2 Record # : 9753 Position: CARE MANAGER I (BILINGUAL IN SPANISH) *$1,500 SIGN ON BONUS* Program: Children's Health Homes Location: Warsaw, NY Hours: Full time - 40 hours - Typically Monday-Friday. Hours must be flexible to meet the needs of families. Hillside Children s Center has a new opening for a Bi-lingual (Spanish/English) Care Manager I with our Children s Health Homes program! This position is full time and hours are flexible. Occasional weekend/evening hours may be required. A $1,500 sign on bonus is being offered for this position. The Bi-Lingual Care Manager I for Children s Health Homes will deliver the six core health home services in a manner consistent with the Children s Health Home of Upstate NY (CHHUNY) Care Management model. This includes convening the care team and facilitating a family-driven and youth guided process to develop and implement the service plan. The Care Manager will support and coach caregivers as they navigate the multiple complex child serving systems. The Care Manager will also work with families to plan for transitions, secure culturally relevant services, implement health promotion and recovery oriented practices, and utilize proactive deescalating responses to disruptive behavior. The person in this role must be able to read, write, and speak Spanish. Care Manager responsibilities include, but not limited to: Responds to all referrals and begins outreach by working actively with the referring entity to engage the individual and convene the care team. Screens, and where applicable, conducts the appropriate comprehensive assessment within the stated required time frames to guide ongoing service delivery. Provides coaching and care management service to develop a comprehensive care plan in collaboration with the individual, family, natural supports and identified community agencies. Effectively identifies and uses available resources using current knowledge and awareness of agency and community services, and assumes an influential role in building recommended service options. Acts as an advocate and broker for individuals and families in making referrals to agency services and community programs, and supports linkage. Candidates must be bilingual in Spanish. High school diploma/ged and at least 2 years of experience working with youth/families in the human services field is required. Under regular supervision and direction, the Waiver Service Provider (WSP) performs functions that assist in the care and treatment of children/youth and families and assisting children/youth to develop life and social skills in a community based setting and in establishing a safe physical and emotional environment for children/youth and families in care. Responsibilities include, but are not limited to: Recognizes differentiating children/youth/families behaviors, participates within designated treatment teams to develop service and behavior management plans, and, as appropriate based on skills and experience, executes such plans aimed at assisting children/youth/families to achieve their personal goals and objectives. Support, guide, mentor, coach and reinforce the child in successful functioning in the home or community.
