1 Carnegie Mellon University Office of International Education INSTRUCTIONS The information requested on the International Student Information form is required from you to issue a Certificate of Eligibility (I-20 or DS-2019) for Carnegie Mellon University. International students must have a Certificate of Eligibility in order to legally enter and/or remain in the United States. Complete all forms accurately and mail it along with all required supporting documents to: Carnegie Mellon University Dept. of Civil and Environmental Engineering Porter Hall 119 Pittsburgh, PA CHECKLIST Read all instructions and review all details carefully. Keep a copy of these completed forms for your records. It is in your best interest to submit clear and concise documentation to help the process go smoothly. Packages containing incomplete information or insufficient funding will not be processed. Forms that are sent by or faxed will not be processed. Include all of the following as one complete package. Type all data electronically. Completed and signed International Graduate Student Information Form Completed and signed Affidavit of Support (if required) Original financial documentation Copy of the picture page of the passport (for student and dependent(s) Copy of CEE admission letter Transfer In Form (ONLY for students who are currently in the U.S. in F-1 visa status) All financial documentation must: be in English (or an official translation). be original documentation (no photo copies, faxes or scans). be dated less than 6 months from the date of submission. clearly state the name of the institution and the account holder in English as well as the available balance in the account (including internet bank accounts). If necessary, foreign currency will be converted upon review of the documents. Financial support must equal or exceed tuition and expenses for the first academic year. We must also consider support for the remainder of the academic program. Therefore, you and/or your sponsor must include additional financial documentation or a letter explaining how future expenses will be paid. Financial documentation from each source should be one single page. Do not submit lengthy bank statements with transaction history. Investment accounts (stocks, bond, mutual funds, etc) will be accepted at 50% of the current value. Submit an original investment statement clearly indicating all of the requirements noted above. You should obtain at least 2 sets of original financial documents. Submit one set with this application and keep one set for your visa application and to carry with you to present to the immigration officials upon entry to the US. Notarized statements are not required. NOT ACCEPTABLE: Life insurance policies, tax statements, payroll statements, personal property, retirement/pension accounts, Chartered Accountant statements, real estate (either the rental of and/or the intended sale of).
2 TRANSFER of SEVIS RECORD Students who are currently attending a school in the United States in F-1 visa status must request a transfer of the SEVIS record before the I-20 can be processed for Carnegie Mellon. Access the Transfer In Form on Coming to Carnegie Mellon from within the U.S. Student shall complete section A. Meet with the foreign student advisor/administrator at the school to determine a release date. The administrator shall complete Section B. Include the form when submitting the International Student Information Form packet. The I-20 for Carnegie Mellon will be processed on or shortly after the release date. Students who are currently attending a school in the U.S. in J-1 visa status should OIE for further instructions. SHIPMENT of I-20 or DS-2019 CERTIFICATE OF ELIGIBILITY The Office of International Education (OIE) at Carnegie Mellon University has partnered with University Express Mail Service (UEMS) for the shipment of immigration documents. This service is at your expense and will allow you to receive your I-20 or DS-2019 through DHL, FedEx or UPS within 3-5 days after the label has been processed. OIE will you when the I-20 or DS-2019 has been processed. The will include your SEVIS ID number as well as instructions on how to create a shipping label. Do NOT create the shipping label until you receive an from OIE instructing you to do so. You may also choose to use eshipglobal to send your International Student Information Form and required documentation TO Carnegie Mellon University. If you choose to do this, start by creating a student account, select Send documents to your University, Carnegie Mellon University and choose the Office of International Education. Be very careful when selecting the correct CMU office address to avoid any delays. To create a student account in eshipglobal, simply follow the instructions using this link: 1. Log in to eshipglobal do not go through DHL, FedEx or UPS directly. (The link works best with Internet Explorer and Mozilla Firefox browsers.) 2. You will be required to register a new student account using your and password. 3. Enter your complete mailing address including postal code, phone number and . (student ID is not required). 4. You will need a credit card (Visa, MasterCard or Discover only) to pay for the service. Pay close attention to all of the information submitted as errors will result in a delay of the processing and shipment of your immigration document. If you have questions about how to use this service, visit the FAQs section of eshipglobal or student.support@eshipglobal.com NOTE: Due to the high volume of packages received in OIE we are unable to confirm the receipt of your forms. 4 FUNDING INFORMATION Before OIE can issue your I-20 or DS-2019, we must receive proof of financial support for the first academic year of study at Carnegie Mellon as well as the potential for continued financial support for the remainder of your program. The minimum amount of funding you will provide must equal or exceed the expenses noted below. For support that will be provided for subsequent years, include financial documentation or attach a letter describing how the funding will be provided. 12 Month Minimum Expenses for Academic Year Tuition (refer to department information) $ Living and miscellaneous expenses $ 19,845 Books / Supplies / Fees $ 3,046 Health Insurance (individual) (estimate)* $ 1,979 TOTAL $ Dependent expenses** $ TOTAL plus dependent expenses $ * Estimate only based on costs for student health insurance. Actual cost may vary. ** If applicable, include $8,064 for spouse, $5,964 for first child and $4,200 for each additional child who will be with you in the U.S. Complete the following Source of Funds section and provide required supporting financial documents. Refer to the INSTRUCTIONS under Acceptable Sources of Funding or Foreign Students, Coming to Carnegie Mellon. SOURCE OF FUNDS Fill in the blanks that apply to your situation. 1) Personal Funds $ Submit an original bank statement or original bank letter showing available funds in your name. Educational loans in your name will be considered personal funds. 2) Family or Individual Sponsor $ Submit an original bank statement or bank letter PLUS the completed Affidavit of Support (See Page 4). 3) Employer or Private Organization $ Submit an original letter from sponsoring organization describing exact amount provided & period of funding. 4) Government Funding $ Submit an original letter from sponsoring organization describing exact amount provided & period of funding. 5) Carnegie Mellon Support $ Submit a photocopy of your Carnegie Mellon admission or award letter. TOTAL $ Total amount should meet or exceed the 12-month minimum expenses calculated above.
