2 Free Clinics Columbus Medical Association Physicians CareConnection (PCC) Services Provided: PCC operates a free clinic for walk-in primary care, scheduled specialty care and dental care. Not included in provided care includes: school, work and sports physicals, disability, social security, workers compensation or any other type of disability claims, STD or pregnancy testing. Only 12 patients seen on a first come, first serve basis per clinic. Patients must sign in between 6 p.m. and 7 p.m. Hours: 1 st & 3 rd Monday evenings. Prescriptions can be written and $4 gift cards are given to purchase one month s supply of prescriptions. Free pregnancy testing and pregnancy classes and free ultrasound are available. Mentoring for mothers and host families is also available. Narcotics are not available at this clinic. Hours: Every Wednesday 6:30 p.m. 8:30 p.m. First come, first serve basis. Location: Palmer Road. SW, Reynoldsburg/Etna, Phone: (740) Lower Lights Christian Health Center Services Provided: This clinic focuses on whole person wellness serving Medicaid, Medicare, CareSource, Molina, uninsured and private insurance patients. These clinics provide prenatal care, immunizations, well-child checkups, physicals, routine care and chronic disease management. Services are offered on a sliding fee scale based upon income; however, all patients are seen regardless of ability to pay. This clinic provides routine medical care, including physical exams and treatment of non-emergency illnesses. This clinic does not provide immunizations, TB screenings, dental or prenatal care. This clinic provides medical, chiropractic care, work and school physicals (Fridays only), counseling, vision and dental referrals, diabetes screening and foot checks, medication and lab services and patient education, social services and spiritual guidance. TB testing is also available on Wednesdays before Friday clinics. Interpreters available if you call ahead. Hours: 2 Thursdays and 2 Fridays a month. 6 The Center for Hope Free Medical Clinic Services Provided: Medical clinic for uninsured individuals at or below 200% poverty levels and those with Medicaid or Medicaid eligible offered by Victory Ministries at the Center of Hope. This clinic provides routine medical care and treatment for nonemergent illness. A meal is provided to patients. Spanish interpretation is available. Hours: 2 nd Thursday of the month. 10 For Women & Their Partners Columbus Public Health Women s Health and Family Planning Center Services Provided: Provides pregnancy testing, healthy weight program, smoking cessation sessions, women s annual and focused problems exam, reproductive life planning and birth control options and limited partner care. Level I infertility service, teen health services and a limited screening for sexually transmitted infection and certain cancers. This clinic accepts Medicaid and Medicare as well as cash, Visa and Mastercard. Fees for non-insured clients are based on a sliding fee scale. No one is denied care for inability to pay. Services include mammograms, Pap tests, diagnostic services, if needed and case management services. 11 Other Clinics Columbus Cancer Clinic Services: Provides low-cost cancer screening exams for men and women (Do not have to live in Franklin County). For men, a head to toe exam including skin, lymph nodes and testicles is available. For men age 50 and older, a prostate exam and a PSA blood test is provided. For women age 18 and older, a head to toe exam including skin, lymph nodes, breast and Pap test is provided. Screening mammograms are also available for women age 35 and older. Medicare and Medicaid are accepted as well as all major insurances for mammograms. A sliding fee scale is available based on income. Hours: By appointment. Phone: (614) Healthcare for the Homeless PrimaryOne Health Services: Provides health care, dental, vision, case management and outreach services for individuals with established homeless status. Intake assessment is required. Location: Various sites. Hours: Monday Friday 8 a.m. 5 p.m. Lung Health Clinic The Breathing Association Description: Provides lung health screenings, on-going lung health care, comprehensive assessments and physicial exams, chest x-rays, spirometry testing, asthma and COPD education and medication assistance to individuals with pulmonary or tobacco related diseases for those with or without insurance. Initial assessment and physical exam are also provided if the client doesn t have a health care provider. Work physicals are also provided at no cost to those who financially qualify at or below 200% of the poverty level. No co-pay will be collected. Financial screening is completed through the HEAP office at the same location for those served at the 1520 Old Henderson Rd. location. 12 Vision Care V PrimaryOneHealth Description: Provides eye exams and eyeglasses. Lowest eye exam charge, based on sliding fee, is $35. Lowest cost for glasses is $30. You may bring your own frame and we will price the lenses accordingly. Some insurances accepted as well as crash or credit card. Services are provided in connection with social service agencies in the Columbus area that submit referrals to the Vision Care Outreach Program. Ask a social service agency you re currently working with to see if they are a partnering agency with Prevent Blindness. Phone: (614) Faith Mission Vision Clinic Description: Free comprehensive eye exams for those without insurance that covers vision and at 200% poverty level. Serving the homeless in any shelter and referrals from other clinics based on the need. Residents of Faith Mission and referrals through Prevent Blindness are given free eyeglasses at the clinic. All others will receive their eyeglasses prescription and a voucher for free eyeglasses through Lenscrafters/Sears if they qualify.
