Please note, the Records Imaging Center moved to the Human Resource Center in 2017 and will no longer reimburse Chemung ARC for custodial services. A decrease of $5,850 is reflected in the attached budget. Custodial services include, but are not limited to, vacuuming, dusting, cleaning the break rooms and bathrooms, and emptying garbage cans. The Office of People with Developmental Disabilities (OPWDD) has approved $451,796 in state aid. This is an increase of $901, as compared to The Southern Tier Industries vocational training program serves over 230 adults with developmental disabilities by matching each individual with an appropriate vocational opportunity, ranging from competitive, to supported, to sheltered employment, with an emphasis on community inclusion as much as possible. Recreation services include leisure, social and therapeutic activities to adult County residents with developmental disabilities. Services are provided within community settings, such as City and County parks, recreational centers and local schools, in order to integrate the participants into community-based services and events wherever possible. 4 In 2017, the Records Imaging Center hired County personnel to support their scanning needs. Chemung ARC will no longer assist with this program, for a reduction of $40,000 in this agreement. Of the $40,000, $20,000 was in the Records Imaging Center budget, and $20,000 in the Department of Social Services budget. The Shredding program budget will remain $15,000 in Shredding services are provided to various Chemung County departments as well as budgeted in each department s respective account Outcomes for ARC Vocational Training Program: Forty percent of individuals that are identified to have a goal of integrated employment will be enrolled in supports such as: community prevocational services, Employment Training Program, Pathway to Employment, Supported Employment or directly placed for competitive employment. In the event Chemung s usage is above the 8 bed capacity, the rate will be $360 per bed per day. Therefore, the Steuben County non-secure detention bed contract is requested not to exceed $168,630. In Chemung County, many Human Service providers assist the same children, youth, and families from childhood to adulthood. While services are necessary, the utilization of multiple providers hinders the ability to streamline and communicate needs among agencies. The purpose of COMET is to create a county-wide integrated information system for children and youth that will interact with many, if not all, the service providers. The goal is to deliver a comprehensive longitudinal perspective on each child in the County, leveraging the technology to provide better, faster and continuously improved services. Per the March 13, 2017 Office of Mental Health award letter, Family Services of Chemung County will receive $1,072,062 in state aid. This is an overall increase of $58,971 as compared to the 2016 amendment request. Of the $58,971 increase, $54,000 can be attributed to budgeting the full annual amount of the Community Support Program. Funds will be used in a collaboration between Family Services and local nursing homes resulting in enhanced services to individuals in a transition phase from their facility to two of the three skilled nursing facilities. Next, the ICM Manager program in the amount of $22,189 will be eliminated from the contract and shifted to the Children s Health Home Care Management program in the amount of $23,228. The total increase due to the shift is $1,039. Lastly, a $3,932 increase is due to cost of living adjustment across various programs OMH programs. Children s Integrated Services preventive funding will total $269,050 in This is an overall decrease of $8,950, as compared to The Therapeutic School Based Mental Health Item # 5

14 (TSBMH) program decreased by $25,000, based on prior year actual expenses. Whereas, cost of living adjustments account for the $16,050 increase across the Family Services preventive workers, Bridger, and Community Prevention Coordinator. The Elmira Heights Therapeutic School Based Mental Health program will not exceed $17,143 in You may recall per resolution , Federal Salary Sharing funds will be used to pay for 65% of this program, while the Elmira Heights School District will be charged 35% in For the 2017 contract year, the following outcomes will be implemented: Mental Health Clinic Outcomes: The average number of visits for those individuals not receiving medication management will not exceed 20 visits 75% of all requests for service will be offered an intake appointment within five business days. 75% of individuals who have been referred from the ER or Hospital will have a visit with the psychiatrist within 30 days. Geriatric and Negative Symptoms Outcomes: 90% of individuals referred will agree to enroll in services. STIRS Outcomes: 80% of individuals served each month will receive Community Rehabilitation Support. 25% of individuals served each month will receive Intensive Rehabilitation. 10% of individuals served each month will receive Ongoing Rehabilitation Services 30% of individuals served each month will receive Clinic Treatment. Health Home Care Management Program Services (Adults) Outcomes: 80% of individuals referred through community based referrals will successfully engage in Health Home Care Management Services. 25% of individuals referred by the HHUNY will successfully engage in Health Home Care Management Services. 75% of individuals receiving Health Home Care Management Services will not be hospitalized or incarcerated while enrolled in services. Crisis Services Outcomes: 65% of all crisis contacts will be handled successfully by telephone. Item # 5

15 Of the individuals seen face to face, no more than 15% will require evaluation at a 9.39 hospital. Adolescent Sex Offender Program Outcomes: 90% of youth served will remain out of higher levels of care during, or for 3 months post discharge from the program 90% of youth will not have a new offense while involved in program. 75% will be successfully discharged from treatment Bridger and two Facilitators Positions Outcome: Family Services will increase their support, training and supervision of the staff assigned to Children s Integrated Services through quarterly case reviews in group format, of at least two cases per quarter. Child Advocacy Center 80% of victims referred to the MH counselor will be offered an appointment within 3 business days Baseline the percentage of children who are successfully discharged as demonstrated by Pre/Post Assessment scores School Based MH Less than 20% of children referred will decline services Of those enrolled in SBMH, less than 25% will be referred for counseling outside of the school setting Baseline that average number of days between referral and assessment completion There are no additional changes to the 2017 Family Services contract. 17 FAMILY SERVICES 2017 CONTRACT REQUEST DSS DSS Forensics Screening - Forensics activities are aimed at linking the consumer to the service system and at coordinating the various services in order to achieve a successful outcome. The objective of care coordination in mental health system is continuity of care and service. Adolescent Sex Offender Program the Family Services of Chemung County, Inc. (FSCC) Mental Health Clinic's Sexual Abuse Therapy Program (SATP) will provide the Adolescent Sex Offender Program through outpatient community-based therapeutic intervention. The program assists the adolescents to control and reduce the risk of their deviant sexual behavior as well as develops and demonstrates pro-social interpersonal skills. The program's therapeutic interventions include individual, group, and family therapies, education, and coordination and cooperation among all involved persons and agencies. Outpatient therapy can only be effective when all parties recognize the serious risk that the adolescent sex offender presents to the community and are willing to work together to provide supervision and monitoring of the client. Specialized assessment and therapy services must be utilized as traditional therapy modalities typically fail to focus on treating the client whose participation is not voluntary and whose risk behaviors are obsessive and addictive. Representative payee Family Services acts as the receiver of United States Social Security Disability or Supplemental Security Income for mental health clients who are not fully capable of managing their own benefits. The Representative payee program has three case managers that assist clients with out-patient mental health services manage their money, and provide protection from financial abuse and victimization. Mental Health CAC Mental Health Counseling Services - Mental Health Counseling Services provides immediate crisis intervention and mental health services for victims and non-offending family members or siblings of physical or sexual abuse, serious domestic violence, and child fatality cases. The Therapist will assist in working through the crisis, assess the need for mental health services, and provide appropriate mental health services and/or referral recommendations for additional mental health services as needed. Mental Health CIS Children s Integrated Services (CIS) Positions MH - The Coordinated Care Services, Inc. (CCSI) Bridger (1), CCSI Facilitator (2) and the Community Prevention Coordinator are provided by funding from the County and supervised directly by county personnel. CCSI Bridger The Bridger position assists families with children between the ages of 0 and 18 years successfully transition to a more independent, community based living. Services include support, advocacy, linkages, diligent follow up and reengagement. CCSI Facilitator The Facilitator position assists families with children between the ages of 0 and 18 years that are experiencing significant behavior issues and/or an emotional disturbance in wrapping services and supports around the child and family to maintain the child in the home, school and community. Community Prevention Coordinator The Community Prevention Coordinator position facilitates and Item # 5 implements suicide prevention strategies at the local level through education, monitoring, and coordinating activities.

18 Therapeutic School Based Mental Health (TSBMH) - TSBMH is clinical staff provided by Family Services to provide any necessary ancillary staffing appropriate for the provision of Individual Education Plan (IEP) counseling and of site school-based counseling for all children in the Elmira City School District (ECSD) who meet the following criteria: Mental Health OMH a. Not be a student receiving services in a 8:1:1 classroom b. Have counseling approved by the Committee on Special Education of the Elmira City School District c. Referred according to the ECSD school mental health counseling process. Mental Health Clinic The Mental Health Clinic Program is a full service comprehensive out-patient program meeting all State requirements and regulations, offering assessment, brief and ongoing therapy, consultation, and education and training. Services are available to anyone in the community, including children, adolescents and adults. Southern Tier Integrated Recovery Services (STIRS) PROS Family Services and Capabilities operate Southern Tier Integrated Recovery Services (STIRS), a PROS Program assisting individuals with functional mental health-related disabilities to successfully identify, pursue, and achieve their life goals at home, work, school, and in the community. STIRS will encompass the fundamental components of recovery, including individualized, personcentered, and strength-based services offered within a holistic, growth-oriented, peer supported framework, promoting consumer self-[direction, empowerment, and responsibility, based upon respect and hope. AOT Coordinator Assisted Outpatient Treatment (AOT) Coordinators are responsible for monitoring and overseeing assisted outpatient treatment (AOT) programs. Advocacy/Support Services Advocacy/support services may be individual advocacy or systems advocacy or a combination of both. Individual advocacy assists consumers in protecting and promoting their rights, resolving complaints and grievances, and accessing services and supports of their choice. Systems advocacy represent the concerns of a class of consumers by identifying patterns of problems and complaints and working with program or system administrators to resolve or eliminate these problems on a systemic, rather than individual basis. Intensive Case Management (ICM) Managers/Service Dollars/Health Home Care Intensive Case Management (ICM)/ Health Home care programs promote optimal health and wellness for adults diagnosed with severe mental illness, and children and youth diagnosed with severe emotional disorder. Wellness and recovery goals are attained by implementing a person-centered approach to service delivery and ensuring linkages to and coordination of essential community resources. Case Managers work in partnership with recipients to advance the process of individuals gaining control over their lives and expanding opportunities for engagement in their communities. Transition Management (TM) Transition Management Services (discharge planning) programs provide support for improved community service linkages and timely filing of Medicaid applications for seriously and persistently mentally ill (SPMI) consumers being released from local correctional facilities. The TM focus will be in obtaining post-release services for these consumers. Crisis Intervention Crisis intervention services, applicable to adults, children and adolescents, are Item # 5 intended to reduce acute symptoms and restore individuals to pre-crisis levels of functioning.

19 Forensics/COPS Transitional Funding Forensics activities are aimed at linking the consumer to the service system and at coordinating the various services in order to achieve a successful outcome. The objective of care coordination in mental health system is continuity of care and service. Medication Grant Program The medication grant program provides medications and other services need to prescribe and administer medication for individuals with a mental illness who leaves the local jails, state prisons or hospitals and have applied for Medicaid. Nursing Home Service - The Psychiatric Nurse Practitioner travels between the two skilled nursing facilities providing approximately eight hours a week at each facility the prescription of psychotropic medications. This collaboration between the nursing homes, Family Services will be further enhanced by the Elmira Psychiatric Center temporarily sending unit staff during a transition phase from their facility to two of the three skilled nursing facilities, and coordinating services with their Mobile Integration Team.
