1 Dorset HealthCare University NHS Foundation Trust Individual Placement (Job) Description Placement F2 - Emergency Department The department The department has 5 consultants, 1 associate specialist, 8 middle grades and a team of 8 junior doctors (4 x FY2s, 4 x GPVTS) providing 24 hour shop floor cover. Nightshifts are covered by a junior doctor and a middle grade doctor with a consultant on-call from home. There are also nurse practitioners during the day until 22:00 and a large team of nurses, health care assistants and a porter. It comprises a minors and majors/resus department dealing with the emergency admissions from helicopter (helipad next to the department), ambulances and walk ins. The hospital has trauma unit status and the ED has a fundamental role in the trauma service provided. We also see paediatrics and run a 24/7 stroke thrombolysis service alongside the stroke team. The type of work to expect and learning All F2 doctors join the SHO rota and are expected to be in the department for the duration of their shift seeing either major or minors patients. This involves clerking, investigating, diagnosing and referring to specialties if indicated. You will see a range of patients from fractures and lacerations to strokes and MIs providing endless learning with the benefit of senior support at all times. You will also be involved with trauma, crash and stroke thrombolysis calls. It is generally fastpaced, meeting the government s 4 hour target. There are departmental teaching sessions once a week for 4 hours provided by the middle grades and consultant and the medical team. These sessions also provide supported teaching for the SHOs. Audits are assigned at the beginning of the post with a consultant mentor. F2s will learn to: Take appropriate histories and examine patients efficiently Order appropriate investigations Assess risk and prioritize cases

3 Placement The department The type of work to expect and learning Where the is based Clinical Supervisor(s) for the F2 - Acute Medicine The team consists of 2 dedicated acute medicine consultants as well as 5 consultant with part of their time in acute medicine, 1 F2 and currently 3 F1s. There are also 2 very supportive nurse practitioners. The acute medicine unit (Ilchester) has 25 beds and an assessment area. An increasing proportion of patients are managed in an ambulatory fashion to avoid admission. The post is almost entirely ward based during normal working hours. There is a morning handover meeting at 0830 every mornings when the take patients are triaged to the appropriate specialty and the night team handsover to day staff. Ward rounds occur in the morning and there is a strong consultant presence. Duties expected of the F2 include: - Clerking new patients when on call - Attending ward rounds -Supervising F1s - Prescribing safely -Supervision and training on ascitic drain and LPs, as well as to watch/perform pleural aspiration and central line insertions under US guidance - Attending teaching sessions for CPD - Communicating with relatives - Participating in the education of medical students Dorset County Hospital Ilchester Unit Dr Paul Andrews/Dr Adeel Ghaffar Main duties of the The F2 doctor is responsible completing jobs and participating in assessment and management of patients. 4 Placement The department F2 Adult Psychiatry Psychiatry: Waterston Acute Assessment Unit (AAU) and Home Treatment Team, Forston Clinic, Herrison Road, Dorchester Rotation: Dorchester Hospital Foundation Programme. The Waterston AAU is a 13 bedded mixed sex acute admission and treatment ward at Forston Clinic Dorchester with an average length of stay of between 2-4 weeks. The Home Treatment Team is a large multi-disciplinary team providing a crisis home treatment service 24 hours a day as an alternative to Inpatient care. This team acts as the gatekeeper for all In-patient admissions to inpatient units, as a single point of contact for all queries/ referrals from professional agencies as well as calls from users & carers( via an Out Of Office Hours nurse service) and the general public (via a Helpline). The service covers all adult residents in the West and North Dorset area including Dorchester, Bridport, Weymouth, Portland, Sherbone, Blanford Forum and Shaftesbury. The Team is based at Forston Clinic. The F2 Doctor will be based primarily on the ward and may have negotiated sessions of working with the Home Treatment Team. The post holder will provide medical backup along with the specialty doctors and the Consultants in the team. The type of work to expect and learning The overall educational objectives are to provide the trainee with the knowledge, skills and attitudes to deal with patients presenting with psychological or behavioral problems associated with severe mental disorders. They would be expected to: Assess new admissions by taking direct/collateral history, carry out mental state examinations and clerk in new admissions, Undertake physical examination of the inpatients and request appropriate investigations (inc. arranging scans, doing ECGs, liaising with GPs, managing straightforward physical conditions, arranging ECT etc.). Prescribe safely, particularly psychotropic drugs Communicate effectively with the patients, relatives and colleagues within the multidisciplinary team and the
5 Community Mental Health Teams. Keep an accurate and relevant medical record electronically Manage time and clinical priorities effectively Use evidence based treatment and/or guidelines when treating patients Understand legal and ethical issues in relation to the management of detained patients. The F2 Doctor, if interested, can perform home visits and carry out assessments in the community with staff from the Home Treatment Team. The Trainee will not be on the out of hour on call rota. The Trainee is expected to attend and contribute to the inhouse Education programme for Psychiatry Trainees. 12 Placement The department F2 General Practice The GP centre allocates 4 different GP surgeries that will take 1 FY2 for a 4 month post. The type of work to expect and learning F2s in general practice spend their day carrying out clinics, seeing patients and making decisions with the support of their GP tutor. All F2 doctors see patients by themselves and practice their focused history taking and examination skills to formulate a diagnosis and management plan. At most surgeries, F2s are expected to work a morning and evening surgery with a lunch break to allow for home visits and administration. 13 Main duties of the The F2 is responsible for running surgeries with minute appointment slots. As a GP F2 you can expect to encounter a huge range of patients with problems to assess. These can include medicine, surgery, psychiatry, ENT, O&G, ophthalmology, paediatrics etc. F2s are expected to recognize when they require assistance from their tutor and ask for assistance and teaching. It is important to recognize when a problem requires specialist input, referrals are made in the form of telephone conversations and letters to secondary care. This is done under the guidance of the clinical tutor. Most practices have a weekly meeting where topics such as vulnerable adults, palliative care and the general working of the practice are discussed; the F2 is encouraged to participate in this meeting. Placement The department The type of work to expect and learning Where the is based Clinical Supervisor(s) for the Main duties of the F2 ICU/Anaesthetics The anaesthetic department is a very friendly department comprising of 17 consultants who cover both ICU and theatres. As an F2 you are involved in a wide variety of specialties predominantly critical care, but also trauma, obstetric anaesthesia, paediatrics and acute pain. Most weeks are spent in ICU but there is opportunity to spend supernumerary time in theatres. There are no nightime on call commitments but 1 weekend in 4 is spent on call during the day for intensive care alongside an anaesthetic trainee. This is a fantastic opportunity to gain experience in ICU as well as anaesthetics. Learning are similar to that of novices on an anaesthetics training scheme, with weekly teaching and simulation suite sessions. Dorset County Hospital Dr Jee In ICU the F2 is expected to review patients and carry out ward jobs i.e. cannulation/radiography requests, referrals. Emphasis is placed on learning how to recognise and initially manage the critically sick patient. The F2 doctor is responsible with other staff for the ward care of patients and the maintenance of the patient s medical record. The following roles are discussed at induction: 1. Attend ICU ward round daily at 08:30 where care of ICU patients is directed by ICU Consultant. Ensure handover sheet up to date and all VTE assessments are completed (VitalPAC). 2. Work with ICU SHO to complete discharge summaries, both for ward patients and deceased / home patients (EDS). 3. Together with SHO & Outreach team, monitor outlying patients who
15 need daily ICU review ie recent discharges 4. Anaesthetic tutorials Wed pm each week except Thur pm once a month (usually last Thur of the month) 5. Audit/ Case Summary and 8 WPBA FY2s are paired with registrars/consultants for theatre sessions. During theatre sessions the FY2s are encouraged to actively participate in the anaesthetic care. The weekend duties (1 in 4) are Friday to Sunday 8am to 8pm Placement F2 - Obstetrics and Gynaecology The department We are a small, friendly unit of 9 consultants, 6 middle grades and 5 SHOs (one career, 3 GPVTS and one F2). We are based in our award-winning maternity suite, which combines antenatal, postnatal and labour care. Our gynaecology inpatients are on a surgical ward. Antenatal and gynaecology clinics are held in the Women s Health outpatients here. Although the consultants also visit a range of community hospitals for clinics, it is rare for SHOs to attend these. The type of work to expect and learning This post enables you to achieve competencies in vaginal examination, management of early pregnancy problems, recognition, screening and management of gynae cancer, to work as part of a multidisciplinary team and to work in theatre, clinic and emergency settings. FY2s are involved in the full range of

FY2 working with the team on the ward, independent ward rounds and large amount of responsibility on the ward. FY2 take part in the daily ward round and complete the medical notes for the patient. During the rest of the day they complete the jobs for each patient on the ward. Post take ward rounds are completed by each consultant in the week. During on-call weekends/weeks FY2 s clerk patients on Acute admissions unit and out of hours cover work on all medical wards across the hospital. They have the opportunity to clerk acute medical patients and take part in their ongoing care. Ward cover involves completed jobs required for patients such a practical procedures, prescribing medications and reviewing those who may be deteriorating or require medical care. Acute Stroke Unit, Dorset County Hospital Dr Proeschel, Dr Jenkinson. Ward rounds, reviewing and treating ill patients, requesting investigations, core procedures, discharge planning and liaising with relatives, working with physios and OTs. 19 Placement F2 - Trauma and Orthopaedics The department Trauma and Orthopaedics Dorset County Hospital, 8 consultants with 3 teams of juniors. The type of work to expect and learning At the start of the there is a three week period of lectures from until 9.00 am after the trauma meeting. These are designed to cover all the important topics in trauma and orthopaedics. At the trauma meeting all admissions and significant cases seen in the previous 24 hours are discussed. This is an excellent learning opportunity and provides practice on answering questions on clinical management in preparation for specialty training interviews. Care of all in patients under the supervision of the middle grade and consultant. There are 2 weekly orthogeriatric ward rounds that may be attended. Ward work and theatre experience. Skills training takes place on some Tuesday mornings, topics include: suturing, hand tying, safe use of local anaesthetics, plastering techniques, knee aspiration. There is also access to a laparoscopic simulator. There are monthly morbidity and mortality / review of practice meetings with lectures on important orthopaedic and trauma topics. Trainees are always welcome in the outpatient department. There are trauma/fracture clinics running every day and specialized clinics for Hand Surgery, Shoulder Surgery, and Knee Surgery etc. These all offer excellent training. There is monthly radiology teaching organized by Dr Alner/Mr Walsh, this can be increased to alternate weeks if sufficient demand. Assessment and initial management of musculoskeletal trauma including

Note: when on call you cover Orthopaedics and ENT or General surgery and urology. VTE assessment using vitalpacs system. Liaising with other members of the MDT nurses, physiotherapists, occupational therapists etc and general practitioners for continuity of care. Organizing and reviewing investigations for inpatients and patients seen on call. Presenting cases at the daily trauma meeting. Handover of patients at the end of your shift. Providing hand over sheets for the weekend team. Typical working pattern in this Mon: 8am trauma meeting followed by ward round and ward work.
