3 Company History In 1996 a business partner and I set up a local Domiciliary Care Agency to provide care to people in their own homes. Over a 12 year period this company grew extensively until it was decided that the best way to grow further was to divide the company into two. So it was that in April 2006 that Town and Country Homecare Limited was formed by my family and me. Since then the company has grown dramatically, which is all the more pleasing because it is a family run business. The business managers in the office are my two daughters-in-law Natalie and Claire, with my son Guy (Claire s husband) taking care of the accounts and marketing side of things, as well as being the Company Secretary. Sandra Gower is our Registered Service Manager who is supported by her two full time co-ordinators Debbie Piesse and Sally Heath and a team of part time co-ordinators. This very experienced office team use their broad knowledge of care provision to ensure a high quality service that supports the very diverse care packages that we look after. They use their expertise, along with the sophisticated computer program to co-ordinate our highly trained care workers in the field. Our Private Care Service business looks after more than 100 service users in their own home, while we still continue to provide services to both Kent & East Sussex County Councils. We have also opened a Mobility Shop called Town & Country Homecare Mobility in Tenterden high street to further help service the needs of our local community. 4 Private Care Service & Critical Visit Monitoring How we can make your life just a little bit easier.... Care homes are often very expensive, and moving from your home into one of these can be quite traumatic, but there is another option which can offer a good alternative & meet your needs much better. We are able to provide you with a package of care tailored to suit your particular needs. Our staff can help you with both Personal Care and/or Domestic Work. This could give you many more years living in your own home at a fraction of the cost of living in a care home. On the page opposite you will see a full list of the services we offer. We also provide Sleep-In or Awake & Working services at night from 10.00pm to 7.00am. We now operate a system called Critical Visit Monitoring Town & Country Homecare Limited constantly strives to offer its service users a better service. Critical Visit Monitoring delivers both comfort and peace of mind to Service users and their families. The system can be set up to deliver automated alerts to our team of coordinators both in and out of office hours when a visit hasn't started by a certain time. This allows us to contact the carer who was scheduled to carry out the visit to ascertain the reason for the delay. If there is a major problem, which without the system we would not have known about, we are now able to make alternative arrangements by utilising our stand-by carers while keeping the service user informed of the situation. Alerts can be set up to activate at varying time scales, depending on the nature of a service users requirements. For example someone who is Diabetic would be deemed to need Critical Care so the alerts are set up to activate within 15 minutes of the planned visit start time. There have been many instances where service users have received better outcomes since this system was put in place. Page 3

If you live outside of this area, we may still be able to offer you a Private Care Service but it will depend on the type of package that you require. The duration of the required visits is often a key factor in deciding if we would be able to take the package on, as carers will often be happy to travel further distances for longer visits. 7 Self Directed Support, Kent Card & Direct Payments (SDS) Self Directed Support Self-Directed Support is a new system. It is about people being in control of the support they need in order to live their life as they choose. Some people manage their support on their own. Others need help from family or friends or people who are paid to help. In the old system, professionals made all the decisions about the care/ support people could have and who would provide it. The person needing support often had no control. With Self Directed Support, once it has been decided what funding the individual will receive from social services, then they or their advocate, family or friend can choose the care they wish to receive and then pay for it using one of the ways detailed below. The Kent Card The Kent Card is a way of receiving your funding from Kent County Council once it has been determined how much you are entitled to receive. It enables individuals to pay for the services that they require from private sector companies and is particularly helpful for those who don't have bank accounts. If the funding received from social services covers only part of the cost of care then the card can be topped up (by you) so that just one payment can be made to the service provider Direct Payments Direct payments are made to individuals (like above) once it has been determined the level of funding they are entitled to receive. Payments are made into a bank account operated by the service user. This enables them to make payments for the services that they require from private sector companies. Because the individual has control of the funds, payments can then be made by any method, to the service provider. 8 Care Quality Commission The Care Quality Commission replaced the Commission for Social Care Inspection (CSCI) in early It is the independent inspector and regulator of health and social care in England whose aim is to make sure that better care is provided for everyone, whether that s in hospital, in care homes, in people s own homes or elsewhere. They regulate health and adult social care services, whether provided by the NHS, local authorities, private companies or voluntary organisations. They protect the rights of people detained under the Mental Health Act. In 2011 the star rating system that helped service users understand the quality of care that care agencies like ours provided was scrapped, however it has not yet been replaced. Latest Inspection Reports & Our Rating Following a key inspection care agencies like ours, are awarded a rating by the Care Quality Commission that best describes the quality of care that we provide. The ratings scale is as follows: 3 Stars - Excellent 2 Stars - Good 1 Star - Adequate 0 Stars - Poor Town & Country Homecare Limited were awarded the top level of 3 stars in June 2007, and we are very pleased to report that we have retained this rating in each subsequent year to date. 9 Our Team Our Office Team Is Comprised As Follows: Deslys Day-Jolly Company Director I have been working in the care industry for over 30 years, starting as a community home carer, back in I spent the following 14 years working for Kent County Council, Social Services Dept, in various management positions in residential homes for elderly people, finishing as the Manager of a Linked Service Centre. I then spent 12 years as co owner of a successful care agency. After the division of this agency I started my own business Town and Country Homecare Limited. The company has grown considerably since its inception, which is all the more pleasing as it is now my family and I that run it. Qualifications: BTEC Continuing Education Certificate in Management Studies. Natalie Day Business Manager Natalie is my daughter in law and has been working for me since She has experience in providing hands-on care; has worked as a care co-ordinator and then moved into management covering personnel issues, invoicing, payroll and all other aspects associated with running a care agency. Natalie lives locally and has two children. Qualifications: Natalie has completed her NVQ level 3 in Business & Administration and has now started the NVQ4 course in the same field. Claire Day Business Manager Claire is married to my son Guy and joined the company in She qualified as a paediatric nurse in 1995 and then as a health visitor and nurse prescriber in She brings with her a wide range of experience in both the hospital and community setting. Claire and Guy have a 2 daughters. Qualifications: Registered Nurse (Child) Dip HE, BSc (Honours) Child Health, BSc (Honours) Public Health, RHV - Registered Health Visitor, ENB Teaching & Assessing and Registered Nurse Prescriber Guy Day Company Secretary Guy is my son and is married to Claire. He retired from the City in He joined the company from the outset and runs accounts and marketing for the business. His aim is to expand our Private Care Service. Guy also owns in partnership, Jackie s News, a newspaper delivery business based in Tenterden, the coffee kiosks at Headcorn & Staplehurst Train Stations and also Town & Country Homecare Mobility. Sandra Gower Registered Service Manager Sandra has 28 years experience in the care industry, previously managing a reputable nursing home, but joined us in 2008 and settled very well into the our team. In June 2011 she was promoted to Registered Service Manager. She is married with 3 children & lives locally. Qualifications: Registered General Nurse, Diploma of Higher Education, NVQ4, RMA ENB , D31 / 32 Assessor & NVQ level 3 in Business & Administration. Debbie Piesse Care co-ordinator Debbie has been with me for 12 years, starting out as a carer and progressing into the office as a care co-ordinator. She lives with her family which includes her two grown up children. She is also the stand in manager in the office whenever Sandra is out of the office visiting clients or attending meetings. Debbie points out that her surname is pronounced peace. Qualifications: NVQ level 3 Business & Administration, NVQ level 3 & 4 in Care. Sally Heath Care co-ordinator Sally has 24 years experience in the care industry, having worked in a nursing home for 15 years and then with us as a carer for 9 years, during which time she has trained for and become our (in house) Manual Handling Trainer. She is a valued member of the Care co-ordinator team, while still carrying out initial visits from time to time. Qualifications: Manual Handling Trainer, NVQ level 2 & 3 in care New Domestic Cleaning Service 9.77 per Hour This great new price for a domestic cleaning service (reduced down from per hour) was introduced to give a quality service at an extremely competitive price, but with the peace of mind of having CRB checked carers / cleaners providing the service in your own home. Escort Duties (40p per mile)- Hospital visits & Shopping Etc (min 1 Hour) Monday - Friday Only. Bank Holidays - Will be charged at double the prices shown. Christmas Day, Boxing Day, New Years Eve ( After 7:00pm ) & New Years Day Will be charged at treble the prices shown. ( A one off set up fee of 25 will be charged when setting up your initial care package ) Sleep-ins - (10:00pm to 7:00am) This is when one of our care workers sleeps at your house and included in the price are 2 call ups where they will get up and help you. Awake & Working - (10:00pm to 7:00am) This is when one of our care workers stays at your house and there are 3 or more call ups where they will get up and help you. This comprehensive package of care costs per week and helps you in your own home, 3 times a day, seven days a week and still with considerable savings compared to the cost of living in a care home.
