1 1 F a t h e r h o o d I n i t i a t i v e BARREN RIVER COMMUNITY COLLABORATIONS FOR CHILDREN REGIONAL NETWORK FATHERHOOD INITIATIVE MINI-GRANT GUIDELINES AND GENERAL INFORMATION The Barren River Community Collaborations for Children Regional Network is pleased to be able to award up to $500 to a limited number of projects that target Fatherhood initiatives. These initiatives can be for any male caregiver such as father, grandfather, uncle, foster father or significant male caregiver of a child. Each organization/group will be eligible to submit one application only. The goal of the training and the Mini-grant is to support programs/activities/events that will: Focus on the father s involvement in child well being with respect to safety and security. Prevent child abuse and neglect as a result of increased father/male caregiver interest and participation in children s lives. Support self-sufficiency that ensures physical, emotional and mentally healthy development of families. Strategies focusing on one or more of the following identified issues will be funded. Empowering fathers to become leaders in their communities. Allowing opportunity for real male input in program/project directives. Fathers demonstrate the innovative use of resources in community. Funds are not strictly for fathers but to advocate for the best interest of the child which research shows involves the entire family. In the child s eyes their mom and dad are crucial to their wellbeing. Therefore, family orientated activities that focus on the importance of the father s/male s role are a good use of these funds. The Barren River CCC Regional Network will accept applications within the following guidelines: Applicants must be located within one of the following counties: Allen, Barren, Butler, Edmonson, Hart, Logan, Metcalfe, Monroe, Simpson, and Warren. Only one application will be accepted from each organization. NO faxed or ed copies will be accepted. All funds must be used and the project completed before May 1, No more than 20% of the proposed grant can be used for food or incentives. All proposals must include a minimum 25% in kind match [more in kind is encouraged] Grant Funds CANNOT be used for: Personnel Salaries Fringe benefits Capital equipment Building Improvements Child Care Gift Cards Additional requirements for funded proposals are as follows: A representative from the funded program must attend the quarterly meetings of the CCC for the remainder of the 2010 fiscal year (tentative meeting dates have been set for November 12, 2009, January 14, 2010, March 25, 2010, and May 13, 2010). Receive one site visit from CCC network member if the Networks so chooses. Submit a program narrative and the Primary Prevention Activities report at project end detailing the accomplishments of the project as well as the successes and barriers experienced by May 1, The proposal must include a formal evaluation to support the outcomes of the initiative.

2 2 F a t h e r h o o d I n i t i a t i v e For the purpose of this grant Father is defined as a male who is actively involved in the child s life. The vision is to see men become more effective, more active, and more involved in their role as fathers with their children. For the purpose of this grant In Kind match is any materials, donations, additional funds, and/or meeting space donation (facility) that supports the project but is not charged to the grant funds. Applications must be received by November 4, 2009 NO LATER THAN 2:30 P.M. Applications received after the deadline will be eliminated and not considered for funding. Only one application per group/organization will be accepted. Final decisions will be announced by November 12, APPLICATIONS MUST BE RECEIVED BY 2:30 PM, NOVEMBER 4, NO FAXED OR ED COPIES WILL BE ACCEPTED. CHECKLIST: Make sure the following is included in your application: One original and three (3) copies of the following must be included. (Faxed or ed copies will not be accepted. Completed Cover Page Attachments budget information page, including in kind match (minimum of 25%)

Use the checklist below to ensure that your application packet is complete. Be as concise as possible. Outline for Proposal: Proposals must earn a minimum of 70 points to be considered for funding. Maximum amount of points available are 100. Introduction: [Maximum Points 5] Who are you/your group? Briefly describe the mission/purpose of your group. Problems/What is the need? [Maximum Points 10] Describe the specific community need that you plan to address. How many people do you plan to serve? Objectives [Maximum Points 10] What do you want to accomplish? What will change for the population served as a result of this grant? How does the initiative build awareness of the father s role? Methods [Maximum Points 30] What will you do? What activities, events, or projects will be solved? What are the details of the activities, events, or projects,? (Date, time, location, etc) What other agencies/organizations do you plan to collaborate with? Evaluation [Maximum Points 25] How will you measure and report on the effectiveness of your event? What tools will you use (surveys, interviews, focus groups, etc.)? Budget [Maximum Points 20] (Funds cannot be used for staff salaries, benefits, etc. See guidelines for further information.) How will you spend the grant monies? Include a summary of materials and list other expenses including description of items, quantity and dollar amount. Use attached form.
