1 2018 STAR Student Application Instructions The Ochsner STAR Program provides qualified high school students, particularly those with limited means and access, a cost-free opportunity to explore the world of healthcare. Through a rigorous, hands-on summer course focusing on science, technology, academics, and research, STAR aims to engage and inspire students to pursue further education and careers in science and medicine. TITLE PAGE

2 Page 1 of 4 Science Technology Academics Research 2018 Application Instructions All components of the application must be received before Tuesday, March 13 th, 2018, 11:59 PM. Before you begin, please review the following application directions and program information. Program Information and Requirements Louisiana residents will be given priority selection for the STAR Program. Applicants must have at least one reliable form of transportation already in place. Accepted students and at least 1 parent/guardian are required to be present during Acceptance Day and Meet & Greet. Students are required to attend the Culmination Ceremony, and parent attendance is encouraged. Accepted students are allowed only two days of absence throughout the program. Accepted students will be required to submit an up-to-date immunization record, complete a tuberculosis questionnaire, and pass a drug screening. Applicants must be at least 16 years old by June 4 th, STAR is open only to students who are advancing into the 12 th grade. Important Dates (for more information on these events, please call Academic Outreach at ) Tuesday, March 13 th All application materials due by 11:59 PM April 2018 Interviews will be held for students who have been accepted into Round 2 Monday, April 30 th Tuesday, May 1 st Wednesday, May 2 nd Tuesday, May 15 th Monday, June 4 th Friday, June 29 th Friday, June 29 th Acceptance Day WebEx (accepted student & a parent/guardian are required to join at least 1 of the 3 offered video sessions) 2018 STAR Meet & Greet for accepted students and parents (accepted student & parent attendance is required) 2018 STAR Program; Monday through Friday, 8:00am 4:00pm 2018 STAR Culmination Luncheon (student attendance required & parent attendance encouraged)

3 Page 2 of 4 Application Submission Instructions The three components of the STAR Application include: (1) Online Student Form NOTE: You can stop, SAVE, and return to the Online Student Form at any time. This does not need to be completed in one sitting. 1.1 General Student Information i. Please be prepared to provide the following: general demographic information, current school details, parent/guardian information, future college and career plans 1.2 Short Answer i. Application will be asked to respond to the following prompts: 1. In 150 words or less, tell the STAR Acceptance Committee something that makes you unique. Below is your opportunity to share beyond what is already highlighted in your STAR Application. For example, your answer can highlight a special hobby, interest, talent, or passion. 2. Why would you like to participate in the STAR Program, and how will your participation in STAR move you towards your future goals? (150 word maximum) 1.3 Essay ( words) Prompt: You were awarded a $10,000 grant for the creation of a community program in your area. Describe the goal of your program, what community need it would serve, how you would use the funds, and how you would implement the program. Be creative! 1.4 Resume (to be attached) i. Highlight school, community, and/or church activities in which you have or are currently an active participant ii. Include any leadership positions you have or currently hold within these organizations. iii. Include awards and/or special recognitions. To access the Online Student Form, follow the below web address: (2) Official School Transcript Submission School transcripts are only official when sent by a school counselor or official from a school address. Transcripts should be sent to STAR@ochsner.org with the subject line of Student Last Name, Student First Name. (3) Letters of Recommendation Applicants must have two recommenders. It is the student s responsibility to ensure the Online Recommendation Forms are submitted by the recommenders by Tuesday, March 13th, Students must provide the following information from his/her Online Student Form to the Recommender: Student First Name: Student Last Name: Student Record ID Code (this code is created when a student starts his/her online application): Each recommender must fill out the online recommendation form and write a recommendation letter. Recommenders should be able to speak about the applicant s character, leadership abilities, and/or accomplishments. Additional Recommender Instructions and a link to the Online Recommender Form are available at * Applications with unanswered questions or missing components will be considered incomplete. * Incomplete applications will not be reviewed.

4 Page 3 of 4 Frequently Asked Questions How will I know that my STAR Application is complete? Upon completion of the Online Student Form, you will receive an from the STAR Administration confirming the submission. THIS DOES NOT MEAN YOUR APPLICATION IS COMPLETE. Once all application components, including transcripts and letter of recommendation, have been received, you will receive a second and final confirmation from the STAR Administration. *You are strongly encouraged to turn in your application before the due date. What should I do if I am having trouble re-accessing my Online Student Form? Do not start a new Application! Please STAR@ochsner.org for help. How can I find out more information about STAR and speak to the administration and alumni? Visit the STAR webpage ( for more information on the STAR application process, curriculum, and requirements. When and how will I be contacted if I have made it to the interview round? The top students, based on application contents, will be accepted into the 2nd round of the application process, which is a formal interview with the STAR Administration. Students who have been accepted into the interview round will be contacted by phone to schedule the interview date and time. Interviews will take place in the late afternoon/early evening on a weekday. Students will be notified by mail if they have not been accepted into this round. When and how will I be notified if I have been accepted into the STAR program? Students who have been accepted into the program will be notified by phone and will be required to attend one of the three offered Acceptance Day Appointments (April 30 th, May 1 st, May 2 nd ) with a parent or guardian. Two students will be accepted as alternates. Alternates will be accepted into the program in the event that one of the accepted students cannot fulfill the requirements. Alternates will be notified in early May as to whether or not they have been selected to participate. Whom do I contact if I have questions or need more information?
