1 INSTRUCTIONS BAC Grant applications are submitted through forms available at brooklynartscouncil.org. Please note that the forms are hosted by Submittable. Applicants are required to create a Submittable account if they do not have one already. This online software allows applicants to save and edit drafts before submitting, and maintain a record of all form submissions to BAC. Application questions are below for reference. All fields are required unless otherwise indicated. Answers that exceed the word count will be cut off. Applications must be submitted no later than 11:59pm on the day of the deadline. Forms will not be re-opened on or after deadline day. Technical issues with the online application form or questions about logging onto Submittable must be directed to support@submittable.com or Other questions can be directed to the Grants Department at grants@brooklynartscouncil.org. PROOF OF ADDRESS UPLOAD Upload the applicant s proof of residency here. This can be a copy of New York City or State-issued driver s license or ID card; utility bill; lease or sublease or tax document with a Brooklyn address. PRIMARY CONTACT (First and Last Name/Title/ /Phone Number/) Name and contact information of the person who is primarily responsible for this application. For individuals, this is often the same as the information supplied above. Organizations use this space to tell us the name of the person we should be in touch with. This person must understand that they will be responsible for this application and will receive communications from us from September - March. Contact info must be currently active. 3 ORGANIZATION BUDGET UPLOADS Organizations on a calendar fiscal year should upload both their project FY16 budget and their most recent completed financial statement. Organizations on a non-calendar fiscal year should upload their current FY16 budget and the most recent completed financial statement. Note: Financial statements can be an audited financial statement, a treasurer's report, or an IRS Form 990. PROJECT OVERVIEW PROJECT TITLE The title of your proposed project. You will be able to change this later if your project is funded. SHORT PROJECT DESCRIPTION (Max 25 words) A short blurb or tagline that describes your project. Do not repeat your project title here. This will be read to present your application to panel. (i.e. collaborative dance project with professional dancers and local teens or pop-up travelling art exhibit ). PROJECT DISCIPLINE Select one of the options from the drop-down menu that best describes your project. This will be the discipline under which the project will be considered for funding. Review the Panel Disciplines definitions if you are unsure about where your project will fit best. TYPE OF PROJECT Select the best option from the drop-down menu. NEIGHBORHOOD(S) SERVED Up to 3 only Base the neighborhood served on the neighborhood in which your project is physically taking place and/or on where you anticipate drawing an audience from. Do not mark more than 3. This should directly correlate to the description of your Project Location(s). AGE GROUPS SERVED Enter as whole number, not percentage. Estimate the total number of people in each of the following age groups that will be involved in and served by your project. Include artists and administrators working on the project, technical staff, audience, community members, etc. If you did not serve an age group, enter 0. Children (0-18): Adults (18-64): Seniors (65+): TOTAL NUMBER SERVED Enter the total number of audience members anticipated. (This should be the sum of your answers to the Age Demographic questions above.) ETHNIC GROUPS SERVED Enter as percentage (Total must=100%) of the total number of people your project will serve, estimate the percentages that fall into the following ethnic groups: Asian; Black/African-American; Hispanic/Latino; White, non-hispanic; Other; Mixed/No Single Group. Percentages must equal 100%. NUMBER OF ARTISTS SERVED The number of artists who will be benefiting in some way from your project or working on the creation of your project (i.e. number of dancers performing, number of teaching artists being paid, etc.). 4 NARRATIVE QUESTIONS MISSION/ARTIST STATEMENT (Max 100 words) Incorporated organizations, please enter your mission statement. Informal organizations or collectives can include a working mission statement if applicable. Individuals should include their artist statement. PROJECT DESCRIPTION (Max 500 words) Describe the project for which funds are being requested, and what the BAC funds will be used for. This should be a succinct overview of your project. You will expand on this summary in the following questions of the application. Include the basics of the proposal: who will execute, be involved in and benefit from the project; what the project is, what the goals and intended outcomes are, what you are asking for; and where & when the project is projected to occur, and why. PROJECT CONTEXT (Max 350 words) How does this project fit into your overall creative and/or curatorial practice, or into your organizational history/goals? Discuss any previous experience you or your organization may have had with similar projects. What previous experiences will you draw on to be able to successfully administer all phases of your project? Is your proposed project an expansion of an existing project or body of work, or a new direction? How does this project relate to other pertinent projects you re currently working on or planning? How does this project support your own goals as an artist or your organizational goals within the community? OUTCOMES (Max 100 words) What outcomes do you anticipate from this project, long and short term? Applicant s assessment can be qualitative or quantitative or both. This section should address the proposed project's goals as they relate to BAC's review criteria and funding priorities. PROJECT LOCATION(S) (Max 300 words) Be as specific as possible about where your project will take place. Name a specific venue that you have already reached out to or confirmed. Discuss multiple locations if applicable. If a venue has been confirmed, upload your venue letter of commitment below. If no venue has been confirmed, discuss your ideal location and backup ideas. If you plan on using non-traditional space (parks, streets, etc), address your ability to do so. If you have received letters of commitment, include them with your supplemental materials. Address whether or not your proposed venue(s) are ADA compliant, and any plans for making your project accessible to individuals with disabilities, if applicable. VENUE LETTER OF COMMITMENT UPLOAD If applicable, please provide a a venue letter of commitment. This could be a printed of interest or confirmation Acceptable file types: PDF, JPEG, doc, docx. PROJECT TIMELINE (Max 300 words) Briefly describe your proposed timeline for this project, being as specific as possible. Discuss the project s history, if applicable; whether or not the project is already in progress. If not, address your ability to complete it. Projects may be recurring, but BAC funds can only be used toward expenses incurred in the 2017 calendar year. Please note: should you be awarded funding, awards are not released until late March PROJECT PERSONNEL/COLLABORATORS/PARTNERS (Max 300 words) Describe the personnel involved and their function in relation to the project. Talk about the company, ensemble, staff, artists, teaching artists, technical personnel, volunteers, consultants, community contacts, etc, and their relation to the project. If there is a call for artists, auditions or other curatorial process, talk about the outreach method: how the artists or work will be selected, whether or not there is a fee to participate, and what communities you will be reaching out to. Include résumés below for the project director and one (1) other member of key personnel identified in this section. Résumés may Page 4 of 6 Project narrative tips: The panel won t fund what they don t understand! Use clear, concise language and try to leave them with as few questions as possible. Make no assumptions of prior knowledge. The narrative and the budget are different tools to tell the same story. Include all the monetary details in your narrative that are in your budget and vice versa. Be positive you re excited about your work. Make sure to show that to the readers. It s a proposal, not a contract be as realistic as possible based on your previous experience and what you would do with this support. When in doubt, go back to the grant guidelines to reference our program goals and proposal expectations.

PROJECT AUDIENCE (Max 350 words) Describe the target audience for your project, and the communities your project will impact. Describe your relation to the audience or community you plan on reaching, and explain how you envision your project enhancing, involving, serving, and/or impacting them. OUTREACH PLAN (Max 200 words) How will your target audience be reached out to/notified about the project? How will your project be publicized? Have you had successful marketing and publicity plans in the past? Be specific as to how and where you will publicize/advertise your project, and what impact you expect this will achieve. What media format(s) will you use (i.e. print, radio, TV, postcards, mailings, flyers, posters, , BAC s online calendar, etc.) PROJECT BUDGET The downloadable excel budget form is available online at brooklynartscouncil.org. We will ONLY ACCEPT this budget form. No other formats will be accepted. BUDGET NARRATIVE (Max 200 words) This section is an opportunity to expand on the project budget s line items - to further explain other projected sources of income, in-kind donations, and how BAC funds would be spent. Please note that BAC cannot be the sole funder for any project, and applicants must show other sources of income for at least 10% of the proposed budget. CONTINGENCY PLAN (Max 150 words) BAC Grants are often significantly less than the original request and only about 5% of grantees receive full funding. Briefly describe how receiving less funding than requested will impact your project. Describe your plans to decrease expenses or seek additional funding if you do not receive the full amount you request. If your project cannot occur without full funding, please state so here. REQUESTED FUNDING AMOUNT Enter the total amount of funding you are requesting. Request amount must be between $2,000 and $5,000, and match the amount on the Project Budget form. BUDGET UPLOAD Upload your completed Project Budget here. Please note that a completed project budget includes line item descriptions, a request amount that meets the eligibility criteria, and is completed on the excel form provided by BAC. SUPPORTING DOCUMENTS All applicants are strongly encouraged to upload all that are application to their application. PRESS KIT Upload a press kit or set of publicity materials from past projects, (i.e. most recent press release; brochures/postcards/identity pieces; clear photo documentation of previous work; quotes pulled from past reviews). ADDITIONAL LETTERS OF SUPPORT If applicable, applicants may upload additional letters of support from city council members, partner organizations, community members, etc. Page 5 of 6

6 WORK SAMPLE WORK SAMPLE NARRATIVE (Max 200 words) A narrative description of what you are submitting as your work sample. This will be read to present the application to panel. Applicants are permitted to submit their work sample in the form of up to 10 uploaded files or two online links. Files can be images, audio, or video. Time-based submissions (video or audio) may not exceed a total of 5 minutes in length. Panelists will not review more than 5 minutes of the work sample. The following information should be included in the work sample narrative: Title of work(s); when it dates from; artists involved; whether it is from a completed project or work in progress. For time-based submissions, include where it was recorded, and provide a cue point for any work sample that is longer than 5 minutes. Literary Arts applicants should include title of manuscript, synopsis/what is included, when it was written, and publishing history if applicable. WORK SAMPLE(S) UPLOAD Upload up to 10 files here. Acceptable file types: pdf, doc, docx, jpg, gif, mp3, mp4, m4a, zip, png, wav, mov, ppt, pptx, avi, wmv, wma.
