The care is based on comprehensive assessments, ensures continuity, is service user centered, culturally sensitive and evidence based. 3 KEY ACCOUNTABILITY EXPECTED OUTCOMES Nursing Practice Practice meets the standards of the profession, ethical and relevant legislated requirements. Accepts responsibility for safety and quality of nursing practice and conduct. Accepts accountability for action, omissions and commissions. Nursing practice is provided in a culturally safe manner and supports the principles of the Treaty of Waitangi/Te Tiriti o Waitangi: The service user and their family/whanau determines care as culturally safe At all times applies the nursing process: skilled assessment, planning, implementation of best practice and evaluation. The environment is managed to ensure safety. Practice reflects competence in assessment, planning, implementation and evaluation of nursing care and clinical tests required to meet service user needs. Undertakes DAO responsibilities if appointed in accordance with training and legislative requirements Management of nursing care All service users receive a comprehensive and accurate nursing assessment to identify needs and risks/ early warning signs that will affect care plans and interventions required. Demonstrate skill in clinical health assessment in the clinical setting. There is regular assessment/ re-evaluation of wellness and progress, and regular MDT review. Is alert to the early warning signs and acts proactively. Nursing care is provided in a timely manner, according to a documented care plan and the skills and knowledge competencies for the specialty. Clinical forms and documents are up to date. All practice interventions are managed in a safe and appropriate manner and according to best practice guidelines. e.g. administration of interventions, treatments, medications Specialist nursing interventions are provided, within the MDT. eg administration of intramuscular injections and metabolic screening. Documentation is accurate and maintains confidentiality of information Handover of service user information is managed using best practice guidelines The service user has adequate explanation of the effects, consequences and alternatives of proposed treatment options Nursing action is appropriate to protect the nurse and others when faced with unexpected responses, confrontation, personal threat or other crisis situations Service user progress/outcomes are evaluated in partnership with the service user and their family/whanau Health education is appropriate to the needs of the service user within a nursing framework Evaluation/reflection on the effectiveness of nursing care is undertaken with peers to achieve improvement Plans include targeted outcomes and goals negotiated with the service users and their family/whanau Specific clinical interventions are implemented in accordance with the Management/Risk Plan Collaborative note writing is completed with the service user where possible Waitemata District Health Board -JOB DESCRIPTION - MH RN Community Acute Feb 2015 Page 3
