1 2016 FASD prevention grants request for proposal The Minnesota Organization on Fetal Alcohol Syndrome (MOFAS) is committed to preventing prenatal alcohol exposure and Fetal Alcohol Spectrum Disorders (FASD) by creating awareness of the danger of drinking alcohol while pregnant and increasing the number of women who plan their pregnancies and engage in healthy behaviors before becoming pregnant. This includes educating that there is no safe time to drink when pregnant or planning on becoming pregnant, and understanding how health behaviors like drinking alcohol while pregnant can impact a women s health and the future of her baby. Grants from 500-2,500 are now available to organizations and agencies in Minnesota that: Educate women on the importance of preconception health and planned pregnancies to decrease the risk of unintentional prenatal alcohol exposure Increase awareness of the danger of drinking alcohol while pregnant with women who are pregnant or could become pregnant Encourage alcohol-free pregnancies by sharing the 049 zero alcohol for nine months - prevention message with women who are pregnant or could become pregnant These grants will fund community-based organizations who are interested in hosting public events in order to share these messages with women and families in their community. 2 2016 FASD prevention grants grant eligibility, guidelines, and deadlines about Fetal Alcohol Spectrum Disorders (FASD) can only happen if a woman drinks alcohol during pregnancy. The effects of drinking alcohol while pregnant lasts a lifetime. For some groups, FASD affects between 2-4% or per 1,000 children, making it the number one known cause of developmental disability. Here in Minnesota, as many as 5,367 babies are born each year with possible permanent brain damage from prenatal alcohol exposure. Help prevent FASD. Share the 049 zero alcohol for nine months - prevention message. purpose The Minnesota Organization on Fetal Alcohol Syndrome (MOFAS) is seeking proposals from qualified applicants to host community events to increase awareness about the danger of drinking alcohol while pregnant, educate women of childbearing age on the importance of preconception health and planned pregnancies, and share the 049 zero alcohol for nine months - prevention message with women who are pregnant or could become pregnant. Particular models we have found to be successful in the past include, but are not limited to: model a: community baby shower Brings pregnant women together to share information on the dangers of drinking and promote 049, while also celebrating each mom-to-be. Along with MOFAS educational message, these events often include baby shower games, cake, and perhaps a raffle. Approximate budget range: 750-1,500 model b: 04.9k walk/run Brings community members together, including women who are or could become pregnant, through a 04.9k walk/run. Using lawn signs along the way and other materials, the MOFAS preconception health and 049 messages are shared with each participant. Approximate budget range: 1,250-2,500 model c: special speaker Brings community members together, including women who are or could become pregnant, to hear an educational talk from a special speaker. This could be an ambassador from your community or an FASD professional from MOFAS network. Approximate budget range: 500 1,000 Please consider your community size and organizational capacity when planning your event. Events must include a minimum ½ hour educational presentation on the above topic areas. If the event includes a table/booth sponsorship, the grantee must provide staffing. Otherwise, MOFAS will attempt to provide a speaker.

3 eligibility Funding preference will be given to community-based agencies that serve individuals in MN, including: 501(c)3 non-profits Schools and educational institutions Governmental agencies Tribal governments In general, grant funding will not be awarded for: Individuals Revenue generating events Public service announcements Advertising/marketing campaigns Conferences Curriculum development review process All grant applications will be evaluated by a review committee comprised of professionals from the maternal child health community, MOFAS board members and staff. MOFAS reserves the right to conduct follow-up interviews with applicants to discuss the grant proposals and any areas that require clarification. use and disbursement of funds All grant funds are distributed on a reimbursement basis only. Spending is also capped on certain categories, including Food and Refreshments and Prizes, which each may not individually account for greater than 15% of the project budget. 7 3. Outline the Promotion of Your Event Describe specifically how your event will be promoted in your community to encourage attendance/participation. Be sure to include these items in your budget if costs will be incurred. Posters, Flyers: How many will you be posting, and where? Radio, TV, Community Calendars: Which stations will you be promoting on? When and how many times do you plan to have the promotions aired? What community groups will you be promoting to as potential attendees? , Listservs, Website: Who will you send s to? Which sites will you post on? Social Media: What social media platforms will you use to promote the event? What is the estimated reach of each of these platforms (followers/impressions)? Other: 4. Describe what the goals are for the planning committee and how often you will meet. 5. Explain Your Focus on FASD Please describe why your organization is interested in hosting an FASD prevention event in your community and how FASD has impacted your community. 6. Complete Your Budget Worksheet Please fill out the attached Budget Worksheet for all anticipated expenses related to the event and describe what the funds will be allocated for. 8 Budget Worksheet Please detail all expenses for the proposed event/activity. Describe and explain what the estimated costs pay for. The proposed budget must be complete and reasonable, must link directly to the proposed event/activity, and must specify how the amounts for each budget item were determined. Grantees are encouraged to apply for only the amount needed for their proposed event. Budget proposals will be evaluated on efficient use of funds. Grantees are expected to track and maintain documentation for all proposed expenses throughout the grant time frame. Grant funds cannot be spent on equipment, this includes items as prizes. Equipment includes: ipods, ipads, Tablets, laptops, projectors, cameras, etc. If you would like to purchase an item and are unsure if it Is eligible, contact prior to purchase.
