Make a Difference! 2017 Rally for School Health at Ford Field in Detroit on Tuesday, October 3rd or 10th The Fuel Up [to Play 60] Rally offers my students a first-hand look at how physical activity and healthy eating activities can be implemented in our school and the research behind it in an age-appropriate way. My students came back pumped up and motivated to make changes not just in school but in their home lives as well. Apply now for a Fuel Up to Play 60 Rally for School Health Grant. Your school could win a motivating, energy-packed day full of fun, movement and learning at Ford Field, home of the Detroit Lions. Elementary or middle schools with a combination of grades 4-8 interested in implementing Fuel Up to Play 60 are invited to apply! What is Fuel Up to Play 60? A nationwide in-school health and wellness program created to empower youth to lead the way in implementing sustainable nutrition and physical activity changes at their school. 2 Fuel Up to Play 60 Rally for School Health Grant will provide the following: Thirty schools awarded by random selection based on school demographics, geographic location, and performance on previous grants. *Multiple buildings in a district may apply, but only one building per district will be selected. Funds to support travel to Ford Field and related travel expenses for up to nine team members; 6 students and 3 adult Program Advisors. Funding amount is determined by distance from the school building to Ford Field in Detroit: - Within 150 miles = $500 - Greater than 150 miles = $1000 plus up to three hotel rooms with valet parking at the Atheneum Hotel in Detroit near Ford Field. A day-long, action packed Fuel Up to Play 60 Rally that will empower and teach students (and the adults who join them!) ways to begin/continue making changes at their school supporting healthy eating and physical activity. A Fuel Up to Play 60 themed Instagram frame to use during Plays and events throughout the year. A Kickoff Kit including promotional items, sent directly to the school to start your school year and build up excitement for other students and teachers to join in your school wellness efforts. The opportunity to apply for additional funds to help implement Fuel Up to Play 60 (up to $4,000 available per school building) is available on each school's Dashboard at Grant Requirements: If your school is selected as a recipient of a Rally for School Health Grant, you ll be required to sign a Memorandum of Understanding (MOU) with United Dairy Industry of Michigan, agreeing to complete the following grant requirements during the school year: All nine team members (including six students) must be available to participate all day at the event (8:30am-3:00pm on either October 3rd or October 10th, 2017). Three adults and six students make up your school team two of the three adults must be registered as Program Advisors at Key school leaders Principal, Food Service Director/Manager, and Physical Education teacher must approve of this grant application submission and will fully support the proposed program and requirements. Grant Requirements Continued: Important Dates May 10th, 2017 (Wednesday) Mandatory webinar/ conference call #1 for all grant awarded schools. At least one adult Program Advisor must be present. Agenda will cover grant requirements, what to expect from the Rally event and how to register your team. You can call in at 7 a.m. OR 3:30 p.m. for a 30-minute call. Three additional 30-minute Program Advisor webinars/conference calls will be mandatory, scheduled in December 2017, February 2018, and March/April exact dates TBD. Participants can choose from call times at 7:00 a.m. OR 3:30 p.m. for each call. May 26th, 2017 (Friday) Deadline for grant winners to submit ALL registration paperwork. November 10, 2017 (Friday) Must coordinate and execute a school-wide Fuel Up to Play 60 Kickoff activity/event on or prior to selected date. Fall 2017 Schools should identify, coordinate and begin implementation of at least one Healthy Eating Play and at least one Physical Activity Play that is sustained for at least four weeks. We encourage Plays to last through the end of the school year. April 1, 2018 Complete all Six Steps of Fuel Up to Play 60 and check them off on the Dashboard at to be nationally recognized as a Touchdown School and be eligible to win prizes. May 18th, 2018 Final submission of three (3) success stories with pictures/videos on one each for Kickoff, Healthy Eating Play and Physical Activity Play. Complete UDIM success story confirmation form. NEW: Must report pre- and post- school milk & yogurt sales, ADP (average daily participation) of breakfast and lunch at the end of the school year. Will need to work with Food Service Director to obtain this. Technology Requirements: School/Program Advisor(s) must acknowledge that computers being used need to have updated PDF reader software. School teams Program Advisor should be competent at opening, saving, and sending files (PDF s, Word documents, and pictures), or be willing to communicate with school s IT(information technology) person for assistance. School teams Program Advisor must be tracking their progress with Fuel Up to Play 60 Steps as they are completed on the website (Dashboard) at

4 SCHOOL TEAM INFORMATION If your school is selected, all team members (including six students) must be available to participate in the all day (8:30 AM - 3:00 PM), Ford Field Fuel Up to Play 60 Rally for School Health (possible dates are October 3rd or 10th). Due to limited space, schools may not bring additional students or adults. Schools that have attended the Rally (previously known as the Kickoff ) before are eligible to apply, however these schools must bring six new students if selected. Using the table below, list your Fuel Up to Play 60 adult school team members and contact information. Have each sign in the space provided. Your team must include: Two adult Program Advisors (who are registered as Program Advisors at Another adult of your choice. It is strongly recommended to bring one of the following: Physical Education teacher, Food Service director or manager, school counselor, principal, parent, classroom teacher, or any other health champion at your school. Program Advisor #1 (This person will serve as the main grant contact responsible for all communication with UDIM regarding team changes and completion of the grant requirements.)
