PROJECT PURPOSE AND DESCRIPTION (All applicants must complete this section) Describe the project and explain why the project is needed. Tampa Bay CDC is submitting this proposal to PROCESS SOLD-ON-LARGO DOWNPAYMENT ASSISTANCE LOANS on behalf of the City of Largo. In submitting this proposal, we seek to assist eligible Homebuyers (whose household incomes are below 80% AMI or those who have special needs) to purchase affordable, safe and sanitary homes throughout the City of Largo. We commit to strict adherence of the guidelines of the City s various funding sources (i.e. HOME, SHIP, CDBG, HTF), and we also commit to conform to all City Housing Program guidelines. The SCOPE OF WORK being proposed under this application includes the following: A) MARKETING AND OUTREACH TO POTENTIAL HOMEBUYERS, LENDERS AND REALTORS B) INTAKE AND ASSESSMENT OF POTENTIAL CLIENTS C) LOAN PROCESSING AND INCOME DETERMINATION D) PROPERTY EVALUATION E) UNDERWRITING REVIEW/SUBMISSION OF FILE TO CITY OF LARGO FOR PRE- APPROVAL PRIOR TO CLOSING F) REFERRAL OF HOMEBUYERS TO PRE-PURCHASE HOMEBUYING AND FINANCIAL FITNESS WORKSHOPS, AND ENCOURAGEMENT OF CLIENTS TO SEEK PRE-PURCHASE COUNSELING,TO ENABLE THEM TO BECOME RESPONSIBLE HOMEOWNERS. 3 G) LOAN CLOSING AND ACCURATE PREPARATION OF SECURITY INSTRUMENTS H) POST-CLOSING REVIEW AND PREPARATION OF FILES AND REIMBURSEMENT REQUESTS FOR SUBMISSION TO THE CITY OF LARGO ATTACHMENT A includes an extensive summary of Tampa Bay CDC s experience with Processing and Closing Downpayment Assistance Loans, and it outlines what services will be offered as part of this Contract, the experience the Agency has in offering these types of services, and the accomplishments Tampa Bay CDC has made within the City limits of Largo and in the entirety of our service areas. Tampa Bay CDC s DOWNPAYMENT ASSISTANCE PROGRAMS STRIVE TO MEET THE FOLLOWING GOALS AND ADDRESS THE FOLLOWING NEEDS: 1. INCREASE AFFORDABLE HOMEOWNERSHIP OPPORTUNITIES AND PROVIDE ACCESS TO AFFORDABLE LOAN PRODUCTS. All of Tampa Bay CDC s Assistance Programs (including the City s Sold on Largo Program) are marketed through our website ( Facebook Page (Tampa Bay Community Development Corporation), monthly Pinellas Homebuyer Workshops (held monthly in English and Spanish and generally attended by approximately people), and at Pre-Purchase Counseling appointments. Tampa Bay CDC provides information to potential Homebuyers on low-rate First Time Homebuyer Mortgage Programs/Lenders, and information is also available on all area Assistance Programs (including Sold on Largo Assistance and Pinellas County/State of Florida Bond Programs). Clients can obtain brochures, flyers, contact information and even applications for any of these Assistance Programs, and they can learn how to combine various Programs to maximize affordability. Per Zillow, the average median home price in Pinellas County (as of 12/31/2017) was $205,900 (an increase of 10.2% over the same time last year), and it shows steady increases since 2012, with a projected increase of 3.5% in With values and prices over $200,000, most low-income people (whose incomes are below 80% AMI) cannot achieve homeownership without the use of Downpayment Assistance or low-interest mortgages. By making information available on these special Programs and assisting Participants with applying for them, this increases their opportunity to achieve homeownership. Tampa Bay CDC staff is on hand at all Workshops to assist Participants with applying for Assistance Programs, and Participants are given the opportunity to make an appointment for a one-on-one session with a Tampa Bay CDC HUD-Certified Housing Advisor in order to receive individualized help with identifying their eligibility for various Assistance or low-interest mortgage programs 2. PRESERVE AFFORDABLE HOUSING UNITS OWNER-OCCUPIED BY LOW-INCOME CLIENTS OR SPECIAL NEEDS CLIENTS by providing information on rehabilitation loans and grants offered by the City of Largo. 3. Provide the Home Buyer with an UNDERSTANDING OF THE RESPONSIBILITIES OF HOMEOWNERSHIP and MAINTAINING AND INCREASING THE VALUE OF THE PROEPRTY. Page 3 of 20

Describe the use that will be made of requested CDBG funds from the City of Largo (ex. renovate five transitional housing units, Add a 150 square foot room; acquire a building; reconstruct 600 linear feet of road; provide the following services). PROVIDE THE FOLLOWING SERVICES: Provide Processing and Closing Services to the City of Largo for Sold on Largo Assistance Loans in order to create more safe, sanitary and affordable Homeownership Opportunities for low-income and special needs residents of the City of Largo. 4. Describe how the project meets one or more of the City of Largo CDBG Consolidated Plan Objectives (listed on pages 1 and 2 of the application instructions) and the priorities listed on Page 2 of this application (if applicable). In delivering this Downpayment Assistance Processing program, Tampa Bay CDC believes it is meeting the following CDBG Consolidated Plan Objectives adopted by the City of Largo: AFFORDABLE HOUSING OBJECTIVES: 1. PROVIDE HOUSING ASSISTANCE FOR FORECLOSURE PREVENTION, RELOCATION, FAIR HOUSING, AND FIRST TIME HOMEOWNERSHIP FAIR HOUSING: All Clients who seek to receive Sold on Largo Assistance must attend Tampa Bay CDC s 8-hour HUD-approved Homebuyer Workshop. The first topic discussed at this Workshop relates to Fair Housing. Participants are advised of their Fair Housing rights as it relates to renting and home buying, and they receive a listing of local and federal agencies who can assist them should discrimination occur. Tampa Bay CDC has partnered with the Pinellas County Office of Human Rights and we utilize this Agency as a resource to assist our Clients with resolution of housing discrimination issues they are facing. FIRST-TIME HOMEOWNERSHIP: In reviewing the services that are offered to the City of Largo, a review of the data verifies that a majority of services are being provided to low-income first-time homebuyers (who had not owned homes in the previous three years). Tampa Bay CDC processes Downpayment Assistance loans on behalf of the City of Largo (275 closed through ), and 100% of those Clients were first-time buyers. Over 95.5% of those Clients who received City Assistance had incomes below 80% AMI). Prior to closing an Assistance loan, the Client must attend a Homebuying Workshop from a Cityapproved Instructor. Tampa Bay CDC is one of the approved Instructors, and a majority of Largo residents who attended one of our Homebuying Workshops were low income (earning less than 80% of the area s median family income). In eleven of the past fourteen years, more than 70% of the Largo residents who attended the Workshops had incomes below 80% AMI. Page 4 of 20

5 PROVIDE DOWNPAYMENT ASSISTANCE TO FIRST-TIME HOMEBUYUERS: In 1992, in partnership with its government funding sources, Tampa Bay CDC structured a downpayment/closing cost assistance program. This Program provides access to interest-free funds to income-eligible individuals and families who desire to purchase a home and who meet the qualifying criteria for a mortgage, but who lack sufficient funds for the required downpayment or closing costs. Since 1994, Tampa Bay CDC has administered this Assistance Program for the following government jurisdictions: Pasco County ( ), Hillsborough County ( ), and the City of Tampa ( and ). Currently, Tampa Bay CDC administers the Assistance Programs for the local jurisdictions of Pinellas County, and the cities of Clearwater, Largo and St. Petersburg. Tampa Bay CDC has a 3-year Contract in effect with Pinellas County to provide processing services for Downpayment Assistance loans through This Contract is currently awaiting an amendment to be done, and until that is finalized by the County, they will be processing DPA loans in-house. It is anticipated that we will resume contracted processing services in May or June With regard to the Agency s overall production, as of 12/31/2017, Tampa Bay CDC has provided assistance to 4,037 low-to-moderate income individuals and families helping them achieve their dream of homeownership by purchasing homes in the tri-county area. These closings represent $289 million of Real Estate sold, $262.7 million in first mortgages and $28.1 million in Downpayment Assistance. (Please see ATTACHMENT B for further demographic/success data). With regards to the City of Largo, Tampa Bay CDC has closed 275 Downpayment Assistance Loans through 12/31/2017. Page 5 of 20

6 ACQUIRE, REHAB/CONSTRUCT AND SELL HOMESTEAD HOUSES TO LOW-INCOME FIRST- TIME HOMEBUYERS: When the City of Largo has properties for sale under their Homestead Program, Tampa Bay CDC makes promotional flyers available at the Workshops and displays them in our office. Tampa Bay CDC provides technical assistance to Largo Area Housing Development Corporation (LAHDC) to carry out the activities associated with the Neighborhood Stabilization Program (NSP) through the City of Largo. Through this Program, the Agency has acquired properties within the City of Largo and managed the renovation of those properties to insure they were preserved and met City code requirements as well as Section 8 Housing Standards. Upon completion, these properties were sold to low-income Clients whose incomes were less than 80% of the area s median income. In addition to acquisition/rehabilitation services, through this Program, Tampa Bay CDC also assisted Largo Area Housing Development Corporation with the construction of new homes in the Eleanor Oaks subdivision. For the period of October 2010 through December 2017, 31 properties were renovated or constructed within the City of Largo. Page 6 of 20

7 2. REVITALIZE OLDER HOUSING AND REPLACE UNSAFE STRUCTURES All Clients who receive Sold on Largo Downpayment Assistance are required to have either a Section 8 Housing Quality Standards (HQS) Inspection (for existing properties), or provide a copy of a Certificate of Occupancy (for newly-built properties). The HQS Inspection is ordered by Tampa Bay CDC, from a Cityapproved Inspector. The Inspector must verify that the property meets all Section 8 Housing Quality Standards as well as the City of Largo s code requirements. If the Inspector notes any violations, these must be repaired by the Seller prior to closing. Tampa Bay CDC will verify that the work has been done by licensed individuals, and where necessary, insure that applicable permits were obtained. Prior to closing, the Inspector must go back to the property to reinspect and verify all repairs required in the original Inspection Report have been satisfactorily completed, and they must provide Tampa Bay CDC with a final report stating so. Because we are dealing primarily with low-income Clients (whose incomes are below 80% AMI), the inspection is critical to insure they will move into a property which is safe and secure and they will not have to incur major expenses to make repairs to a sub-standard property. In reviewing property condition, close attention is paid to properties built prior to 1978 to determine if there is evidence of lead paint hazards. If there is any evidence of possible lead paint or any peeling or chipping paint, the Seller must permit extensive lead testing (as per the State of Florida guidelines), and if necessary, would agree to any required lead paint abatement. The cost of any lead paint testing and/or removal would be the responsibility of the Seller, and all repairs would need to be completed prior to closing. If the Seller is not willing to incur these costs, the property will be ineligible for assistance from the City of Largo. Even though a Section 8 inspection is completed, the Clients are notified that the Section 8 Inspection does not replace the need for a Home Inspection, and they are highly encouraged to also obtain a Home Inspection of their own (which is much more comprehensive). This is stated in the Pending and Approval letters that Clients receive from our office, and its importance is also discussed at the Homebuyer Workshops. All Clients who seek to receive Sold-on-Largo assistance also must attend the 8-hour Homebuying Workshop from a City-approved Instructor. Tampa Bay CDC is approved by the City to offer these Workshops, and one of the goals of our Workshops is to conserve the existing housing stock. The Workshop provides the Home Buyer with an understanding of the responsibilities of homeownership, and we recently revised our curriculum to put more emphasis on the importance of post-closing maintenance and safety measures. By learning how to maintain a home, this will hopefully maintain or increase the value of the property, and a long-term benefit is gained when re-sale occurs and the Homeowner can realize the equity in their home. Workshop Participants are also provided with information on area Rehabilitation Programs including the City s Programs (in the event repairs or upgrades are needed on the home). Provide information to substantiate the project as an Eligible Activity (described in the application instructions). Describe how one of the following qualifying criteria are addressed by the project: The project, or that part of the project funded by CDBG, principally benefits low- and moderate- income Largo residents or low- and moderate-income areas in Largo, or The project presents or eliminates slum and blight in Largo. 8 THE PROJECT PRINCIPALLY BENEFITS LOW-AND-MODERATE INCOME PERSONS: Tampa Bay CDC has provided Processing services to the City of Largo since In that time, Tampa Bay CDC has closed 275 Downpayment Assistance Loans through 12/31/2017. The majority of these loans (more than 93%) were provided to Clients whose income was below 80% AMI. An analysis of the most recent three years shows the following: 2015: 10 Sold-on-Largo Assistance loans closed 10 (or 100%) were below 80% AMI 2016: 9 Sold-on-Largo Assistance loans closed 8 (or 88.9%) were below 80% AMI 2017: 10 Sold-on-Largo Assistance loans closed 9 (or 90%) were below 80% AMI As noted earlier, Tampa Bay CDC provides technical assistance to Largo Area Housing Development Corporation (LAHDC) to carry out the activities associated with the Neighborhood Stabilization Program (NSP), and through this Program, 31 properties were either renovated or constructed within the City of Largo. Tampa Bay CDC processed the Assistance loans for all of these properties, and due to the price of some of these units, Clients were permitted to have incomes of up to 120% AMI. However, for all Clients who received Sold-on-Largo Assistance loans, 100% were below 80% AMI (since that is the posted maximum income allowed under that Program). All Clients who apply for Sold-on-Largo Assistance are required to attend the 8-hour Homebuyer Workshop from a City-approved Instructor. Tampa Bay CDC is one of the approved Agencies to offer these Workshops to residents of the city of Largo. While the Workshops are available to any individual or family, statistics from the past fourteen years show that we are primarily serving/benefiting low-income households. In nine of the past fourteen years, more than 70% of Workshop Participants had incomes below 80% AMI. This trend is also true for City of Largo residents who our Agency has educated. In 2017, 128 Largo residents attended a Homebuyer Education Workshop (an increase of 44.5% from 2016 attendance). Of those, 68.8% were households with incomes below 80% AMI, A review of the past fourteen years shows that Tampa Bay CDC primarily served very-low and low-income households. In that time, Tampa Bay CDC educated 1,183 Largo residents. In eleven (11) of those fourteen (14) years, more than 70% of the Largo attendees had incomes below 80% AMI. The statistics show the following: 2004: 115 Largo residents were educated; 69.8% were below 80% AMI 2005: 74 Largo residents were educated; 71.6% were below 80% AMI 2006: 94 Largo residents were educated; 73.4% were below 80% AMI 2007: 75 Largo residents were educated; 73.3% were below 80% AMI 2008: 72 Largo residents were educated; 72.2% were below 80% AMI 2009: 84 Largo residents were educated; 73.8% were below 80% AMI 2010: 80 Largo residents were educated; 71.3% were below 80% AMI 2011: 78 Largo residents were educated; 72.7% were below 80% AMI 2012: 67 Largo residents were educated; 69.2% were below 80% AMI 2013: 87 Largo residents were educated; 70.4% were below 80% AMI 2014: 80 Largo residents were educated; 72.7% were below 80% AMI 2015: 78 Largo residents were educated; 73.5% were below 80% AMI 2016: 71 Largo residents were educated; 72.4% were below 80% AMI 2017: 128 Largo residents were educated; 68.8% were below 80% AMI Page 8 of 20

With regards to documenting services to low-moderate income residents, all requests for Downpayment Assistance or Counseling services require Clients to complete an Application. Program Applications are available in both English and Spanish, and they are available in hard-copy format or an online format via Tampa Bay CDC s secure website. All Clients who participate in Homebuyer Workshops are required to complete an Attendance Form. ATTACHMENT C contains a copy of the most current Downpayment Assistance Application, revised in July 2017). The Applications and Attendance Forms capture basic and demographic information including household size (adults and children), income, age, gender, race, marital status, head of household status, disability and military status. Tampa Bay CDC utilizes CounselorMax as our Client Management system. Data from all Applications and Attendance Forms is input into CounselorMax, and we are able to generate monthly reports which show the type of activity, number of participants/recipients, income and ratio analysis, and demographic breakdown. These reports track all program performance goals and they are used as a tool to make changes and adjustments to the Programs to better serve our Clients and to better achieve the goals proposed to our Funding Sources. PROJECT AREA (All applicants must complete this section): Submitted projects must provide services and/or benefit City of Largo residents a) Describe the project area and client base that will benefit from this project (e.g. Citywide, countywide, Community Redevelopment District, homeless population, persons with HIV/AIDS, etc.) 1. PROJECT AREA CITYWIDE Tampa Bay CDC s Homeownership Center is located at 2139 N.E. Coachman Road, Suite 1, Clearwater, Florida While the Tampa Bay CDC office is located in Clearwater, it is only 5 miles from Largo City Hall (per Mapquest), so its proximity is convenient and should not pose too much of an issue for Largo residents to come to our office if they so choose. However, if a Client is unable to come to our office due to a disability of special need, we are more than willing to accommodate their need and make arrangements to meet them at a neutral location within the city limits of Largo. Based on our prior experience, the location of our Clearwater office is usually not a factor in dealing with Largo Clients as the majority of our interaction with them is done via telephone or , and submission of documents is done online 85 90% of the time (rather than in-person). At our Clearwater HomeOwnership Center, Tampa Bay CDC administers all Downpayment Assistance, Counseling and Education Programs, and in addition to serving Largo residents, we serve Clients from Pinellas County, Pasco County, City of Clearwater, and City of St. Petersburg. Some of our Programs even extend to statewide. For the convenience of our Downpayment Assistance Clients who may not be able to come to the office in Clearwater, Applications and required paperwork can be submitted online (through our secure website or through directly to the Lending/Processing Staff, or they can be faxed to (727) For Clients with disabilities, please note that Tampa Bay CDC s Homeownership Center is handicap-accessible, and Tampa Bay CDC currently employs a Team Member who is a sign-language interpreter, so she is able to assist if we have Deaf Clients seeking our services. ATTACHMENT D contains location/service area map for the Tampa Bay CDC s office. 2. CLIENT BASE THAT WILL BENEFIT FROM THIS PROJECT The Client base that will primarily benefit from this Project are Largo residents who are first-time homebuyers with incomes below 80% AMI. Tampa Bay CDC has provided Processing services to the City of Largo since In that time, Tampa Bay CDC has closed 275 Downpayment Assistance Loans through 12/31/2017. In reviewing the data from these closings, we see that a majority of services are being provided to low-income first-time homebuyers (who had not owned homes in the previous three years). PROJECT/PROGRAM OUTCOMES: A) Affordable Housing opportunities are expanded for low-income homebuyers. With the rising housing prices, many low-income Buyers cannot achieve homeownership without Downpayment Assistance and low-rate mortgages. With the increased need for this population, it is hoped that the number of Downpayment Assistance cases closed for the City of Largo will increase from 10 last year to in the fiscal year. It is also a goal to expand opportunities provided to special population beneficiaries. B) Assistance will be provided to an increased number of Homebuyers who earn less than 80% AMI. Tampa Bay CDC proposes to close Assistance loans for the City (an approximate 50% increase from closings this fiscal year). C) Homeownership within the City s Revitalization/Redevelopment areas and Income Eligible Neighborhoods is increased, and the City s housing stock is preserved and improved and preserved through Section 8 Housing Inspections. D) Homebuyers avoid loans with excessive rates and fees, and by utilizing the City s Assistance Funds in conjunction with other Assistance Programs, the Client can obtain mortgages with more affordable monthly payments. E) Homebuyers avoid foreclosure because they have received pre-purchase counseling and have been educated on budgeting, financial literacy and the responsibilities of homeownership. Moreover, Tampa Bay CDC s HUD-approved Counseling Department will maintain communication and follow-up with all Largo Clients who close for a two year period after closing, to insure continued affordability and success. Homeowners will be contacted by Tampa Bay CDC s Counseling Department 4 times during the two year period following their closing to insure they are not experiencing any financial difficulties. If problems arise, intervention and solutions can be implemented prior to delinquency and/or possible foreclosure action. Page 12 of 20

We will see if the property is homesteaded or not. Final determination is made when Tampa Bay CDC obtains the Notice to Seller form from the Seller. Background/Program Experience Include the length of time the agency has been in operation, the date of incorporation, the purpose of the agency, and the type of corporation. Tampa Bay Community Development Corporation was incorporated on December 8,1982 and is a 501c3 Non-Profit Agency whose mission is to provide Education, Counseling, Housing Opportunities to Revitalize Neighborhoods for low to moderate income households. (See ATTACHMENT M for the Agency s 2018 Non-Profit Determination information). The CDC is a HUD-approved Counseling Agency (accredited since 2002). In September 2010, the Agency became a NeighborWorks America chartered organization and in 2012, Tampa Bay CDC was awarded the additional designation as a NeighborWorks Homeownership Center. This designation is federally-recognized and it is awarded to agencies who offer full cycle services, including pre-purchase counseling and education, postpurchase counseling, home retention counseling, foreclosure prevention counseling, and downpayment assistance lending. THROUGH 12/31/2017, TAMPA BAY CDC HAS ASSISTED A TOTAL OF 56,961 CLIENTS with the following Programs: Downpayment Assistance Loans Closed 4,037 Homes Built and Restored 563 Clients Educated at Homebuyer Workshops 35,953 Clients Receiving Pre-Purchasing Counseling 8,520 Clients Receiving Foreclosure Counseling 7,888 Page 16 of 20

This book is reviewed with the Employee by the Payroll Department, and it does contain policies on Equal Employment Opportunity/Affirmative Action as well as the procedures for filing grievances. QUALIFICATIONS OF MICHAEL J. BAUER PRESIDENT/CEO: Michael Bauer has been working with Tampa Bay Community Development Corporation for approximately 1 year and he is currently serving as its President/CEO. He is responsible for oversight of all Programs administered by Tampa Bay CDC (including regulatory and contract compliance, reporting, financial actions, staff management, marketing and business development). Prior to joining Tampa Bay CDC, Mr. Bauer had 10 years of Affordable Housing development and experience in New Orleans and Louisville. QUALIFICATIONS OF CARRIE M. VITALE VICE PRESIDENT/LENDING: Ms. Vitale is hereby identified as the Project Manager. She has been with Tampa Bay CDC for over 24 years and is currently serving as its Vice President. Ms. Vitale is responsible for oversight of processing, eligibility determination and closing coordination for Purchase Assistance loans, which includes the following duties: supervising staff, marketing & outreach, underwriting and approval of all Downpayment Assistance requests, closing and timely delivery of closed, compliant file to the applicable Funding Source, and insuring compliance with the guidelines of the various government funding sources, as well as those of HOME, CDBG, NSP, SHIP, Pinellas HTF, or any other funding source utilized in the delivery of these Programs. During her tenure, Ms. Vitale has administered Programs funded by SHIP, HOME, CDBG, NSP, Pinellas HTF, and HOPE, and miscellaneous funds (i.e. Federal Home Loan Bank Assistance funds), and under her Supervision, 4,037 assistance loans have closed, totaling over 28.1 million dollars.. Prior to joining the CDC Team, Ms. Vitale was employed for eight years at Life Savings Bank and directed its Home Mortgage Programs. In this capacity, she became familiar with the underwriting standards of HUD, Fannie Mae, Freddie Mac, and the VA. This has enabled her to better educate potential homebuyers on the guidelines of these various Programs and it has enabled her to work more efficiently with our Lending Partners. Before entering the Lending/Mortgage industry, Ms. Vitale worked in the Social Service field after obtaining her Bachelor s Degree in Social Work. Page 17 of 20

18 QUALIFICATIONS OF DEBRA JACKSON: Debra Jackson has been working with Tampa Bay CDC since 1985 and she is currently serving as its Vice President of Finance. She is responsible for transacting all financial actions required to facilitate CDC s various Programs. She is also responsible for monthly reporting to all Agencies (as per applicable Contracts) and for reporting to the Tampa Bay CDC Board of Directors. Tampa Bay CDC has an Independent Audit conducted annually by an outside CPA Firm, and there have never been any findings or improper procedures during Ms. Jackson s tenure. QUALIFICATIONS OF TAMMY TREADWAY: Tammy Treadway has been working with Tampa Bay CDC for approximately 1.5 years. She is currently serving as its Loan Processor, and her responsibilities include application intake and review, obtaining proper verification of household size, income and assets to document income eligibility according to 24CFR Part 5, insuring satisfactory Section 8 HQS Inspections have been obtained, and submission of file for Underwriting Review. Prior to joining the Tampa Bay CDC Team, Ms. Treadway was in the insurance industry. She is in the process of obtaining her Mortgage Broker License and has recently attended SHIP Income Certification Trainings through the Florida Housing Coalition. 3. Financial Capacity Attach a copy of the agency s current operating budget (SEE ATTACHMENT K), including revenues and expenditures. Identify commitments for ongoing funding. Describe the agency's fiscal management, including financial reporting, record keeping, accounting systems, payment procedures and audit requirements. In addition to the current contract with the City of Largo, Tampa Bay CDC has ongoing contracts with the City of Clearwater, the City of St. Petersburg, and Pinellas County. The Contract with Pinellas County has a 3-year term through This Contract is currently awaiting an amendment to be done, and until that is finalized by the County, they will be processing DPA loans in-house. It is anticipated that we will resume contracted processing services in May or June With regards to reporting, Tampa Bay CDC maintains copies of all closed files and all applications currently being processed and those previous processed that were deemed ineligible. The Agency has a five (5) year record-retention policy and agrees to retain all programmatic records for five (5) years from the ending date of the County s fiscal year (October 1 through September 30) in which this contract is paid in full, expired or terminated. Tampa Bay CDC agrees that it will provide the City of Largo with any information, records, reports and data as may be required. We acknowledge that all records are subject to audit and, as needed, we agree to permit the City of Largo or its designee to audit or examine records at any time during normal business hours. Methods for measuring and storing records for Housing Programs include, but are not limited to: a) Two web-based pipeline management tools (CMAX and DSC) b) Secured cloud-based data storage c) Retention of hard copies of Client files (closed files, those currently in process, and previous applications that were ineligible) d) Accounting software for financial records management e) Exportable USB password-protected devices Page 18 of 20

19 All file data is entered into the CMAX Client Tracking System, so Tampa Bay CDC can easily make available to the County any data required for reporting purposes. Debra Jackson has been working with Tampa Bay CDC since 1985 and she is currently serving as its Vice President of Finance. She is responsible for fiscal management and for transacting all financial actions required to facilitate CDC s various Programs. She is also responsible for all accounting systems, payment/reimbursement procedures, internal controls, recordkeeping required under IRS rules and as required under our various Funding Contracts. Ms. Jackson provides financial reporting to the Tampa Bay CDC Board of Directors. Tampa Bay CDC has an Independent Audit conducted annually by an outside CPA Firm, and there have never been any findings or improper procedures during Ms. Jackson s tenure. 4. Monitoring Briefly describe how you will monitor progress in implementing the program. Describe how funding and related project costs will be tracked separately in the agency and how the agency will ensure that expenses are not paid from duplicate sources. Describe who will be responsible for monitoring progress. Carrie Vitale (Project Manager) and Debra Jackson will be responsible for monitoring and tracking progress in implementing the program. Debra Jackson maintains separate accounting records for each contract the Agency has for Processing Assistance loans and she also maintains the two accounts from which Downpayment Assistance funds are advanced at closing. Carrie Vitale is responsible for sending reimbursement requests to each Funding Jurisdiction after a Downpayment Assistance file has closed. Since we have 4 separate Contracts and these are specific to certain geographical areas, requesting payment from duplicate sources would not be possible. Reimbursement requests can only be sent to the Agency who is the Mortgagee for the Downpayment Assistance transaction. Ms. Vitale will also input data from each closing into a spreadsheet which tracks demographic information for each closed file. This excel report allows us to provide our Funding Sources with custom reports on files that have closed. 5. Insurance/Bonding/Worker's Compensation State whether or not the agency has liability insurance coverage, in what amount, and with what insuring agency.
