2 Record # : 9753 Position: CARE MANAGER I (BILINGUAL IN SPANISH) *$1,500 SIGN ON BONUS* Program: Children's Health Homes Location: Warsaw, NY Hours: Full time - 40 hours - Typically Monday-Friday. Hours must be flexible to meet the needs of families. Hillside Children s Center has a new opening for a Bi-lingual (Spanish/English) Care Manager I with our Children s Health Homes program! This position is full time and hours are flexible. Occasional weekend/evening hours may be required. A $1,500 sign on bonus is being offered for this position. The Bi-Lingual Care Manager I for Children s Health Homes will deliver the six core health home services in a manner consistent with the Children s Health Home of Upstate NY (CHHUNY) Care Management model. This includes convening the care team and facilitating a family-driven and youth guided process to develop and implement the service plan. The Care Manager will support and coach caregivers as they navigate the multiple complex child serving systems. The Care Manager will also work with families to plan for transitions, secure culturally relevant services, implement health promotion and recovery oriented practices, and utilize proactive deescalating responses to disruptive behavior. The person in this role must be able to read, write, and speak Spanish. Care Manager responsibilities include, but not limited to: Responds to all referrals and begins outreach by working actively with the referring entity to engage the individual and convene the care team. Screens, and where applicable, conducts the appropriate comprehensive assessment within the stated required time frames to guide ongoing service delivery. Provides coaching and care management service to develop a comprehensive care plan in collaboration with the individual, family, natural supports and identified community agencies. Effectively identifies and uses available resources using current knowledge and awareness of agency and community services, and assumes an influential role in building recommended service options. Acts as an advocate and broker for individuals and families in making referrals to agency services and community programs, and supports linkage. Candidates must be bilingual in Spanish. High school diploma/ged and at least 2 years of experience working with youth/families in the human services field is required. Candidates with a Bachelor s degree in the human services field (psychology, sociology, human services, nursing, counseling, etc.) are preferred. May include some evening and weekend hours. The Care Coordinator II is responsible for the delivery of quality psychosocial care to the youth that we serve. This position works with Clinicians and other care providers to assure overall needs of the client are met. The Care Coordinator II may be assigned to lead projects and mentor Care Coordinator I s as needed. Responsibilities of the Care Coordinator II include but are not limited to: Works as a team member in assessing a youth/family needs, and is responsible for developing, assessing and modifying the individual service plan in collaboration with the youth, family, and multi-disciplinary treatment team. When appropriate, provides psycho-educational training for youth and families in a variety of settings based on group/child/youth/family needs and Care Coordinator experience. Actively involved in the implementation of a strength-based service plan including site/home visits and assessments, identifying and coordinating collateral resources, recognizing gaps and needs, assessing results, and communicating with the multi-disciplinary treatment team. Utilizes experience, education, age-specific knowledge and cultural competence in order to assess and support youth and families to achieve positive and successful outcomes and routinely successfully uses problem-solving skills with self and others. Under regular supervision and direction, the Waiver Service Provider (WSP) performs functions that assist in the care and treatment of children/youth and families and assisting children/youth to develop life and social skills in a community based setting and in establishing a safe physical and emotional environment for children/youth and families in care. Responsibilities include, but are not limited to: Recognizes differentiating children/youth/families behaviors, participates within designated treatment teams to develop service and behavior management plans, and, as appropriate based on skills and experience, executes such plans aimed at assisting children/youth/families to achieve their personal goals and objectives. Support, guide, mentor, coach and reinforce the child in successful functioning in the home or community. Areas of focus may include task completion, communication, socialization, interpersonal behavior, sensory/motor development, community activities, activities of daily living, problem-solving, managing money, and eliminating maladaptive behaviors. Provide planned short-term relief for family/caregivers, through direct care of the child, in order to support the family/caregivers in their capacity to nurture the child. Proactively maintains current knowledge of the children/youth/families emotional/behavior status, assesses the effectiveness of service plans, and supports children/youth/families interactions with family and local community. Provides life skills and social skills training for children/youth/families based on skills, training and experience. The DSP III recognizes differentiating behaviors, participates within designated treatment teams to develop service and behavior management plans, and, based on skills and experience, executes such plans aimed at assisting youth and families in achieving their personal goals and objectives. Bachelor s degree and 1 year of relevant experience OR Associate s degree and 1.5 years of relevant experience OR High School Diploma/GED and 2 years of relevant experience is required. Candidates must have at least 1 year of experience as a Direct Support Professional if he/she does not have a Bachelor s degree.
