Golden LEAF disaster recovery grant funds will be used to develop a flood reduction feasibility study. The study will identify engineering options to reduce the direct flow of water into the town during river flooding events. Prior to Hurricane Matthew, Bertie County began exploring ways to add more affordable housing for teachers in the County. The State Employees Credit Union (SECU) is working in Bertie County to finance the development of 24 housing units exclusively for teachers and state employees. Golden LEAF disaster recovery grant funds will support construction of new infrastructure. The Rescue Pump Station near Granville Park has flooded and been subsequently repaired with each of the last major storms. Fallen trees and sediment accumulation are blocking the flow of water in area streams. Golden LEAF disaster recovery grant funds will be used to remove storm debris from various streams. Bladen County received $242,000 from the Department of Agriculture to complete this project. The County water system lacks sufficient reserve power capacity to keep all of the wells operating in the event of a total power loss as experienced during Hurricane Matthew. Golden LEAF disaster recovery grant funds will support installation of generators at four critical water supply wells. Flooding as a result of Hurricane Matthew damaged the Town-owned building used by the Elizabethtown Volunteer Rescue Squad, a 501(c)(3) nonprofit organization. FEMA funding will be used to demolish the building and construct a stormwater pond. The Town has acquired a nearby site large enough to accommodate not only the Volunteer Rescue Squad but also the town Fire Department. Colocation of these services is expected to better serve the Town and surrounding unincorporated areas. Golden LEAF disaster recovery grant funds will be used to design, engineer, and construct the station. The town-owned Yaupon Pier in Oak Island suffered extensive damage as a result of Hurricane Matthew. Following inspections by the Town's insurance company, consulting engineers, and FEMA, the remaining pier structure was condemned. Golden LEAF disaster recovery grant funds will be used to demolish and rebuild the pier. FEMA and Insurance funds are not sufficient to complete this project. The Town of Boardman obtains its water supply from the County Water System via a water main installed under the Lumber River at the US Hwy 74 crossing. Flooding from Hurricane Matthew caused a break in the supply line crossing the river bed. Golden LEAF disaster recovery grant funds will be used to replace the water main and restore the primary water supply for the town. 2 1/25/18

Due to mold, mildew, and unsafe conditions, the building was condemned. Golden LEAF disaster recovery grant funds will be used to complete design, engineering, and construction of a replacement town hall. FEMA funding is inadequate to replace and mitigate future flooding. Flooding from Hurricane Matthew destroyed a significant number of available rental units. The Lumber River Council of Governments, Housing Finance Agency, and the Town are working with a developer to construct 36 units outside of the floodplain. Golden LEAF disaster recovery grant funds will be used to extend water and sewer to serve the new complex. Fair Bluff's Town Hall, Police Department, Visitor's Center, and Public Works facilities were destroyed by Hurricane Matthew. Golden LEAF disaster recovery grant funds will be used to renovate an existing building to be acquired by the Town to serve as the Town Hall, Police Department, and the Visitor s Center. Funds will also support up fit of an office for the Public Works Department. Because the Town did not have flood insurance on these buildings, FEMA Public Assistance will not be sufficient to replace these facilities. Following initial award, this grant was increased to provide contingency funding for the replacement of one of the Town's wells. Funds for replacement will be provided by the Department of Environmental Quality. Flooding as a result of Hurricane Matthew destroyed the station owned and operated by the Fair Bluff Fire Department and Rescue Squad, a 501(c)(3) nonprofit organization. FEMA funding and insurance proceeds are not sufficient to replace the station. Golden LEAF disaster recovery grant funds will be used to complete construction of a new station outside of the floodplain. 3 1/25/18

Golden LEAF disaster recovery grant funds will be used to develop a Stormwater Plan for the Downtown Municipal Service District and necessary outfalls. Golden LEAF disaster recovery grant funds will be used to improve the stormwater infrastructure to reduce frequent flooding. Hurricane Matthew and Tropical Storms Hermine and Julia generated a significant amount of debris in the streams and tributaries of the County, impeding drainage and posing future flood risks in the county. Golden LEAF disaster recovery grant funds will be used to repair and clear blocked drainage infrastructure. Damage caused by Hurricane Matthew affected the City's ability to maintain McCotter Canal, impeding access to the Canal and two water supply wells. Golden LEAF disaster recovery grant funds will be used to stabilize the canal banks and reconstruct the access road to repair damage. As a result of Hurricane Matthew, a significant amount of debris is impeding water flow in more than 100 miles of creeks and streams within unincorporated County. Hurricane Matthew caused a significant amount of debris to accumulate in Cross Creek. Golden LEAF disaster recovery grant funds will be used to remove and dispose of debris from Cross Creek. As a result of Hurricane Matthew, a significant amount of debris is impeding water flow in Blounts Creek, Branson Creek, Hybarts Branch and Buckhead Creek in the City of Fayetteville. Golden LEAF disaster recovery grant funds will be used to remove and dispose of storm debris. Flooding from Hurricane Matthew washed out the City maintained access road crossing a dam into the Rayconda Subdivision. A temporary gravel road currently provides residents and emergency vehicles access to the community. Golden LEAF disaster recovery grant funds will be used to complete construction of a new roadway. Flooding from Hurricane Matthew damaged the stormwater system located along Spruce Street in the Forest Hills neighborhood. The system was overcharged by high water causing streets to be overtopped, resulting in erosion and flooding of homes located in lowlying lots. Golden LEAF disaster recovery grant funds will be used to replace undersized stormwater infrastructure. As a result of Hurricane Matthew, a significant amount of debris is impeding water flow in Beaver Creek and Bones Creek in the City of Fayetteville. The debris buildup poses a significant risk of flooding during heavy rains. Golden LEAF disaster recovery grant funds will be used to remove and dispose of storm debris. 5 1/25/18

Three city owned buildings and 25 city owned grave sites are at risk should the bank further destabilize. Golden LEAF disaster recovery grant funds will be used to address issues with the buildings and soil borings for the bank. FEMA Public Assistance is not sufficient to complete this project. As a result of Hurricane Matthew, a significant amount of debris is impeding water flow in Rockfish Creek and Little Rockfish Creek in the Town of Hope Mills. Golden LEAF disaster recovery grant funds will be used to remove and dispose of storm debris. Hurricane Matthew unexpectedly brought wind, water and debris onto the northeast side of Roanoke Island and into the channel leading into the Wanchese harbor, blocking the channel. Golden LEAF disaster recovery grant funds will be used to reimburse the county for its share of the emergency dredging of the channel. Flooding as a result of Hurricane Matthew destroyed the town-owned fire station leased by the Princeville Volunteer Fire Department, a 501(c)(4) organization. The Volunteer Fire Department serves the town and unincorporated areas of the county. Edgecombe County will acquire the land, manage the construction, and lease the completed building to the Princeville Volunteer Fire Department. Golden LEAF disaster recovery grant funds will support construction of the station. Hurricane Matthew caused major flooding in the M.A. Ray Center, Clark Park, and surrounding neighborhood. Golden LEAF disaster recovery grant funds will be used for repair and replacement costs not reimbursed by FEMA Public Assistance. 6 1/25/18
7 Greene County Greene Greene County EOC $650,000 Greene County currently lacks a facility capable of serving as an emergency operations center (EOC) during major events. As a result, state emergency management must deploy additional resources to provide and facilitate response and recovery. The County will be taking over a recently decommissioned National Guard Armory in Snow Hill. Golden LEAF disaster recovery grant funds will be used to make necessary renovations to this facility. High water as a result of Hurricane Matthew damaged the historic Will Will Willis Store & Fish Willis Store & Fish House and Dock. Golden LEAF disaster recovery The Ocracoke Hyde House and Dock - Disaster $175,820 grant funds will be used to replace the support system of the Will Foundation, Inc. Recovery Willis Store & Fish House structure, demolish and rebuild the dock, and repair exterior and interior areas of the structure. Benson Benson Benson Johnston Johnston Johnston Town of Benson - Replacement Fire Engine 2 Town of Benson Replacement Public Works Building Town of Benson Stormwater Infrastructure Rehabilitation $422,158 $500,000 $565,187 During Hurricane Matthew, the Benson fire department responded to 62 emergency calls, 32 of which were water rescue calls. During one of these water rescues the Department s 1996 Freightliner fire engine was submerged in water for an extended amount of time and flooded out. Golden LEAF disaster recovery grant funds will be used to replace the fire engine. Insurance is not sufficient to replace this equipment. The force of floodwater from Hurricane Matthew unearthed a 60-inch storm drain pipe adjacent to the Town of Benson s Public Works building, flooding the structure and damaging equipment. The Town has identified an available vacant building to serve as the new Public Works building. Golden LEAF disaster recovery grant funds will be used to improve the site, up fit the building, and replace equipment. Flooding from Hurricane Matthew caused significant damage to several areas of Benson s stormwater infrastructure, causing sinkholes and other issues. FEMA Public Assistance is not available because the town did not have easements prior to Hurricane Matthew, though the town maintained the system. Golden LEAF disaster recovery grant funds will be used to complete stormwater repairs throughout the town. 7 1/25/18

FEMA Public Assistance is not sufficient to complete repair. Golden LEAF disaster recovery grant funds will be used to support repairs and mitigation of the ditch. The Town of Smithfield believes Hurricane Matthew affected its stormwater infrastructure, threatening properties outside the flood hazard area. To better understand how to resolve the issues, the Town is working to update a study conducted in Golden LEAF disaster recovery grant funds will be used for development of a stormwater capital improvement plan. Hurricane Matthew damaged the roof of the Town s water treatment plant contributing to damage of the electrical control panel. Golden LEAF disaster recovery grant funds will be used to replace of the electrical control panel. Flooding as a result of Hurricane Matthew contributed to degradation of an existing culvert crossing of Highway 17. Pollocksville operates a water distribution main adjacent to Highway 17 that crosses the culvert. Golden LEAF disaster recovery grant funds will be used to replace the waterline crossing with a horizontal directional bore below the channel. The City of Kinston experienced substantial flooding due to Hurricane Matthew, resulting damage and destruction of residential properties. A private developer has proposed constructing a new 60-unit apartment complex at the end of Doctors Drive, located outside of the 100-year floodplain. Doctors Drive is currently a dead-end street. During Hurricane Matthew parts of the street were flooded making it impassable. Golden LEAF disaster recovery grant funds will be used to extend Doctors Drive an additional 2,800 feet to connect with Airport Road and to extend an 8" water line along the extension to address fire protection and water supply needs. 8 1/25/18

The buildings were submerged in several feet of water making them unusable and causing the staff from the Extension Office to temporarily relocate to space in downtown Kinston. Golden LEAF disaster recovery grant funds will be used to replace the Cooperative Extension building on an elevated lot to mitigate the risk of future flooding. Insurance is not sufficient to complete this project. Flooding, power surges and outages, as well as tree damage, resulting from Hurricane Matthew affected the Town's pump stations. Because the pump station require upgrades beyond pre-disaster condition, FEMA Public Assistance is not sufficient to complete this project. Golden LEAF disaster recovery grant funds will be used to complete repairs and hazard mitigation improvements for two of these sites. As a result of Hurricane Matthew, fallen trees and debris impede the natural flow of water along the Tar River. Golden LEAF disaster recovery grant funds will be used to remove and dispose of storm debris. Stormwater from Hurricane Matthew strained stormwater infrastructure in Nashville along Elm Street and under adjacent public parking areas. Damage to pipes, including separated joints, resulted in multiple leaks and sinkholes. Golden LEAF grant funds will be used to replace the Elm Street storm drainage infrastructure. Hurricane Matthew damaged a section of the Parkers Canal culvert. Preliminary engineering analysis recommends removal of the culvert and replacement with an open channel. Golden LEAF disaster recovery grant funds will support replacement of this stormwater infrastructure. Hurricane Matthew exacerbated deterioration of the boat access ramp in Sunset Park. This ramp serves as the only public water access point in the area. Golden LEAF disaster recovery grant funds will be used to replace this infrastructure. 9 1/25/18

Golden LEAF disaster recovery grant funds will support replacement of failed portions of sewer infrastructure. Flooding as a result of Hurricane Matthew further deteriorated stormwater infrastructure in the Montclair subdivision. Collapsed and failing pipes have resulted in localized flooding and formation of sinkholes. This Golden LEAF grant will support replacement of stormwater infrastructure to restore drainage to pre-disaster capacity and eliminate existing hazards. Grant funds will support improvements to two pump stations serving Grifton, Ayden, and Winterville. These pump stations have been subjected to frequent flooding, most recently as a result of Hurricane Matthew. These improvements will enable the pump stations to maintain operations in a future disaster event. Two taxilanes at the Pitt-Greenville Airport were submerged for 6 days when waters from the adjacent Tar River flooded the airport following Hurricane Matthew. The taxilane pavement is failing and breaking apart. The Airport has previously been awarded grant funds from the Department of Commerce and Department of Transportation. Due to higher than anticipated bids, Golden LEAF grant funds are requested to complete repair work. Hurricane Matthew caused rapid stormwater runoff that contributed to the failure of stormwater infrastructure under Mill Street in Fountain. Failure is evidenced by sinkholes in the town s stormwater right-of-way and partial collapse of sections of the street. Golden LEAF disaster recovery grant funds will be used to replace the existing stormwater infrastructure under Mill Street. 10 1/25/18

The Lumbee Tribe of North Carolina, through its 501(c)(3) nonprofit Lumbee Land Development, Inc., will develop a total of 50 units spread across three new single family housing developments outside of the floodplain. Most of the funding for the project will be provided through existing non-disaster related federal programs including the Native American Housing Assistance and Self Determination Act of 1996 (NAHASDA) and through HUD and the Title VI Loan Guarantee Program. Golden LEAF disaster recovery grant funds will be used to support extension of water and sewer infrastructure to the new housing developments. Several facilities owned by the City were flooded and damaged by Hurricane Matthew. Because the City did not have flood insurance, FEMA will reduce the City's allowable Public Assistance claim for those facilities by as much as 70%. FEMA will also require the City to acquire and maintain flood insurance on these facilities. This Golden LEAF grant will meet unmet needs including building repairs, design services, and mold remediation. Damaged facilities include a water treatment plant, a fire station, two electric utility buildings, and two recreation buildings. As a result of Hurricane Matthew, the Tanglewood neighborhood experienced significant flooding. Golden LEAF disaster recovery grant funds will be used to replace stormwater infrastructure. A critical outcome for the project is establishing a more reliable access into the hospital, which was inaccessible during and after the storm. To prevent future potential flood occurrences thru the CSX railroad penetration under I-95, Golden LEAF disaster recovery grant funds will be used construct a mechanical floodgate system and earthen pumping platform. CDBG-DR will support a portion of this project cost. 11 1/25/18

Insurance proceeds are not sufficient to rebuild the station and replace destroyed equipment, including a fire truck and rescue vehicle. Golden LEAF disaster recovery grant funds will be used to complete the station and replace the rescue vehicle. Flooding as a result of Hurricane Matthew caused significant damage to County Public School facilities. Golden LEAF disaster recovery grant funds will be used to complete repair and renovation of W.H. Knuckles Elementary and Transportation Department. These buildings did not have flood insurance therefore FEMA Public Assistance is not sufficient to replace these facilities. Funds will also be used to up fit the Maintenance Warehouse to partially replace functions from the destroyed Central Office. As a result of flooding caused by Hurricane Matthew, the Public Schools of County lost a total of 94 vehicles. These vehicles served a variety of purposes including maintenance, driver's education, student programs, and central office support. Additionally, other equipment was either damaged or destroyed. Golden LEAF disaster recovery grant funds will be used to repair or replace equipment where insurance, FEMA, and other funding is not sufficient. Golden LEAF grant funds are requested to design, drill, and install a well onsite to mitigate against future water loss. St. Paul's lacks sufficient stormwater infrastructure to remove water from the natural bowl at Johnson and Elizabeth streets. Approximately 50 acres of the Town, including more than 90 homes, businesses, and churches, drain to this location. Golden LEAF disaster recovery grant funds are requested to install drainage infrastructure to mitigate future flooding. CDBG-DR will support a portion of the project cost. 12 1/25/18

The pump station located off Hillandale Road is the main receiving station from the northeast side of Fremont which receives all of the wastewater from the Town of Eureka. Saturated soil conditions following Hurricane Matthew caused the building to settle approximately two inches. Golden LEAF disaster recovery grant funds will be used to replace the damaged pump station with a submersible pump station. As a result of Hurricane Matthew, city-maintained stormwater infrastructure in the Glenwood Trails neighborhood of Goldsboro was damaged, resulting in flooding and sinkholes on private property. Golden LEAF disaster recovery grant funds will be used to replace this damaged infrastructure. Flooding as a result of Hurricane Matthew impeded access to the Seven Springs Community Volunteer Fire Department, currently located in the flood zone. The Fire Department has applied for a buyout and acquired land outside of the floodplain. Golden LEAF disaster recovery grant funds will support construction of a new station. The EMS building in Seven Springs was flooded during Hurricane Matthew. Golden LEAF disaster recovery grant funds will be used to relocate the EMS station to a new location outside of the floodplain. 13 1/25/18

Community Development Initiative Wayne Disaster Area Genoa Lift Station Replacement Disaster Recovery Loan Program $178,000 $2,250,000 $2,000,000 $750,000 During tropical storms Julia and Hermine, and Hurricane Matthew, woody debris, such as branches, logs, and stumps were deposited in streams creating an excessive amount of material that acts to impede natural water flow and causes excessive erosion and deterioration. Golden LEAF disaster recovery grant funds will be used to remove and dispose of storm debris. As a result of Hurricane Matthew, a significant amount of debris is impeding water flow in Wayne County streams. Golden LEAF disaster recovery grant funds be used to complete stream cleanout projects and thereby reduce the risk of flooding to homes and businesses, agriculture fields, poultry and swine facilities, and surrounding natural habitats. The Genoa Lift Station has frequently been subjected to flooding, most recently by Hurricane Matthew. Golden LEAF grant funds will be used to rebuild the lift station. Grants provided to entities to make loans to small businesses affected by Hurricane Matthew, Tropical Storms Julia and Hermine, and the western wildfires. 14 1/25/18
