1 PROJECT: AUDIO VISUAL DESIGN/BUILD DATE: October 9, 2018 MERRIAM COMMUNITY CENTER REQUEST FOR QUALIFICATIONS: The City of Merriam, in cooperation with its Owner s Representative, CBC Real Estate Group, is looking to select a design/build partner to complete audio/visual design, procurement and installation for the Merriam Community Center on a qualification s basis. REQUIRED SCOPE OF SERVICES: In early 2018, a competitive selection process occurred and the Steering Committee for the project selected the McCarthy Design/Build team, including partners from; McCarthy Construction, The Clark Enersen Partners, Perkins+Will and SK Design Group. Soon after selection of the Design Build team, a second aquatic design selection occurred, and Aquatic Design Group was added to the team. The new Community Center is planned to be a multifunctional building appealing to all demographics of Merriam. As planned and currently programmed this facility will include; a new 15,000 sf gym with elevated 3 lane running track, classrooms, community meeting rooms, child watch/babysitting space, gallery and exhibit space, catering kitchen, administrative offices, fitness and group exercise space, locker rooms, and storage. In addition, the building will house both indoor and outdoor aquatic space. Indoor aquatic elements will include; a lazy river, therapy pool, family slide, lap lanes and other open water and pool deck areas. Outdoor aquatic elements will include an interactive spray ground and a 6-lane pool with diving area. The audio-visual design/builder will be hired by the City of Merriam and will report directly to the City of Merriam. The work will begin immediately upon acceptance of a qualified proposal. The building design is presently in construction documents and the City is looking to add the AV member to the team to complete design and coordination of this scope of work. Floor plans have been included in this proposal for reference. Scope of work will include; 1) Full integrated audio-visual design 2) Procurement of all related audio-visual equipment 3) Installation of all equipment procured and needed to operate the system 4) Configuration/integration of system 5) Extended service contract to Owner Responding firms should include the following in their proposal; 1) Firm description and team Please provide a brief description of your firm and team members that will be assigned to this project specifically for the design process. If installation will be by a sub-contractor, please identify this firm and explain relationship between design and installation firms. Please also include firm history of any subcontractors. Provide resumes for key staff and any subconsultants.

2 2) List of current and past assignments for which Design/Build AV services were provided including description of projects, if the entity for which the services were provided is public or private, estimated value, and proposed team member(s) assigned to each project. Community center/fitness Center experience should be noted. 3) Clarification of ability to provide full scope of services as listed in this proposal. 4) Please explain percentage of work of your firm that is dedicated to audio-visual design and installation. COMPENSATION: Although selection will be made on a qualifications basis and ability to complete services as indicated, please also provide the following; Design Services: $ Reimbursable Expenses: $ TERMS AND CONDITIONS: 1. This RFP does not commit The City of Merriam to award a contract, or to pay any costs incurred in the preparation of a qualification in response to the RFP. 2. Proposer(s), their authorized representatives, and their agents are responsible for obtaining, and will be deemed to have, full knowledge of the conditions, requirements, and specifications of this RFP. 3. The City of Merriam reserves the right to cancel this RFQ or to reject any or all qualifications received prior to contract award. 4. The City of Merriam reserves the right to request clarification of any qualification after all qualifications have been received. The request can be in the form of oral presentation or personal meetings. 5. The City of Merriam reserves the right to open qualifications privately or unannounced and to reject any and all submittals and waive irregularities and informalities in any qualifications that are submitted and to be the sole and final judge of all qualifications. 6. The City of Merriam reserves the right to discontinue its evaluation of submittals from any respondents who submit false, misleading or incorrect information.

Late submissions cannot be considered and will either be returned unopened or refused. City of Merriam shall be the sole determinant of the submission arrival time. City of Merriam and CBC Real Estate Group reserve the right to waive to any minor irregularities or informalities and to reject any and all proposals. CONTRACT: Upon acceptance of a qualified and acceptable proposal, The City of Merriam will provide a signed agreement for your records.
