4 What is the purpose of this workbook? The information that you record in this book will allow you to demonstrate your knowledge and understanding of the practice skills required to support people who have difficulty in moving about. It is a record of your learning and can be used as evidence of your professional development and may contribute towards your SVQ in Care. While it is a record of how you support people in your workplace, it also relates to the unit HSC360 DK9V 04 at Level 3 Move and Position Individuals How do I use this workbook? Complete each section as you find out about the practice skills relating to that section. You should relate the notes you record to your own place of work and the person or people you support. Remember this is your own workbook and can be used to evidence your knowledge for SVQ level 2 DK49 04 (HSC223) Contribute to moving and handling individuals so please remember not to mention people by name (initials only) in this book. The columns on the right hand side of the workbook provide space to claim evidence knowledge points for SVQ level 2. When you have completed the book and can demonstrate your practical competence to your supervisor, you should arrange to meet with a moving and assisting facilitator or coach and discuss your progress. Following a successful demonstration of your skills, knowledge and understanding of moving and assisting you should all complete the statement of competence at the end of this book and transfer this statement to your CPD portfolio. If you are working towards your SVQ level 2 Health and Social then the completed workbook along with the signed Competency Statement should be presented to your SVQ Assessor to validate and authenticate your knowledge and claim the evidence within the knowledge part of your SVQ Units. Once your Assessor has validated your knowledge they will provide a statement which will acknowledge your evidence and complete a report stating this within the last page of your workbook. Your completed workbook should then be presented along your SVQ Portfolio when you have completed your SVQ. Page 4 of 18
