2 REQUEST FOR PROPOSALS SUBSTANCE ABUSE PREVENTION AND TREATMENT SERVICES Ozaukee County is seeking organizations that are willing to provide prevention and treatment services to individuals with alcohol and other drug abuse issues under the Substance Abuse Prevention and Treatment Block Grant. The recipient of the grant will remain in effect until Ozaukee County deems necessary to request a Request for Proposal from other agencies. There will be up to three focus areas an organization may apply for. The focus areas are listed below with the dollar amount attached to each: 1. Pregnant women ($20,000) 2. Prevention services ($22,000) 3. Treatment services ($43,354) Proposals shall be postmarked by the United States Postal Service on or before 12:00 midnight of October 29, 2016, and received no later than 10 days from the submittal deadline. Hand delivered proposals shall be received no later than 5:00 p.m., on October 31, 2016, at the Ozaukee County Clerk s Office located at 121 W. Main Street, Port Washington WI Proposals postmarked or hand delivered after the submittal deadline shall be considered late and rejected. There are no exceptions to this requirement. 3 The Substance Abuse and Mental Health Services Administration (SAMHSA) provides funding to the State of Wisconsin Department of Health Services through the Substance Abuse Prevention and Treatment Block Grant. Ozaukee County receives $85,354 of this funding to provide prevention and treatment services within the county. The Department of Health Services Division of Care and Treatment Services is responsible for collecting and reporting required data to SAMHSA. The Federal Block Grant requires not less than 20 percent of Substance Abuse Prevention and Treatment Block Grant funding be spent on primary alcohol and other drug abuse prevention services. In order to provide more accurate data to SAMHSA on prevention programming, the Division of Care and Treatment Services collects information from counties using the Substance Abuse Prevention-Services Information System. The selected organization will be required to collect data as it is a mandate under the Federal Substance Abuse Prevention and Treatment Block Grant and National Outcomes Measurement System. There is preference in admission to pregnant women who seek or are referred for and would benefit from Substance Abuse Prevention and Treatment Block Grant funded treatment services. Further, all entities that serve women and who receive block grant funds must provide preference in the following order: To pregnant injecting drug users first. To other pregnant substance abusers second. To other injecting drug users third. Women with dependent children fourth. Homeless persons with co-occurring disorders fifth. To all others individuals. Interested organizations must submit a written proposal that includes the following information: 1. Which focus area the applicant is applying for; 2. How the applicant will meet the requirements of the focus area; 3. If applying for the Pregnant Women focus area, how will pregnant women be identified and what services will be provided; 4. If applying for the prevention focus area, what prevention services will be provided; 5. If applying for the treatment focus area, what evidence based programming will be utilized; 6. Staff breakdown of the focus area(s) of the applicant; 7. Fiscal breakdown of how the Substance Abuse Prevention and Treatment Block Grant will be spent. 1

4 Proposal Application Instructions General instructions for completing applications: Proposal Applications shall be submitted to Ozaukee County Department of Human Services, attn. Liza Drake. The applicant organization s name should be retained on the top right hand corner of each page. Page numbering of the Proposal Application should be consecutive, beginning with page one and continuing through for each section. Proposals may be submitted in a three ring binder (Optional). Tabbing of sections (Recommended). A written response is required for each item unless indicated otherwise. Failure to answer any of the items may consider an applicant s proposal as incomplete. The Proposal Application comprises the following sections: Proposal Application Focus Area Statement Program Overview Experience and Capability Project Organization and Staffing Service Delivery Financial Other I. Program Overview Applicant shall give a brief overview to orient evaluators as to the program/services being offered. II. Experience and Capability A. Necessary Skills The applicant shall demonstrate that it has the necessary skills, abilities, and knowledge relating to the delivery of the proposed services. B. Experience The applicant shall provide a description of projects/contracts pertinent to the proposed services, for example previous experience with individuals struggling with alcohol or other drug abuse issues. 2

5 C. Quality Assurance and Evaluation The applicant shall describe its own plans for quality assurance and evaluation for the proposed services, including methodology. D. Coordination of Services The applicant shall demonstrate the capability to coordinate services with other agencies and resources in the community. III. Project Organization and Staffing A. Staffing 1. Proposed Staffing The applicant shall describe the proposed staffing pattern and proposed caseload capacity appropriate for the viability of the services. 2. Staff Qualifications The applicant shall provide the minimum qualifications for staff assigned to the program (including licensure when providing treatment services). B. Project Organization 1. Supervision and Training The applicant shall describe its ability to supervise, train and provide administrative direction relative to the delivery of the proposed services. 2. Organization Chart The applicant shall describe the approach and rationale for the structure, functions and staffing of the proposed organization for service activity and tasks, including the position title, name and full time equivalency of each staff and line of responsibility/supervision. Both the Organization-wide and Program organization charts shall be attached to the Proposal Application. 3

6 IV. Service Delivery Applicant shall include a detailed discussion of the applicant s approach to applicable service activities and management requirements. The use of evidence-based programming is encouraged. Applicants can use the following sites to review approved programming: and A. Service Activities 1. Describe your general philosophy framework and standards, which guide your planned activities and service delivery. 2. Describe the main components of your service delivery, but not be limited to (include staff assigned to each activity): B. Management Requirements 1. Describe work assignments and responsibilities for the management of the organization in the following areas: a) Personnel. b) Administration. 2. Describe work assignments and responsibilities for: a) Quality assurance. b) Evaluation. c) Reporting. Applicants shall identify their baseline for the performance measures. Given available resources and other external factors, the applicant shall formulate both reasonable and achievable performance objectives, and the approach to be taken in meeting these objectives for the contract period. The measures are subject to ongoing review and revision. 4

7 V. Financial A. Pricing Structure Applicant shall submit a cost proposal utilizing the pricing structure. A detailed description of how the money will be spent shall be attached to the Proposal Application. 5
