1 Basic Nurse Assistant Program Spring 2018 The CNA program has very strict attendance policies. Please note: ALL CLASS AND LAB TIMES ARE REQUIRED BY THE ILLINOIS DEPARTMENT OF PUBLIC HEALTH. Attendance on the first day of class is MANDATORY and cannot be made up. If a student must be absent on a subsequent day, no more than two class periods may be missed and made up during a scheduled make-up day which may differ from your regular scheduled class time. Any additional absences or failure to make up missed time will result in dismissal from the program. Attendance the first day of clinical is also MANDATORY. Registration Requirements: 1. New Students New LLCC students may be required to attend a New Student Orientation session prior to registration. You may contact Registration at or your local Education Service Area (ESA) for more information regarding orientation. 2. Reading Students must meet one of the following criteria: ACT reading score of 22 or higher SAT Writing/Reading score of 520 or higher Have passed Reading 098 LLCC Reading Assessment with a score of 60 or higher You may contact LLCC s Placement and Testing office or your local Education Service Area for further information about the Reading Assessment test. 3. Criminal Background Check A fingerprint-based criminal background check is required by law for all nurse aide students and must be completed prior to registration. Students will need to complete the attached Health Care Worker Background Check Authorization and Disclosure form legibly and in its entirety. Please include an address or fax number to where you would like your fingerprint paperwork sent and return the completed form to one of the following: CNA Office, Montgomery Hall room 1102 Registration Advising Local ESA a copy to CNA.program@llcc.edu Students who have previously completed a background check must complete a new Health Care Worker Background Check Authorization and Disclosure form to grant LLCC permission to access previous background check information. Once the disclosure form is processed, students will be provided with a Livescan form to take to an approved vendor for fingerprinting. The student will be issued a receipt by the vendor that will need to be turned in utilizing one of the above listed methods. Once the background check steps are completed and all required paperwork has been submitted, students will receive an stating they have completed the background check requirement for the CNA course. Students with disqualifying convictions on their background checks will not be allowed to participate in the CNA course. Students may apply for a waiver and enroll once it is received.

3 Crimes that disqualify you from working as a nurse s aide (these include both felonies and misdemeanors): 1. Violations under the Criminal Code of 1961 a. Solicitation of murder, solicitation of murder for hire b. First degree murder, intentional homicide of an unborn child, second degree murder, voluntary manslaughter of an unborn child, involuntary manslaughter and reckless homicide, concealment of homicidal death, involuntary manslaughter and reckless homicide of an unborn child, and drug-induced homicide c. Kidnapping, aggravated kidnapping, child abduction, and aiding and abetting child abduction d. Unlawful restraint, aggravated unlawful restraint, and forcible detention e. Indecent solicitation of a child, sexual exploitation of a child, sexual misconduct with a person with a disability, exploitation of a child, and child pornography f. Assault; aggravated assault; battery; battery of an unborn child; domestic battery; aggravated domestic battery; aggravated battery; heinous battery; aggravated battery with a firearm; aggravated battery with a machine gun or a firearm equipped with any device or attachment designed or used for silencing the report of a firearm; aggravated battery of a child; aggravated battery of an unborn child; aggravated battery of a senior citizen; or drug-induced infliction of great bodily harm g. Tampering with food, drugs, or cosmetics h. Aggravated stalking i. Home invasion j. Criminal sexual assault; aggravated criminal sexual assault; predatory criminal sexual assault of a child; criminal sexual abuse; aggravated criminal sexual abuse k. Abuse and criminal neglect of a long-term care facility resident l. Criminal abuse or neglect of an elderly person or person with a disability m. Endangering the life or health of a child n. Ritual mutilation, ritualized abuse of a child o. Theft; theft of lost or mislaid property; retail theft; identity theft; aggravated identity theft p. Financial exploitation of an elderly person or a person with a disability q. Forgery r. Robbery, armed robbery, aggravated robbery s. Vehicular hijacking, aggravated vehicular hijacking t. Burglary, residential burglary u. Criminal Trespass to a residence v. Arson, aggravated arson, or residential arson w. Unlawful use of weapons, unlawful use or possession of weapons by felons or persons in the custody of Department of Corrections facilities; aggravated discharge of a firearm; aggravated discharge of a machine gun or a firearm equipped with a device designed or used for silencing the report of a firearm; reckless discharge of a firearm; aggravated unlawful use of a weapon; unlawful discharge of firearm projectiles; unlawful sale or delivery of firearms on the premises of any school x. Armed Violence 2. Violations under the Wrongs to Children Act a. Endangering life or health of a child b. Permitting sexual abuse of a child 3. Violations under the Illinois Credit Card and Debit Card Act a. Receiving stolen credit cards or debit cards b. Receiving a lost or mislaid card with intent to use, sell, or transfer c. Selling a credit card or debit card, without the consent of the issuer d. Using a credit or debit card with the intent to defraud e. Fraudulent use of electronic transmission 4. Violation under the Criminal Jurisprudence Act a. Cruelty to children

4 5. Violations under the Cannabis Control Act a. Manufacture, delivery, or possession with intent to deliver or manufacture cannabis; cannabis trafficking; delivery of cannabis on school grounds; delivering cannabis to a person under 18; calculated criminal cannabis conspiracy 6. Violation under the Nurse Practice Act a. Practice of nursing without a license 7. Violations under the Illinois Controlled Substances Act a. Manufacture or delivery, or possession with intent to manufacture or deliver, a controlled substance other than methamphetamine, a counterfeit substance, or a controlled substance analog; controlled substance trafficking; manufacture, distribution, advertisement, or possession with intent to manufacture or distribute a look-alike substance; calculated criminal drug conspiracy; criminal drug conspiracy; delivering a controlled, counterfeit or look-alike substance to a person under 18; and engaging or employing a person under 18 to deliver a controlled, counterfeit or look-alike substance 8. Violations under the Methamphetamine Control and Community Protection Act Please note: You may have been convicted and not sent to jail. Often people are fined or given probation, but these are still convictions.

7 Course Requirements 1. Physical examination: Physicals must be completed and turned in to instructor by week 2. Physicals must be completed on the LLCC CNA physical form. A recent physical completed within a year of the last day of class, may be transcribed by a physician on the LLCC CNA physical form. 2. Tuberculosis test: 2-step skin test, Quantiferon blood test or chest x-ray prior to attending to the clinical experience If past positive skin test, a chest x-ray or Quantiferon blood test is required. May be obtained at local health department or by physician. 3. Uniform: Clean mostly white or black leather or mesh tennis shoes Watch with a second hand - Smart watches are not permitted in the CNA classroom, lab or clinical setting. Royal blue scrub top, pants and gait belt Must be purchased from LLCC Bookstore 4. Textbook: Title: Mosby s Essentials for Nursing Assistants, 5 th Edition by Sorrentino, ISBN ; and accompanying workbook, ISBN Must use printed version. E-Books are not be permitted. Begin your workbook as soon as you receive it. Instructors prefer the first 6 chapters to be completed prior to class, however you may work ahead on other chapters. You are not required to complete chapters 7 & 28 and it is not necessary to do the Case Studies & Additional Learning Activities. 5. State competency written examination Given at monthly scheduled times after successful completion of classroom and clinical portion of class. Must be taken in order to successfully complete the course and be placed on the Nurse Aide Registry. General Information 1. Course content: This course is 132 hours; 86 hours theory/lab and 46 hours clinical and has been approved by the Illinois Department of Public Health. 2. Apparel: Students may wear street clothes to class. Uniform consisting of clean, mostly white or black tennis shoes and royal blue scrub top and pants are required for labs and clinical. 3.
