback on the patient s status and progress to enable effective collaborative working.

Main duties and responsibilities Professional / Clinical responsibilities To be an autonomous practitioner and be professionally and legally accountable for all aspects of professional and clinical work. To be able to work independently with the freedom to take decisions about the patients management and discharge, but with the support of a senior physiotherapist available when appropriate for the management of complex patients, or if advice is necessary on workload and problem patients in a specialised unit. It would be expected that the physiotherapist would recognise their own limitations and actively seek advice when necessary. To undertake assessments and treatments of patients effectively and efficiently using highly developed clinical reasoning skills and manual assessment techniques in a wide variety of settings with a wide variety of patients and conditions. To use a high level of analytical skills developed with ones own experience and learning, to assess, treat and produce a physiotherapy diagnosis, using information from the patient, carers, hospital notes, physical assessment techniques, visual analysis and occasionally conflicting elements from different sources. To carry out effective treatment plans and interventions independently developed by self, based on knowledge of treatment skills such as manual therapy techniques, electrotherapy, patient education and other alternative options, with support available from a senior physiotherapist for complex or problem patients. Formulate prognosis and recommend best course of intervention, developing comprehensive discharge plans within a wide variety of settings and specialities, with support available from a senior physiotherapist for complex or problem patients To have the analytical skills, experience and knowledge to recognise other problems presented by the patient. This may require referring patients to other services for the appropriate intervention, with support when appropriate from a senior physiotherapist for patients with complex problems or needs. To work effectively with staff from other disciplines in the multi disciplinary team to ensure care pathways are followed to the benefit of the patient in a seamless and timely manner.

5 To work with a holistic approach to patient care, being able to draw on own experience and learning to most effectively decide on the best management of each patient. Continually develop knowledge, skills and clinical reasoning to a high level during rotations to ensure that a high standard of practice is maintained and transferable across clinical areas and specialities. To manage clinical risk within own clinical workload To participate in rosters and seven day working within a team To be an active member of respiratory evening and on-call rota. When working on emergency duty on-call rota, undertake the assessment and treatment of acutely ill patients with respiratory problems, who may have a complex presentation and maintain associated records as an autonomous practitioner To support senior staff in the education of student physiotherapists as required. To attend multi-disciplinary meetings and be able to discuss patient care, patient progress and be involved in discharge planning To be responsible for the safe and competent use of all electrotherapy equipment, gym equipment and patient appliances and aids. To also ensure that assistants attain competency prior to using these appliances To effectively use IT skills when developing exercise programmes and advice leaflets for patients, including the efficient use of programmes such as Physio tools, Microsoft word and excel, and the use of Microsoft power point for teaching and presentations. Work within Trust clinical guidelines and Chartered Society of Physiotherapy (CSP) guidelines to have a working knowledge of national and local standards, and monitor own quality of practice. To be responsible for maintaining accurate and comprehensive problem orientated medical records (POMR) in line with legal and departmental standards that meet Chartered Society of Physiotherapy (CSP) standards on record keeping Organisational responsibilities Assist in the supervision and co-ordination of assistant staff and students as required by senior physiotherapy staff. Have complete responsibility for the organisation and planning and prioritisation of own diary and workload to meet service and patient priorities, being flexible enough to be able to cover absences planned and unplanned of physiotherapy colleagues, so effective patient management is maintained To effectively manage own time in such a way that all activities, projects and duties are done in a timely and efficient manner. To be effective in administration and clerical tasks such as booking patient appointments, photocopying, answering phones and message taking, organising interpreters, filing and recalling patient notes and x-rays, and any other routine administrative tasks that need doing on a daily basis. Organise placements and induction for work experience students. Be part of the induction programme for new staff and be able to effectively introduce new staff to the specialist area in which you are working. Management and Leadership To adhere to internal policies and practice regarding leave (e.g. annual, study) to ensure an appropriate level of staff cover is maintained across the service. To deal with initial complaints sensitively and to pass these on to the Highly Specialised Therapists or Lead Therapist.

6 To take part in the induction of new members of staff and students Financial responsibilities To possibly monitor stock and equipment levels in own service area and report any stock requirements to the Highly Specialised Therapists Communication & Information management To communicate effectively with all physiotherapy staff, with good understanding of professional and clinical issues to support efficient practice. To communicate in different formats, such as by phone, , face to face and letter, with other staff from a wide variety of hospital departments both clinical and organisational so that good communication between physiotherapy and all other services is maintained to the direct benefit of patients. To communicate in a professional, friendly and open manner with patients, carers and the general public to maintain an excellent profile for the department amongst its users. To communicate with patients and carers both verbally and nonverbally, often with the need for an interpreter, complex and comprehensive information, clearly and effectively, so that consent to treatment and compliance with treatment is gained. To maintain confidentiality for all patients and users of the service. To communicate effectively with service users who may be angry, upset, fearful or have communication difficulties such as poor language skills, hearing loss or learning disabilities. Taking into account psychological status, by using both verbal and nonverbal communication skills and experience built up during practice as a physiotherapist. To be able to effectively communicate with patients via interpreters, both professional and family members, and maintain equivalent assessment and treatment for all patients regardless of their language status. To be able to give patients and carers clear, precise written and verbal information regardless of their communication status or skills, to ensure effective interventions. To be able to clearly and concisely communicate to other services, details of a patient and their condition to enable an effective referral to other services to take place. To be able to effectively use presentation skills in a formal or non-formal setting, to communicate comprehensive and complex information to patients, carers, physiotherapy colleagues and the multi-disciplinary team (MDT). ducation & Professional Development With support of senior physiotherapists and via the appraisal process, maintain own competency to practice through CPD activities, and maintain a portfolio which reflects personal development Participate in staff appraisal scheme as an appraisee and be responsible for complying with and developing your agreed personal development programmes to meet set knowledge and competencies and to be looking to develop skills beyond that of a junior level towards specialising in a particular area of Physiotherapy. Undertake the evaluation of your work and current practices and identifying own learning needs and the resources needed to become a highly specialised physiotherapist. Be an active member of the in-service training programme by leading, attending and participating in, in-service training programmes, tutorials, individual training sessions, problem patient sessions and peer review.

7 To occasionally be responsible for leading journal clubs to peer group and physiotherapy team within speciality area. To contribute to the performance assessment and appraisal of newly qualified physiotherapists and physiotherapy assistant staff. To contribute to teaching student physiotherapists to graduate level on physiotherapeutic skills and knowledge within core clinical areas. Independently plan, prepare and carry out, teaching sessions to peers, students and other MDT colleagues. Risk & Governance Management To report any incident in a timely manner, completing all relevant paperwork and discussing with other qualified staff To be aware of health and safety and infection control issues in relation to the gymnasium and patient care To report any equipment faults in a timely manner and chase the outcome of this as required To be aware of all health and safety laws, resuscitation protocols, manual handling techniques and safeguarding children protocols by adhering with mandatory training To comply with organisational, departmental and professional policies, procedures and guidelines To be aware of and respect the right to patient choice and dignity and promote the Trust s equal opportunities policy. Service Development & Research Maintain and develop current knowledge of evidenced based practice in the areas of each rotation, developing knowledge of particular conditions and patient types to a high standard. During each rotation to undertake a project which will; a) Add to own and/or the teams evidence based practice, b) Or be an audit of current practice either clinical or professional, which will contain recommendations on future development of practice for the benefit of patients and/or the team. c) Or produce patient information to enhance service delivery Be actively involved in professional clinical groups, such as the trust Journal Club, Peer Review Groups and other professional development activities. General To carry out any other duties deemed necessary by the Physiotherapy Lead in discussion with the post holder to ensure adequate provision of service. To attend all Mandatory training and keep up to date. To ensure a safe and healthy environment for all staff and service users. To adhere to all Trust policies and guidelines at all times, including procedures on infection prevention and control in order to minimise patient risk and ensure high quality patient care. Adheres to Trust Information Security Policy at all times, in particular ensuring that there are no breaches of confidentiality. To carry out other duties commensurate with the grade as directed.

8 The job description is not intended to be exhaustive and it is likely that duties may be altered from time to time in the light of changing circumstances and after consultation with the postholder. The post holder might be required to work across the Trust at any time throughout the duration of his/her contract, which may entail travel and working at different hospital. ffort, skills and working conditions Physical skills Physical effort Mental effort motional effort 1. Be responsible for giving out and advise on use of, equipment to patients such as mobility aids, TNS machines, home nebulisers and other equipment. This will include monitoring and using signing out systems and bring to the attention of senior staff when equipment needs re-ordering. 2. To be responsible for any equipment used in the course of treatment and assessment of patients, so that it is safe to use by self, other staff and/or patients. 3. Carry out assessments and treatments of a variety of conditions with moderate physical effort throughout the day. 4. Undertake manual physiotherapy skills in the assessment and treatment of patients that require a high level of dexterity, precision, hand-eye co-ordination and good sensory co-ordination. 5. To comply with the Trust manual handling policy and local therapeutic handling guidance at all times, including using hoists, positioning and transferring patients and using manual techniques for a wide variety of conditions both on land and in the hydrotherapy pool. 6. To use manual handling techniques effectively in the lifting and handling of patients and awkward loads such as walking aids and hoists. 7. To use a high level of mental effort frequently during the day when using clinical reasoning, teaching peers and students, using high level technical equipment such as Intensive care equipment, and whilst using IT. 8. To be able to cope with frequent interruptions during clinical and non clinical work, while demonstrating the ability to multi-task on a frequent basis. 9. To help other members of the MDT in their duties as appropriate to the care of patients under physiotherapy treatment, this may include helping to change beds, incontinence pads and wash patients as necessary 10. To deal sensitively with patients who have high levels of anxiety, fear and aggression caused by pain, dementia, limited mobility or may be terminally ill. These patients may use verbal or physical aggression and subject therapists to varying levels of physical or verbal abuse.

To maintain a calm and professional manner when dealing with service users who may behave inappropriately, be aggressive or emotionally disturbed and whilst being subject to multiple often conflicting demands Working conditions 12. To be exposed to bodily fluids on a frequent basis, such as urine, faeces, vomit and sputum, and often to be exposed to noxious smells. To help patients with toileting, washing and dressing when appropriate 13. Health and safety at work The post holder has a duty of care and personal obligation to act to reduce healthcareassociated infections (HCAIs). They must attend mandatory training in infection prevention and control (IP&C) and be compliant with all measures required by the trust to reduce HCAIs. All post holders must comply with trust infection screening and immunisation policies as well as be familiar with the trust's IP&C policies, including those that apply to their duties, such as hand decontamination, personal protective equipment, aseptic techniques and safe disposal of sharps. All staff must challenge noncompliance with infection, prevention and control policies immediately and feedback through the appropriate line managers if required. Safeguarding adults and children mployees must be aware of the responsibilities placed on them to maintain the wellbeing and protection of vulnerable children and adults. If employees have reason for concern that a patient is 'at risk' they should escalate this to an appropriate person i.e. line manager, safeguarding children's lead, matron, ward sister/change nurse, site manager, consultant. (October 2002). Conflict of interest The trust is responsible for ensuring that the service provided for patients in its care meets the highest standard. qually it is responsible for ensuring that staff do not abuse their official position for personal gain or to benefit their family or friends. The trust s standing orders require any officer to declare any interest, direct or indirect with contracts involving the trust. Staff are not allowed to further their private interests in the course of their NHS duties. Confidentiality and data protection All employees are expected to comply with all trust policies and procedures related to confidentiality and data protection and to work in accordance of the Data Protection Act For those posts where there is management or supervision of other staff it is the responsibility of that employee to ensure that their staff receive appropriate training (e.g. HISS induction, organising refresher sessions for staff when necessary). Budgetary management If you have responsibility for a budget you are expected to operate within this and under the trust s standing financial instructions (available in the intranet s policies section) at all times.

Smoke Free Trust Barts Health NHS Trust is a smoke free health provider. This means that smoking or vapping and charging electronic cigarettes are not permitted anywhere on our hospital grounds or premises. Our Smoke Free Policy aims to support smokers who are ready to stop and create a smoke free environment. As a healthcare provider it s important we do everything we can to reduce smoking among patients, visitors, staff and the risk of harm to others. All employees are expected not to smoke in their uniform or with Trust ID cards on public display. Frontline clinical staff are expected to advise patients about the risks of smoking and support them by offering a referral to the smoking cessation service and for inpatients also offer nicotine replacement therapy (e.g. nicotine patches) or medicines; to help alleviate the discomfort of nicotine withdrawal. Smokers are up to four times more likely to stop smoking successfully with support than by going it alone and are more motivated to stop smoking whilst in hospital. NHS managers code of conduct As an NHS manager, you are expected to follow the code of conduct for NHS managers (October 2002).
