1 2018 Hearthstone Builder Humanitarian Award NOMINATION FORM Thank you for entering the Hearthstone BUILDER Lifetime Public Service Award. Sponsored by BUILDER and Hearthstone, the award honors builders who have demonstrated a lifetime commitment to public service. ELIGIBILITY Nominees must be for-profit, single-family builders or lot developers. They may be individuals or companies, employees or owners. Builders may nominate themselves, be nominated by other builders, or be nominated by charities, nonprofit organizations, or government organizations they have helped. THE AWARD A cash award will be donated to the winning builder s nonprofit charity or community service organization. The winner and their charity will receive coverage in BUILDER magazine and online. SELECTION A panel of industry judges will select the winner that has made the most significant contribution to public service over their lifetime.
