2 Bureau of Workers Comp (BWC) is a state insurance program that is designed to protect the employee if they incur a work-related injury. Hospice of the Western Reserve has been self-insured since 2012 and works with Care Works Consultants. This allows for more control over the process. This allows for immediate approval and treatment. This allows employees to get treated and to return to work more quickly. This allows us to use transitional work which saves time, money, and protects your job.

3 Careworks Consultants Inc. (CCI) assists Hospice of the Western Reserve to manage the financial side of the Workers Compensation Program. They can be reached at (found on card). Packets available from Supervisors.

4 The Workers Compensation Coordinator (WCC) manages the day-to day aspects of the Workers Compensation Program. The WCC must be notified of all changes in treatment and time taken off from work. Kathy Maltry and Chris Provard (the WCCs) can be reached at

5 The next section will discuss what to do in case of a work-related injury.

6 1. Notify your supervisor immediately! 2. Complete the Ohio BWC First Report of Injury (FROI) form and the Employee Report of Accident, Injury, and Illness form found in the Self-Insured Injury Reporting Packet. Forward both completed forms to Human Resources within 24 hours of your workplace injury. Send via confidential fax to

7 3. Seek medical treatment from an appropriate in-network or Bureau certified provider. See packet for recommendations for your injury management or call the Employee Health for assistance. Remember any physician must accept Worker s Compensation to be your provider of care. 4. Urgent Care is recommended for non-life-threatening injuries followed by follow up with an Occupational Health specialist (all referrals must be approved prior to the visit).

8 Present your Self-Insured Workers Compensation identification card to every medical provider that treats you for a work-related injury. Follow up with HWR s Workers Compensation Coordinator at after you have received treatment. Always have your card on your person.

9 When you receive treatment, complete the First Report of Injury (FROI) form. This is a Ohio claim form from the BWC. There is one in your injury packet that you can use or the Medical treatment facility will also have one.

10 Complete an Authorization to Release Medical Information form. This form allows CCI and HWR to work with the physicians involved in your care. It is important to fax these forms to the Employee Health office immediately to help in managing your claim.

11 1. Call Notify your Supervisor as soon as possible. 3. Supervisor notifies the Workers Compensation Coordinator.

12 The next section will tell your what to do after receiving care for your work-related injury.

13 Employee is responsible for following up with his/her Supervisor and the Worker s Compensation Coordinator with any changes in his/her condition. If released to return to work, complete the Hospice of the Western Reserve forms on the next page.

14 There are three forms that must be completed as soon as possible and submitted to your Supervisor (found in SI packet).

15 The Witness Form is optional, but if it can be completed, it must be completed as soon as possible and submitted to your Supervisor.

16 If released to Return To Work with restrictions, the physician must complete a MEDCO-14 form and return it to the Workers Compensation Coordinator to determine if a Transitional Work Plan can be started to accommodate work restrictions (found in SI packet).

17 If your physician prescribes a medication after your injury visit, use this card when filling the prescription at one of the pharmacies participating in our network. This will help to avoid any difficulties with billing.

18 The Transitional Work Program (TWP) assists you in returning to work when you have restrictions due to an injury. Limited time frame (0-60 days). May include work at another site and/or with another team. Duties will depend on restrictions identified by the physician of record (POR). This is not light duty, but instead is an accomodation to meet the restrictions outlined by the Physician of Record (POR).

21 Help yourself to prevent injuries by: Not texting and driving! Driving the speed limit. Knowing your surroundings at all times. Using proper techniques when lifting or moving anything. Taking care of yourself by getting enough sleep and being healthy. Taking your time!
