6 Foundation for a Healthy St. Petersburg
7 Curtis Holloman Senior Director of Grants & Programs Mr. Holloman has Masters Degrees in Political Science from Appalachian State University in North Carolina and in Business Administration from Temple University. He most recently served as Deputy Director of the Robert Wood Johnson Foundation Local Funding Partnerships, which is a national grant making program to support innovative communitybased projects to address root causes of barriers for maximum health outcomes.
8 Leah Barber-Heinz Director of Communications & Advocacy Ms. Barber-Heinz has Masters Degrees in Public Administration and in Social Work from Florida State University and is a native from St. Petersburg. She was most recently the CEO of Florida CHAIN (Community Health Action Information Network) predominantly leading the charge of Medicaid Expansion in the State of Florida.
9 Belinda Henderson Director of Finance Ms. Henderson has a Bachelor s degree from the University of South Florida and over 15 years of experience in Accounting. She was with Bayfront Medical Center and Hospital until she transferred to the Foundation two years ago as Lead Accountant. She was made Director of Finance in May of this year.
10 Leila Martini Director of Policy & Research Ms. Martini has a Doctorate in Public Health from the University of South Florida, Masters degrees in Library and Information Sciences and in Public Health. She was most recently working on an evaluation for the Centers for Disease Control and Prevention in State Policies around tracking of chronic disease. Previously, she was an employee of USF where she served as Assistant Director of Tobacco Training and Cessation at the Area Health Education Center.

13 Foundation for a Healthy St. Petersburg Conflict of Interest Policy A Conflict of Interest exists where personal, financial, or other interests compromise an individual s: Objectivity Professional Judgment Professional Integrity Ability to perform responsibilities to the Foundation Please look for our policy posted on our website. Conflict of interest scenarios will be posted to our website. In 2016 we will release our grant review procedures and post those to the website.

What conditions are required for meaningful collaborations? 5. What conditions are missing to support the best collaborations?
21 Listening Sessions What did we hear from residents? Health disparities by location, race, class, age Many residents expressed this sentiment Food is expensive! Especially healthy food! Grandparent quote many residents echoed cost/availability of food Cannot afford health care Single mom in college repeated by many residents Generational Apathy coined by a resident a loss of residents who care about the community. Residents want the Foundation to bring the community together. Motivate more residents to be involved.
22 Listening Sessions What did we hear from providers? Break down silos to help the community work together and have an impact Build trust between the Foundation and the community improvement requires buy-in and trust. Transparency, inclusiveness, motivational support, and highlight successes. Lead we re missing leadership with a respect for history and existing community assets. Be neutral and an honest broker. Never stop listening.
