6 SECTION VII: Letter of Agreement In consideration of the Canadian Curling Association also known as Curling Canada (CCA) awarding me a Curling Canada Foundation 2018 For the Love of Curling Scholarship, I, hereby agree to the following: 1. I acknowledge that I have reviewed the Code of Conduct (Code) made available through 2. I hereby agree to comply with all provisions of the Code and understand that the Code is an intrinsic part of the sport of curling generally and of this Award in particular, and I agree to abide by this Code. 3. I have read the disciplinary proceedings provisions of the Code and agree that any conduct in contravention of the Code that I might engage in will be dealt with by those provisions. 4. I consent to the CCA and their agent or appointees: a) Photographing; b) videotaping; c) filming and/or; d) recording my voice and further; e) Exhibiting, publishing, and broadcasting my name or any photographs, videotapes, films, or other likenesses of me or any sound recordings of my voice in any promotion, reports, or advertising about or in connection with the CCA. 5. I hereby waive the right to any payments or royalties in connection with the actions described above, regardless of whether such exhibition, publication or broadcast is on a commercial basis and regardless of whether a fee is paid to anyone in connection with such exhibition, publication or broadcast. 6. For the purpose of building awareness for the Canadian Curling Association/Curling Canada, the Curling Canada Foundation, the For the Love of Curling scholarship program and the sport of curling, I consent to: a) Being readily available for interviews for the purposes of creating communications to be used for fundraising. b) Write one blog post, in coordination with the Foundation Manager, to be used in the promotion of the scholarship program or as stewardship for CCA donors. c) Organize one fundraising event or initiative in support of the Curling Canada Foundation national youth programs. d) Act as an Ambassador for Curling Canada Foundation national youth programs, by volunteering at one CCA event or initiative, as schedules and geographic locations permit. e) Wearing logos, name badges and other CCA identification on apparel provided by the CCA.

8 SECTION VIII: REQUIRED ATTACHMENTS In addition to the completed application form and signed letter of agreement, please include: Personal Essay [30 Points] Write a personal essay (maximum 2 pages), that demonstrates how; a) the undeniably strong sense of community in our sport has played a positive role in your personal curling journey, b) our sport has helped to shape the person you are today on and off the ice and c) you are and will be an ambassador for youth curling in Canada. Note: The essay is given careful consideration by the selection committee. Please ensure your essay is given significant attention in your application. It need not be longer than 2- pages. Two (2) Letters of recommendation [10 Points] o Please ensure those providing a reference letter include their name, phone number, length of time they have known you, and reasons why they believe you are deserving of a scholarship. A copy of your Grades or Transcript for the last academic school year. Acceptance letter from a university or college if this will be your first year. A head and shoulders photo, of high enough quality to be used in digital and print publications JPG format. It is the applicant s responsibility to contact Curling Canada to verify that all required forms are on file by the deadline date.
